The facility currently operates with a four-star license, issued January 13, 2023 earning 7 points in the education component, 2 points in the program standards component and 1 quality point by offering having completed the 30-hour business course. The last annual compliance visit was conducted April 23, 2025. The compliance history before today’s visit was 99%. Upon arrival, I was greeted by the provider, Sherri Wellman. The children were resting. Ms. Wellman provided notebooks with paperwork and documentation needed for me to review. I reviewed six (6) children’s files were monitored and found in compliance. Program records were reviewed. The last fire drill was conducted March 6, 2026 . A shelter-in-place drill was conducted on January 13 , 2026. Posted items were monitored and found in compliance. I observed children the indoor learning environment and found supervision and capacity to be in compliance. Children were observed participating in transition from nap, floor play, blocks, and interacting with the provider. The provider was observed interacting with the children in a nurturing and caring manner. Infant records including safe sleep check were reviewed and found in compliance. Feeding schedules were reviewed and one did not have a parent signature. A violation was cited. The provider has current First Aid, CPR, and ITS-SIDS training. Health and Safety and On-Going training was monitored and found in compliance The provider lives in the home with one household member. Both have current criminal background qualifying letters on file. ABCMS was monitored and found in compliance. You currently have no children requiring medications enrolled. Diaper creams were monitored and found in compliance. General safety and storage of hazardous materials was monitored and found in compliance. There are two (2) dogs in the home. The pet acknowledgment form is on file. All pets are current with rabies. The following violation was cited today: Violation Number Comment Rule 1990 Infant feeding plans did not include the required information, including but not limited to the type and amount of milk, formula and food, and the frequency of feedings, and/or was not available for reference. One (1) child enrolled born 5/19/2025 had a feeding schedule on file without a parent signature and date. .1706(i) Compliance Statement: Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Your program currently operates a Four Star Rated License with an effective date of January 13, 2023. The program’s 18-month compliance history before today’s visit is 100%. A Routine Unannounced Visit was conducted May 22, 2025. Upon arrival, I was greeted by the provider, Sherri Wellman. I observed the children in the indoor learning environment and found ratio and supervision to be in compliance. Children were observed participating in personal care routines, literacy and free choice of indoor learning activities. Caring and nurturing tones were used and the children were engaged with the caregiver throughout the visit. The following items were monitored during today’s visit, supervision, staff/child ratio, first aid, CPR, special training, ITS-SIDS training, criminal background checks, emergency medical care plan, administering of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions. The last shelter in place was conducted July 8, 2025. A violation was cited. The license was posted, and the restrictions were in compliance. The verification form of required information from the last annual compliance visit conducted on April 23, 2025, was used to confirm FA/CPR, ITS-SIDS training and criminal background qualifying letters. ABCMS was reviewed November 3, 2025, and found in compliance. The following violation was cited today: Violation Number Comment Rule 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. We discussed that you would make the decision on or before your Annual Compliance Visit and I will begin processing the new license packet at that time. I left a copy of the Pathway 1 chart and explained how to reach the three (3) star level. We discussed you will review all components and reconsider completing the scales once you review the webpage and think more about the requirements. NC Rated License Assessment Project (NCRLAP) I shared the NCRLAP website training information with you. There are training and resources available at https://www.ncrlap.org/ for you to prepare for the FCCERS-3 if you choose to complete the assessments to apply for your license. Additionally, the Self-Study information can be accessed here. MOODLE Trainings DCDEE has added a new training module about Child Development. The training will provide a basic overview about child development and provide information about resources and professional agencies in North Carolina. DCDEE has also added another new training about Child Care Rule Rollout which includes new rules effective July 1, 2025. Both training modules can be found on the DCDEE Moodle Learning Platform at https://www.dcdee.moodle.nc.gov/. In-service training hours are received upon completion of the training sessions. Three childrens files were monitored today. The provider has current First Aid, CPR, and ITS-SIDS training. The provider lives in the home with one household member. Both have current criminal background qualifying letters on file. The following violations were documented. Verification was not on file that a outdoor safety check was conducted in April 2024. 10A NCAC .1721(e)(5)(A-F) 905 Record of monthly fire drills, giving the date, time of day and length of time to evacuate, and operator's signature, were not available. Verification was not on file that a fire drill was conducted for the month of April 2024. .1721 (e )(2) 1403 Prescription or over-the-counter medication and topical, non-medical ointment, repellant, lotion, cream, fluoridated toothpaste, or powder was administered to any child without the required information.
