Your program currently operates a Four Star Rated License with an effective date of November 19, 2019. Group Leaders, S. Porter, and K. Talley, assisted me with the visit. The Program Coordinator is currently on leave. The facility operates before school as well and has a total of forty-two children enrolled. The media center and art room are used as primary spaces for the children. However, the children report to the cafeteria upon dismissal of school. Activity areas were set up in the media center and art room. Activity areas included science, math, homework activities, writing, art, and global world culture. Today, children were observed participating in arrival procedures, transitions and snack. Snack consisted of baked Doritos and 100% apple juice. Staff were observed supervising and interacting with the children in a caring and nurturing manner. The following items were monitored: supervision, staff/child ratio, CPR, First Aid, BSAC training, criminal background qualifying letters, EMC plan, administering medication, storage of hazardous products/medications, general safety, discipline, adequate/approved space, program records, license posted and permit restrictions. The Staff and Training Worksheet was received today. There has been one new employee hired since the last annual compliance visit conducted on May 14, 2025. The file for the new employee was monitored. A substitute was also present today. Required information for the substitute was on file. The Staff and Training Worksheet was used to verify existing staff were current with First Aid, CPR, and criminal background checks and had completed BSAC training. The last fire drill was conducted on December 18, 2025. A lockdown drill was conducted on October 6, 2025. There were three violations observed today. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One new staff member had a First Aid certificate on file however, the training was not completed by an approved organization. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One new staff member had a CPR certificate on file however, the training was not completed by an approved organization. .1102(d) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented shelter-in-place or lockdown drill was conducted October 6, 2025. .0604(u);.0302(d)(8) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before January 27, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. He stated he completed training with another employer prior to his current position. I explained to him that certifications for First and CPR must be taken with an approved organization and that he would be able to register for the training through the school system. It was explained to the Group Leaders that a shelter-in-place or lockdown drill must be conducted every three months. S. Porter stated a drill would be conducted this week. DCDEE has added a new training module about Child Development. The training will provide a basic overview about child development and provide information about resources and professional agencies in North Carolina. DCDEE has also added another new training about Child Care Rule Rollout which includes new rules effective July 1, 2025. Both training modules can be found on the DCDEE Moodle Learning Platform at https://www.dcdee.moodle.nc.gov/. In-service training hours are received upon completion of the training sessions. A checklist was used to monitor your facility and was provided for your records at the end of the visit. The current 4 star rated license was issued in 2019. The current 18 month compliance history prior to today's visit was 94%. I met with Phyllis Pauling, Site Coordinator, upon my arrival. I arrived before the school age children were dismissed to the program. We were able to talk and you provided me with the licensing notebook you have organized with records. I reviewed children, staff, and program records while you and two group leaders set up centers in the media center and Art Room, both approved spaces. The current Public School Off Site Verification Forms for Staff Records, Children Records and Transportation were current and on file. You completed Recognizing and Responding to Suspicions or Maltreatment last in March 2019 and should have completed it again by March 2024, as it is part of required health and safety trainings you must complete within 5 years of last completion while you remain employed with CMS ASEP. All other information was observed meeting compliance. The last fire inspection was conducted on 8/21/23. The last sanitation inspection was conducted on 11/7/23. Information required to be posted was observed posted on the ASEP bulletin board outside the ASEP office. The emergency drill log, incident log and monthly playground inspections were reviewed and found meeting compliance. Each group set up interest centers for children to utilize, group 2 has center time in the media room, group 1 has center play in the Art room. The current activity plans and daily schedule were posted on the ASEP board and found meeting compliance. Material was observed in good repair and developmentally appropriate for the ages served. I reviewed daily attendance where you log in arrival, parents sign children out at the door, you ensure they document the time they leave. For outdoor gross motor space you use used the field and stationary equipment outside the cafeteria during today's visit. One staff checked the playgrounds prior to children's arrival. While outdoors children played on the stationary equipment and adjoining field. One staff was monitoring the stationary equipment area while the other monitored play on the fields. I observed 7 pegs protruding up on the boarder wall holding in the mulch around the stationary equipment. They were sticking up enough to cause tripping hazards. I observed children's arrival to the program, hand washing, outdoor time, and snack. Supervision and enhanced staff/child ratios were observed being maintained today. Nurturing tones were heard when staff greeted children at arrival and throughout the observation. The current menu was reviewed and found meeting compliance, snack served today followed the current menu. You reported currently you do not have any children required to have medication administered. No violations were cited during today's visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Indoors were found meeting compliance. Outdoors I observed 7 pegs on the boarders surrounding the mulch for fall zone of the stationary equipment were observed sticking out some causing a tripping hazard and protrusion. 10A NCAC 09 .0601(a) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. One staff did not complete Recognizing and Responding of Suspicions of Maltreatment within 5 years of last completion. She completed the training last on 3/19/19. .1103(b) Compliance Statement: Childcare providers are expected to maintain compliance with all applicable child care requirements at all times. All violations cited today shall be corrected immediately. Phyllis Pauling, program coordinator, will email a compliance letter explaining how each violation cited today has been corrected. The compliance letter must be received by the consultant, andrea.anderson@dhhs.nc.gov, on or before June 10, 2024 due to the school year ended and she taking the summer off. Failure to correct the violations and/or submit the compliance letter to me by the date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Technical Assistance was provided on the following: Safe Environment Outdoors: I encourage you to talk to your custodial staff and principal, to see if they can assist you in maintaining compliance with the boarder wall. As the ground settles and children play on in the area the pegs holding the boarders down start pulling up. When you see this happening you need to hammer them back down. Make sure you are checking them daily, weekly and monthly as you check the playground for use to ensure compliance is maintained. Health and Safety Trainings: I shared the DCDEE Health and Safety Training log, as I think it is easier to use then the one CMS ASEP provides you each year. All the topic areas are listed on the log that are required to be completed the 1st year of employment and within every 5 years you remain employed with CMS ASEP. You didn't realize Recognizing and Responding to Suspicions of Maltreatment was one of the topic you needed to complete at least every 5 years. I reminded you that you take that training from NC Prevent Child Abuse website. https://www.preventchildabusenc.org/ Rated License Reassessment: The current 4 Star rated license was issued 11-19-19. The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024. Reassessment for your facility will be during Cohort 1. Cohort 2: Planning year runs from July 1, 2024 to June 30, 2025. Reassessment year runs from July 1, 2025 to June 30, 2026. If interested in having the SACERS-U conducted as part of the reassessment and to maintain at least 4 points in program standards can request one during the planning year as a practice run. If you score above a 5.0 it can be used during the reassessment year. If you score below a 5.0 during the planning year, you can decide if you want to complete the assessment during the reassessment year at no charge and that score will be used to determine the program points of the rated license.
