The facility currently has a Four- Star rated license, the 18-month compliance history score is 93%. The facility license and Summary of the Law was prominently posted. Today I was greeted by Tara Ellis, Owner. Mrs. Ellis assisted me during today’s visit and the visit summary was shared with her. Permit information-the program operates with a Four-star license, issued February 1, 2023 earning four (4) points for education, five (5) points for program, and one (1) quality point by meeting one programmatic option. License restriction includes: First and Second Shift; Maximum of 5 preschool children at any time; Children in care on ground level only; Children may not be in the garage area. Program is still located at 317 Peachtree Road, Charlotte, NC 28216. The phone number 347-439-5339 The FCCH is owned by a private owner. There were 4 (four) children in attendance today. There are six (6) children enrolled for first shift and there are three children enrolled for second shift. The children present were observed during naptime. The meal today met the childcare nutritional requirements. The facility is in compliance with the firearm safety requirements. Mrs. Ellis First Aid and CPR expires on 9/15/2026. Criminal record qualifying letters for the provider are current. Mrs. Ellis’ qualifying letter is due by 7/14/2026. Family member J. Francois is due by 6/25/2025. Mrs. Ellis’ health questionnaire expires on 5/3/2025. Supervision, capacity, and space were observed in compliance today. The last sanitation inspection was conducted in October 2024. The center followed it’s permit restrictions. Fire drills and shelter in place/lock down drills were monitored and in compliance. The center’s last shelter in place drill was documented for October 2024. Playground inspections were monitored and in compliance. The Division of Child Development Family Child Care Home Item Number Listing was used as a guide to review all child care rules and monitor for all applicable child care requirements. There were no violations cited today. Violation Number Comment Rule 1831 A valid qualification letter was not on file and available for review at the facility. Two new family members did not have documentation of a qualification letter on file (R. Ellis and M. Ellis) G.S. 110-90.2 (b) & (d) & .2703(e) .1702(b)(1) Violations must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 1/23/2025, stating how each violation was corrected and how compliance will be maintained in the future include the violation number, statement of compliance, license ID and your signature in the letter or e-mail. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Due to them getting US citizenship yet, their background checks have not been completed yet. Mrs. Ellis was informed that family members that moved in must have a qualification letter on file within five business days of the move in date. The provider was given a chance to ask questions and did not have any at this time.
