Your program currently operates with a three-star license, issued 8/4/23, earning 4 points in the education component, 2 points in the program standards component and 1 quality point for serving no more than 2 children under one year of age. The last annual compliance visit was conducted 6/17/24. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed during today’s visit and all information is current. It is important to frequently check your email to remain informed about any new changes that have or may occur. The family childcare home’s compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of 5/6/25. The NC Secretary of State website was reviewed on 5/6/25 and this facility is not currently owned or operated by a corporation. Please contact me prior to any changes regarding ownership of this facility. Five (5) children were present. The children were engaged in free play upon arrival. Learning materials and activity area supplies were kept on low-lying shelves easily accessible to the children. The children were engaged in building blocks, looking at books, and playing dress-up. The outdoor area included ride-on toys, balls, swings, and stationary equipment. Proper handwashing was observed. Lunch was being prepared and consisted of chicken tenders, green beans, bananas, whole grain bread, and milk. The following violations were documented. Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. Located in an unlocked outside storage building was a container of Trimoc lawn and weed killer, a container of hot shot home insect control, and a container of roundup. .1719 (a) (1) 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. Foxy and Patches the cats did not have documentation on file and available for review. .1719(b)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 5/21/25, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Documentation of a current rabies vaccination must be on file for any animal in the home. Foxy and Patches the cats did not have documentation on file and available for review. You stated you had not realized it was time for their shots to be updated. Documentation of the vaccination must be kept on file and available for review. Storage of Hazardous Items: Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Located in an unlocked outside storage building was a container of Trimoc lawn and weed killer, a container of hot shot home insect control, and a container of roundup. You stated your husband had been in there and must of forgotten to lock the door. You locked the building during the visit. When you perform your daily playground check, check to ensure all storage buildings are locked. Comments: Be knowledgeable of the child care laws and rules and environmental health rules; review them with your staff and assist them with understanding and implementing them to maintain compliance and keep children safe and healthy. Your program currently operates with a two-star license, issued 9/7/22, earning 1 point in the education component, 2 points in the program standards component and 1 quality point for serving no more than 2 children under one year of age. Your contact information on the Division of Child Development & Early Education (DCDEE) website was reviewed during today’s visit and an update was made to the email address listed on the owner screen. It is important to frequently check your email to remain informed about any new changes that have or may occur. The last annual compliance visit was conducted 8/9/22. The family childcare home’s compliance history was reviewed with the operator. The program’s compliance history was ninety seven percent as of 7/11/23. A walk-through of the family child care home was completed today, all indoor and outdoor areas were monitored. Three (3) children and the provider were present when I arrived. Personal care routines were observed. The children participated in free play in activity areas and transitions. Learning materials and activity area supplies were kept on low-lying shelves easily accessible to the children. The outdoor play area in the back yard provided additional space and materials to promote physical development and gross motor skills. Lunch included fish sticks, bananas, green beans, whole grain bread, and milk. The following violation was documented. A riding lawn mower was stored behind a building beside the unfenced outdoor play area. .1719 (a) (1) 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Cleaning products were stored in the unlocked laundry room across from the bathroom used by the children. .1719 (a)(7) 908 Health questionnaire was not completed annually. The last completed annual health questionnaire on file for the provider was dated 1/3/22. .1703(a)(1) 1895 Application did not include health care needs or concerns, symptoms of and the type of response required for the health care needs or concerns. Cleaning products were stored in the unlocked laundry room across from the bathroom used by the children. All cleaning products must be kept in locked storage. Additionally, a riding lawn mower was stored behind a building beside the unfenced outdoor play area. Lawn mowers must be kept in locked storage. Locked storage rooms and cabinets include those which are unlocked with a combination, electronic or magnetic device, key, or equivalent locking device. These unlocking devices must be kept out of the reach of a child and must not be stored in the lock. Children’s Application – (Health Care Needs) Emergency information is the key to obtaining needed care in emergency situations. In the event of an emergency, child care providers must know about a child’s special health care needs, so they can offer protection for that child. This requires you to obtain the health and safety needs of individual children prior to enrollment. The health care needs section of the enrollment application for 2 out of the 6 children enrolled was blank. You indicated you had audited your children’s files and somehow missed it. You also indicated you were going to complete another audit of your files once the children were down for rest time to ensure the parents completed that section. Once parents complete the enrollment application, you must review it to ensure the information captures what is required before you sign off. Health Questionnaire - An emergency is a situation that poses an immediate risk to health, life, property, or environment. Most emergencies require urgent intervention to prevent a worsening of the situation. You have not completed the annual update of your health questionnaire. It was last updated on 1/3/22. The health questionnaire form attests to the individual’s ability to provide care and provides potentially lifesaving information to medical professionals if you are unable to speak for yourself. This form must be completed by day one of hire and updated as changes occur or at least annually.
