L. Dixon, Administrator, assisted me with the visit. Your program currently operates with a notice of compliance issued 9/10/2010. The last annual compliance visit was conducted 11/20/25. The sanitation inspection was completed on 11/5/25 with a “Superior” classification. The last fire inspection was conducted on 8/19/25 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of 4/6/26. The NC Secretary of State website was reviewed on 4/6/26 and Unity Christian Academy of Haw Branch, Inc. was listed as current- active. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed during today’s visit. All information is now current. It is important to frequently check your email to remain informed about any new changes that have or may occur. The children were engaged in morning learning routines free play and snack. In space #1, the children were transitioning to outdoor free play. The children engaged in drawing on concrete pad with chalk, riding bicycles, swings, and free play. The outdoor space includes stationary and portable materials. In space #2, children were transitioning from group activity to snack. Snack is provided by the children’s families. The following violation(s) were documented. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. A surge protector located on the wall behind the refrigerator had five (5) uncovered outlets. 10A NCAC 09 .0604(c) 1301 Center did not maintain a record of daily attendance. Thirteen (13) children were present in space #2, attendance had not been documented for today. GS 110-91(9) Violations documented were corrected during today’s visit. No further action is required. Technical Assistance: Electrical Outlets- Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. For this reason, all electrical outlets, including those on power strips, not in use that are in areas accessible to children must be protected with a safety plug or a tamper-resistant electrical outlet. A surge protector located on the wall behind the refrigerator had five (5) uncovered outlets. You stated that it was new because of the refrigerator and did not realize that they needed to be covered. You added outlet covers during today’s visit. Add a note near the outlets to serve as a reminder to ensure that outlets are covered. Attendance: The health and safety of individual children requires that information regarding each child in care be kept and available when needed. Attendance records should accurately reflect the number of children in care at all times. Attendance records in each classroom should include each child that is enrolled in the space and documented as present or absent. Thirteen (13) children were present in space #2, attendance had not been documented for today. Staff stated that it was her last copy and she had not filled it out because she needed to make copies first. You filled out the attendance form during the visit today. Staff are to fill out attendance records upon arrival/departure of children. Reminders: • The EPR plan needs to be updated by July 2026. Additional comments: Sex Offender Registry: North Carolina General Statute 14-208 requires sex offenders to register with the North Carolina Department of Justice. The law states that a sex offender shall not knowingly reside within 1,000 feet of the property on which any public or nonpublic school or child care center is located. This does not apply to child care centers that are located on or within 1,000 feet of property of an institution of higher education where the registrant is a student or is employed. All licensed child care centers must register to receive e-mail notification when a registered sex offender moves within a one-mile radius of the center. Michelle O'Kelley, Child Care Consultant, accompanied me on today's visit. L. Dixon, Administrator, assisted me with today’s visit. Your program currently operates with a a Notice of Compliance issued 9/10/2010. The last annual compliance visit was conducted 1/4/24. The sanitation inspection was completed on 2/14/24 with a “Superior” classification. The last fire inspection was conducted 1/3/24. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety two percent as of 8/27/24. The NC Secretary of State website was reviewed on 8/27/24 and Unity Christian Academy of Haw Branch, Inc. was listed as current- active. We reviewed your facility contact information listed on the Division of Child Development and Early Education (DCDEE) website and all information is current. Children in space #1 were observed in the outdoor space climbing on stationary climbers and riding in coupe cars. In space #2, the children completed personal care routines before transitioning to the outdoor space. The outdoor space was observed to have stationary climbers, swings, riding toys, and balls for the children to engage in gross motor activities. There was also a large sand area with sand toys and a sand box area for the children to find fossils. The following violation(s) were documented. Violation Number Comment Rule 303 Children were not adequately supervised at all times. Upon arrival, space #1 was observed in the outdoor space. The two (2) staff members assigned to space #1 were observed sitting on a picnic table under the shade structure located directly outside. A child had gotten stuck for several minutes while attempting to climb the rock wall structure and was unable to climb down until I assisted the child off the structure. The teachers were unaware that the child was stuck until it was brought to their attention. .1801(a)(1-5) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 9/19/24, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. They often do this on playground equipment. Playgrounds have a higher risk of injury than indoor play areas. Even if playgrounds meet the highest safety standards, serious injuries can happen if children are unsupervised. Adults who pay attention and are aware of children’s behavior are in the best position to keep them safe. Appropriate supervision of children is important for their safety, injury prevention, and making sure of quality child care. Program staff must be able to see and hear the children to be ready to help in an emergency. With proper supervision, program staff can quickly remove children from any potential harm. Upon arrival, space #1 was observed in the outdoor space. The two (2) staff members assigned to space #1 were observed sitting on a picnic table under the shade structure located directly outside. A child had gotten stuck for several minutes while attempting to climb the rock wall structure and was unable to climb down until I went and assisted him off the structure. After assisting the child off the structure, I discussed with the staff members that while on the playground, it is important to be actively supervising the children by walking around and one staff member always placed near the climbing structures and swings so that they can render immediate assistance. One of the staff members said that she should have been near the climbing structure and will stay near it. Staff members need to be positioned in the outdoor space so that they are close enough to the children to be able to give assistance immediately. A follow up visit will be conducted. They can also be very dangerous if the wrong type or wrong amount is given to the wrong person or at the wrong time. A medical action plan is required for children that need emergency medication, the form needs to be completed by the parent or a health care professional. In space #1, a child enrolled did not have a medical action plan on file for anaphylaxis. We discussed the importance of understanding the symptoms the child shows and the instructions to administer the Epi-Pen that is on site for the child. In space #1, a child enrolled had an EpiPen on file that expired in June 2023. We discussed that it would be helpful to notate on the allergy list the expiration date of the medication and written permission form for medication as a visual that is looked at frequently. In space #1, a child enrolled had an EpiPen and Albuterol inhaler on file without written permission from the parent. You stated that you had sent a permission from home but did not receive it back from the parent. In space #1, a child enrolled had an Albuterol inhaler on file that was not stored in the original pharmacy labeled container. We discussed that all medication needs to be stored in the original pharmacy container. It would be helpful for one person to be designated to receive medication to ensure that the medication is current, and all forms are completed correctly. I emailed you a copy of a medication flyer to review when receiving medication to know how long the written authorization will be valid. You printed off the medical action plan for anaphylaxis and permission forms for the EpiPen and Albuterol inhaler during the visit and you called the parent to inform them that current medication is needed and the forms that need to be completed. We discussed that it would be helpful to add checking medications to the monthly checklist that you are creating. Emergency Drills: You must conduct either a shelter-in-place or a lockdown drill every three months to ensure that the operator and children are aware of what actions to take in the event of a life-threatening emergency. An emergency drill was not completed in the fourth quarter of 2023. We discussed that it would be helpful to review the emergency drill form every month when conducting a fire drill. You stated that you are creating a monthly checklist and that emergency drills will be added to ensure that one is completed quarterly. Consider marking your calendar as a reminder of when drills are due. Additional Comments: NITTO is free with 14 contact credit hours earned upon completion. It is located on DCDEE’s Moodle website. Your program currently operates with a notice of compliance, issued 9/10/10. The last annual compliance visit was conducted 1/24/23. The sanitation inspection was completed on 5/9/23 with a “Superior” classification. The last fire inspection was conducted 8/7/23 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine percent as of 8/26/23. The NC Secretary of State website was reviewed on 8/28/23 and Haw Branch Church, of Christ was listed as current- active. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed during today’s visit. All information is now current. It is important to frequently check your email to remain informed about any new changes that have or may occur. The children were engaged in morning learning routines, singing, movement, story time and indoor free play. The outdoor space includes stationary and portable materials. All meals are provided by the children’s families. The following violation(s) were documented and immediately corrected. An annual fire inspection ensures that your facility has the right safety equipment in place and is in compliance with the applicable fire code for your building. Electric Fans - Operating a childcare facility requires careful planning and proactive and protective measures to ensure the safety of the children in your care. An electric fan was stored on the floor near the teacher’s desk in Space 1. You indicated the fan is used during rest time as white noise. Safe spaces, indoors and outdoors, help children to feel free to explore and experiment which, in turn, fosters development in all areas. Fans that are not out of the reach of children must be mounted securely to prevent the fan from falling on a child. Fans must have a mesh wire guard which prevents children’s fingers, toys, or other small objects from meeting the fan blades. You moved the fan and indicated it would not be used until it can be mounted. Aerosols - There are over two million human poison exposures reported to poison centers every year. Children under six years of age account for over half of those potential poisonings. An aerosol can of Aileene’s Original Gloss Finish Acrylic Sealer was observed in a basket on top of a cabinet in Space 2. All aerosol cans are highly flammable and present a hazard because of the compressed gas used as a propellant. If punctured, the contents may be released so forcefully that injuries can result. Because of this all aerosols must be kept in locked storage. Locked storage rooms and cabinets include those which are unlocked with a combination, electronic or magnetic device, key, keypad, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and must not be stored in the lock. Recognizing & Responding to Suspicions of Child Maltreatment – Childcare staff are mandatory reporters of child abuse and neglect. Education is important in identifying manifestations of child maltreatment that can increase the likelihood of appropriate reports to child protection and law enforcement agencies.
