The last Annual Compliance visit was made on 2/18/2025. The 1 Star Rated License was issued on 6/7/12. I was let into the facility by operator Texann Lightner. She was present with four (4) children. The April 2025 version of the Family Child Care Home Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 92% as of today. Children was observed having free play and routine needs met. There were toys and activities available. Attendance was monitored. Summary of law, schedule, summary of law, first aid sheet, and emergency numbers were posted. Monthly Fire Drills, Emergency Preparedness Drills, and Playground inspections were monitored. Children's files were monitored for Application, Emergency Care Authorization, Medical/Immunization, Discipline Statement, Summary of Law, No smoking statement, Shaken Baby Syndrome policy, and Outside fence permission. The provider's file was monitored for Criminal Qualifying Letters, First Aid and CPR coverage, Health and Safety Trainings, Shaken Baby Syndrome Policy, Special Trainings, On-going Training hours, and Annual Health Questionnaire. This facility does not provide transportation. Allergies and storage of medication was discussed. The outdoor, bathroom and kitchen areas were monitored. The following violations were cited during today's visit: Violation Number Comment Rule 1301 Operator did not complete the required number of on-going training hours as specified in rule. "Adequate supervision" shall mean that: (1) For pre-school age children, the family child care home operator shall be positioned in the indoor and outdoor environment to maximize his or her ability to hear and see the children at all times and render immediate assistance; (2) For school-age children, the operator shall be positioned in the indoor and outdoor environment to maximize his or her ability to hear or see the children at all times and render immediate assistance; (3) The operator shall interact with the children while moving about the indoor or outdoor area; and (4) For children of all ages: (A) the operator shall know where each child is located and be aware of children's activities at all times; (B) the operator shall provide supervision according to the individual age, needs, and capabilities of each child; (C) all of the conditions in this Paragraph shall apply except when emergencies necessitate that adequate supervision is impossible. Written documentation of emergencies stating the date, time, and reason shall be maintained and available for review by Division representatives upon request; and (D) for children who are sleeping or napping, the operator shall not be required to visually supervise them, but shall be able to hear and respond without delay to them. Children shall not sleep or nap in a room with a closed door between the children and the operator. The operator shall be on the same level of the home where children are sleeping or napping. (5) The opportunity each day for each child under the age of 12 months for supervised play while awake and alert while positioned on his or her stomach; (6) Developmentally appropriate activities as planned on a written schedule and activity plan. Materials or equipment shall be available indoors and outdoors to support the activities listed on the written schedule and activity plan; (7) A written schedule that shall: (A) Show blocks of time assigned to types of activities and include periods of time for both active play and quiet play or rest; (B) Show times and activities that are developmentally appropriate for the ages of children in care; (C) Reflect daily opportunities for both free choice and guided activities; (D) Include a minimum of one hour of outdoor play throughout the day, if weather conditions permit; (E) Include a daily gross motor activity that may occur indoors or outdoors; and (F) For children under two years old, interspersed among the daily events shall be individualized caregiving routines such as eating, napping, and toileting (8) A written activity plan that shall: (A) Include activities intended to stimulate the following developmental domains, in accordance with North Carolina Foundations for Early Learning and Development, available on the Division's website at https://www.ncchildcare.ncdhhs.gov//pdf_forms/nc_foundations.pdf: (i) emotional and social development; (ii) health and physical development; (iii) approaches to play and learning; (iv) language development and communication; and (iv) cognitive development. Supervision, Staff Child Ratio, Approved Space, License Posted, and Permit Restrictions were monitored today. 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. Recognizing and Responding to Child Abuse, Prevention of Infectious Diseases, handling and storage of hazardous materials, Prevention of Shaken Baby Syndrome need to be completed. This violation was documented as corrected during todays visit. 2056 Documentation of the operator's Emergency Preparedness and Response plan was not completed and/or maintained on the template provided by the Division of Emergency Management. The EPR Plan was not available. This violation has not been corrected. Sign in/out and safe sleep checks were monitored today. Children were observed having rest time. The following violations were cited during today's visit: Violation Number Comment Rule 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Moodle, pro solutions, or any approved agency that provides training can be used to complete the required training topics. Your EPR Plan has to be updated annually. Once updated within the portal, print page 28 as documentation of the date the annual update was completed. If you do not access the portal annually, your plan and account could be deleted. The last Annual Compliance visit was made on 3/23/2023. The 1 Star Rated License was issued on 6/07/2012. I was let into the facility by operator Texann Lightner. She was present with five (5) children. The August 2023 version of the Family Child Care Home Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 94% as of today. Children were observed napping, having routine needs met, and a teacher directed art activity. Attendance was monitored. Summary of law, first aid sheet, and emergency numbers were posted. Monthly Fire Drills, Emergency Preparedness Drills, and Playground inspections were monitored. A sanitation inspection was completed in 09/19/2023. Children's files were monitored for Application, Emergency Care Authorization, Medical/Immunization, Discipline Statement, Summary of Law, No smoking statement, Shaken Baby Syndrome policy, and Outside fence permission (if applicable). The provider's file was monitored for Criminal Qualifying Letters, First Aid and CPR coverage, Health and Safety Trainings, Shaken Baby Syndrome Policy, Special Trainings, On-going Training hours, and Annual Health Questionnaire. This facility does not provide routine transportation. There are no children enrolled with medication. The outdoor play area was monitored. The bathroom and kitchen area was monitored. The following violations were cited during today's visit: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. Lysol in an aerosol dispenser was not kept in locked storage when children were in care. .1719 (a)(7) 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Children were observed having free play and routine needs met. The following was observed posted: Current license, Summary of Law, and Emergency Care Plan. Monthly playground inspections and fire drills were monitored. Emergency Preparedness Drills were monitored. One (1) staff file was monitored for Criminal Record Checks, Emergency Info/Health Questionnaire, First Aid/CPR training, and special trainings. One (1) newly enrolled child's file was monitored. The following violations were cited during todays visit: Violation Number Comment Rule 1805 Copy of the safe sleep policy was not given and explained to the parent/guardian of each infant on or before the first day the infant attended the home and a signed statement from each parent/guardian acknowledging the receipt and explanation of the policy was not on file.
