Name of Operation: University Park Creative Arts ASEP Facility ID: 60004415 Consultant: RASHEKA WHEELER Operation Type: Center Case Number: Visit Date: 3/5/2026 Number Present: 7 Completed Date: 3/5/2026 Age: From 7 To 9 Total Minutes: 65 Time In: 03:25 PM Time Out: 04:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of my unannounced visit was to monitor for compliance with applicable childcare requirements for a third temporary time period visit. Michelle Cuthbertson, Child Care Consultant, accompanied me on today’s visit. Upon arrival, after signing the portal for visitors and asked the office staff if she could contact Myra Conner, the assigned Program Coordinator for this location. We met with Ms. Conner and conducted the walk-through of the facility to verify approved spaces and monitored center files/program files. Currently this center operates with a Temporary License, issued on September 19, 2025. The restrictions include Daytime Care only. The facility has a capacity of 150 students ages 5-12 years old. The temporary time period expires on March 19, 2026. Indoor Learning Environment: Upon arrival, staff/child ratio was observed and was in compliance. There is one (1) group of children operating during today’s observation. The group was observed transitioning from the public school to the afterschool enrichment program. Children observed preparing to transition to the bathrooms for use. We spoke to the Program Coordinator about making sure they use approved bathrooms throughout the assigned license spaces. Materials were observed placed throughout the tables in the cafeteria. We spoke about adding additional materials for each category. There were (7)seven children present today and a total of 18 enrolled. Outdoor Learning Environment: Was briefly viewed during today's visit. Children did not use the OLE during the visit. CMS Public Schools participate in Offsite records for Transportation, Staff Records, and Children’s Records. Records: Staff: No new staff was hired since my last visit February 6, 2026. Program: We reviewed all the required records. The fire drill report was completed monthly, and the last documented drill was 2/23/26. Nutrition: Your facility was in compliance during today’s visit with Child Care Meal Patterns Requirements. The snack menu was reviewed, and it met the meal pattern requirements. Apple Cinnamon cheerios and milk. The menu posted did not match what was served to children. Weapons: Your facility reported that they were in compliance during today’s visit with Child Care Requirements regarding firearms. One (1) violation was cited and corrected during today's visit. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The snack served did not match the menu that was currently posted. 10A NCAC 09 .0901(b) Compliance History: North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Rated License/QRIS Pathway #1-Program Assessment for Child Care Centers: The facility SACERS-U assessment was completed on 2/16/2026. A score of 4.8 was received. The Program Coordinator was given an opportunity to discuss improvements in all areas that were scored lower than 4.0 on the SACERS-U assessment. We indicated that the items deemed a violation of child care violations would be monitored during the next unannounced visit to the facility. We highly recommend adding additional materials do the learning areas to enhance the activities needed to provide a quality 4-star rating environment at all times. Family and Community Engagement practices and 3 additional choices were selected; the documentation was received: Communication Option: C-1: Two-way communication with families, email, telephone text, teacher workday documentation. Engagement and Leadership: EL:1 The child care center has a structured, on-going process to receive and review suggestions and recommendations from families. The Program Coordinator stated that they have continuous conversations with families and ask how additional support can be given to enrolled children. Engagement and Leadership: EL-6 The Center administrator offers an annual evaluation or survey of the center to families, including the option to reply anonymously. Education Opportunities: EO-6-The center administrator provides paid planning time for lead teachers (group leaders) to prepare information for family and teacher conferences, time to conduct the conferences and support needed to conduct conferences by telephone, virtually or in-person. The facility’s CQI Plan was received and reviewed with the assigned Program Coordinator during today’s visit. Documentation was received verifying what was already completed. The program will have a year to make any changes or satisfy the goals outlined in the signed CQI Plan for the facility. I also mentioned that each individual CQI plan should be monitored, and documentation of progress should be maintained on file for the assigned child care consultant to verify during the program’s annual compliance visit. Education worksheets were completed during the visit. One (1) Administrator-5 Stars One (1) Program Coordinator -3 Stars One (1) Group Leader-4 Stars According to the information received during today's visit, the program may have enough information for a 4 Star Rating.
