I was greeted at the door by the administrator, I stated the purpose of my visit and I proceeded with the monitoring visit with Dawn Stoeckel, Director/Owner. This facility currently operates with a four (4) star rated license effective 06/28/2018 with 1st shift (Daytime care) and meets enhanced ratios. The NC Secretary of State website was viewed, and The Butterfly Garden Child Development was current/active. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. Indoor Learning Environment: Upon arrival staff/child ratio was observed and was in compliance. Children were adequately supervised during today’s visit. The children were engaged in circle time/ music and movement. Children also played in activity centers playground, transitioning from playing to handwashing. Children were served turkey sausage, pasta noodles, green beans, pears, and milk. Outdoor Learning Environment: Your playground is separated into two (2) different sections. The following items were observed in the outdoor learning environment: balls, riding toys, blocks, swing sets, a hoop for throwing balls, push toys and dramatic play kitchen. The outdoor environments had several natural elements of bamboo, herbs, tree stumps as well as other natural elements mixed with purchased materials. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. Daily schedules, activity plans, staff/child ratio worksheets, arrival/departure procedures, summary of law and menus. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. INSPECTIONS: The last sanitation inspection was conducted 06/24/25. The last fire inspection was conducted on 01/30/25. The last documented fire drill was conducted 09/28/25. The last documented shelter-in-place or lockdown drill was conducted 9/30/25. The last documented playground inspection was conducted 10/18/25. Last compliance history was 86% Violation Number Comment Rule 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. There was no evidence of a thermometer in the infant room refrigerator. 15A NCAC 18A .2806(j)(2) 847 Parent's medication authorization did not include required information. Date and amount was missing. 10A NCAC 09 .0803(4)(6-9) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of childcare requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Childcare consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. REQUIRED RESPONSE: (for any violations) You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address below by November 06, 2025. The format to follow is: -Name of your facility, -ID number, -date of the visit, -date you are submitting the letter, -citing each violation number, -when AND how it was corrected AND how you will prevent the reoccurrence of this violation, -Close with YOUR SIGNATURE REQUIRED RESPONSE: If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. I was greeted at the door by the assistant administrator, I stated the purpose of my visit and contacted the administrator/owner. I proceeded with the monitoring visit and Dawn Stoeckel, Director/Owner, assisted with the rest of the visit when she arrived. This facility currently operates with a four (4) star rated license effective 06/28/2018 with 1st shift (Daytime care) and meets enhanced ratios. Indoor Learning Environment: Upon arrival staff/child ratio was observed and was in compliance. Children were adequately supervised during today’s visit. The children were engaged playing in activity centers playground, transitioning from outdoor to indoor handwashing. Children were dressed up in costumes today. Children were served, tortilla, turkey, cheese, corn, oranges, and milk for lunch. During the visit the afterschool children arrived and went outside to play. There were a total of twelve children. Outdoor Learning Environment: Your playground is separated into two (2) different sections. The following items were observed in the outdoor learning environment: balls, riding toys, blocks, swing sets, a hoop for throwing balls, push toys and dramatic play kitchen. Children were actively playing on the toddler and preschoolers’ playground. Children has access to a large slide inflatable. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. Daily schedules, activity plans, staff/child ratio worksheets, arrival/departure procedures, summary of law and menus. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. INSPECTIONS: The last sanitation inspection was conducted 06/18/24 The last fire inspection was conducted on 0 /27/24. The last documented fire drill was conducted 09/27/24. The last documented shelter-in-place or lockdown drill was conducted 8/27/24. The last documented playground inspection was conducted 10/18/24. Last compliance history was 86% During this visit the consultant had printing issues and the report was emailed to the provider to make copies of the report. Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Violation Number Comment Rule 714 Openings in equipment, steps, decks, handrails, and fencing were not less than 3 1/2 inches or greater than 9 inches. The names listed were old employee making the list invalid. .0802(a)(1)(A-B); 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Dawn Stoeckel, owner, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Stoeckel and one additional staff member. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. A staff member constantly turned her back to one and two year old children while they were eating lunch. As a result, the staff member was unaware a one year old child had a mouth full of food when the staff member laid the child down to change the child’s diaper. .1801(a)(1-5) 404 All staff did not wash their hands thoroughly after diapering each child. On July 18, 2024, one three occasions, a staff member did not wash their hands after changing diapers of children. 15A NCAC 18A .2803(a) 405 A child's hands were not washed after each diaper change. On July 18, 2024, a staff member did not wash the hands of three children after their diapers were changed. 15A NCAC 18A .2803(c)(2) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. On July, 18, 2024, staff left mop water in the classroom accessible to children for an extended period of time. .2820(b) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member constantly used an harsh tone when telling one and two year old children to "sit" and "no". G.S. 110-91(10) 904 Child was handled roughly. A staff member picked a one year old child and aggressively sat the child in a chair during lunch time. In addition the same staff member guided a two year old child around a table abruptly by the child's forearm causing the child to loose their footing and bump their head against a chair. .1803(a)(1) Violations must be corrected immediately. I proceeded with the monitoring visit and Dawn Stoeckel, Director/Owner, assisted with the rest of the visit when she arrived. The NC Secretary of State website was viewed, and The Butterfly Garden Child Development was current/active. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. This facility currently operates with a four (4) star rated license effective 06/28/2018 with 1st shift (Daytime care) and meets enhanced ratios. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. Daily schedules, activity plans, staff/child ratio worksheets, arrival/departure procedures, summary of law and menus. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. INSPECTIONS: The last sanitation inspection was conducted 5/5/23 The last documented fire drill was conducted 10/31/23. The last documented shelter-in-place or lockdown drill was conducted 8/30/23. The last documented playground inspection was conducted 10/25/23. The last documented compliance prior to today’s visit was Learning Environments: Upon arrival staff/child ratio was observed and was in compliance. Children were adequately supervised during today’s visit. The children were engaged playing in activity centers playground, transitioning from outdoor to indoor handwashing. Children in space three were observed eating cereal with milk and playing in activity centers. Diapering was monitored today in the toddler class. Outdoor play was observed, and children had access to balls, hoops, climbing structure and swings. Children were served fish patty, mixed fruit, French fries, and milk for lunch.
