I was greeted by Mrs. Brenda Campbell, and I explained the purpose of the visit. Mr. Campbell was present in the home during the visit. The facility currently holds a Three Star License issued January 3, 2020 and had a compliance history score of ninety-four (94) percent prior today’s visit. The facility’s last Annual Compliance Visit was conducted on October 31, 2024. The NC Secretary of State website was reviewed on October 27, 2024 and Superlative Child Care LLC was listed as current-active. There were no children present during the visit. There is one (1) toddler enrolled. There are no children enrolled on second shift. The facility was monitored today with the Family Child Care Home Monitoring Checklist and the 2025 Family Child Care Home Item Number Listing. A copy of the checklist was provided to Mrs. Campbell after the visit. The curriculum that is used is Creative Curriculum. The following was observed posted: Current License, NC Summary of Child Care Law, Schedule, Activity Plan, and Emergency Numbers. Monthly Fire Drills, Shelter-in-Place/Lockdown Drills, Playground Inspections, the Emergency Medical Care Plan, Incident log, and the Emergency Preparedness and Response (EPR) Plan was observed posted; compliance was met. A menu was posted and compliance was met for nutrition requirements. The Safe Sleep Policy and Safe Sleep Poster were observed posted. There are no infant children enrolled. The daily attendance and arrival and departure times were reviewed and met compliance. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit. Soap and toilet paper were available in the bathroom for use. The toys and equipment were in good repair. In the kitchen, there was a working fire extinguisher, working thermometer in the refrigerator, and working smoke detectors. Mrs. Campbell's file was monitored and observed for Health Questionnaire, Emergency Information, CPR and First Aid, ITS-SIDS Training, and a Criminal Background Check (CBC); Mrs. Campbell's husband has a current CBC; no one else resides in the home. The roster was incomplete for the Automated Background Check Management System (ABCMS), and a violation was cited. The outdoor play area was clean and the toys were in good repair; safety hazards were observed. The child enrolled does not have any medications; there are no medications stored; the child enrolled does not have any allergies. Mrs. Campbell does not provide transportation, and Mrs. Campbell does have a vehicle in case of an emergency. The sanitation inspection was conducted January 15, 2025 , with a demerit score of six (6). The Emergency Preparedness and Response Plan (ERP) was reviewed. One (1) violation was cited. Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The roster was incomplete in ABCMS. G.S. 110-90.2 & .2703(r) -Mrs. Campbell stated that the child that is currently enrolled attends the facility on Tuesdays and Wednesdays. -An unannounced visit will be conducted to observe supervision and staff child ratios. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before November 10, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. •Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. • Please note, the hard copy of the Change of Information form will no longer be needed or accepted. The Division of Child Development and Early Education is preparing a summary of the rules, an online training module for Moodle, and an implementation plan. The implementation plan will allow time for administrators/operators to learn and understand the different pathways and the changes within our system. We encourage you to visit the QRIS Modernization page on the DCDEE website to learn more now. -The DCDEE team looks forward to working with child care facilities across the state to transition to the new QRIS system, also known as Pathways to the Stars, located in Section .3200 of the Child Care Rules. The transition will begin soon with the following informational opportunities: • In August, we provided a rule roll out module in the DCDEE e-learning Moodle platform and hosted webinars. • In September, child care consultants hosted in-person facility operator/administrator meetings within the counties served; provided additional guidance on the changes, the transition plan and timeline. • Beginning in October, child care consultants will begin discussing the new rules in Section .3200; Standards for Two through Five Star Rated Licenses during licensing visits. Consultants will review the pathway options, identify facility needs, answer questions, and work with the facility operator to establish an individualized timeline for transition to a new rated license within twelve (12) months based on the pathway chosen. -Child Care Consultants from Mecklenburg County hosted training sessions on QRIS. Mrs. Campbell stated that she attended a QRIS Session. The session provided valuable insights on QRIS, also known as Pathways to the Stars, and is designed to help you understand and prepare for the transition to the new QRIS system. -Get Ready for the 3’s / QRIS: Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions. Visit the NCRLAP’s website, https://www.ncrlap.org/ for more information about updated resources, credit hour trainings, and outreach assessment opportunities to help you become familiar with these tools. For official environment rating scale assessments for a NC Star Rated License, the NCRLAP will use the Revised editions until February 1, 2025. Please review the information on the NCRLAP's website to become familiar with the new third edition. -An email was issued on July 31, 2025, regarding the Quality Every Day (QED) Program. Quality Every Day (QED) is a quality initiative at Child Care Resources Inc., funded by Smart Start of Mecklenburg County. They are recruiting for the QED Program this year for those that may be interested in coaching and training for teachers and directors. There are different tracks for participation (Licensing renewal, Program start-up, staff-child interactions and director support); and there are new opportunities this year like updated ERS-3 trainings, Leadership college coursework for administrators and Communities of Practice for both Child Care Directors and Family Child Care providers. They still have opportunities for college coursework and stipends for coursework completion. -Technical Assistance was provided on the QRIS Pathway to the Stars. Mrs. Campbell stated that she would like to apply for a four (4) star rating through Pathway 2; forms (Pathway to the Stars) were completed during the visit. The following was explained: provider meets, at minimum, the requirements set forth in Option One, Option Two, Option Three, or Option Four as follows: (1) Option One: (A) NC Family Child Care Credential or its approved equivalent; and (B) Successful completion of at least 12 semester hours in early childhood education or child development coursework at an accredited college or university; and (C) Have at least one year early childhood work experience. (2) Option Two: (A) NC Family Child Care Credential or its approved equivalent; or (B) Successful completion of at least 15 semester hours in early childhood education or child development coursework at an accredited college or university; and (C) Have at least one year early childhood work experience. During that visit, no children were present. I was greeted by owner/operator Ms. Brenda Campbell, and I explained the purpose of the visit. I observed one (1) toddler present eating lunch, and the following lunch items were served: turkey, chili beans, collard greens, apple sauce, cornbread and milk. After lunch, I observed Ms. Campbell completing a learning activity (alphabets) with a child on a tablet, and a violation was cited. Supervision: Adequate supervision was provided. Nurturing tones were heard when Ms. Campbell spoke with the child. Staff/Child Ratio: The staff/child ratio met compliance. Discipline: There were no discipline concerns. Permit Restrictions: All permit restrictions were observed meeting compliance today. Child Care Space: Child care was provided in the approved child care space. One (1) violation was cited. Violation Number Comment Rule 543 Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. Screen time was observed provided to a one (1) year old child. .1718(c) -Ms. Campbell stated that she will be closed for the month of July for home repairs that will be made to the facility. I was greeted by Mrs. Brenda Campbell, and I explained the purpose of the visit. Mr. Campbell was present in the home during the visit. The facility currently holds a Three Star License issued January 3, 2020 and had a compliance history score of ninety-six (96) percent prior today’s visit. The facility’s last Annual Compliance Visit was conducted on November 8, 2023. The NC Secretary of State website was reviewed on October 31, 2024 and Superlative Child Care LLC was listed as current-active. There were no children present during the visit. There is one (1) infant and one (1) toddler enrolled. There are no children enrolled on second shift. The facility was monitored today with the Family Child Care Home Monitoring Checklist and the Family Child Care Home Item Number Listing. The curriculum that is used is Creative Curriculum. The following was observed posted: Current License, NC Summary of Child Care Law, Schedule, Activity Plan, and Emergency Numbers. Monthly Fire Drills, Shelter-in-Place/Lockdown Drills, Playground Inspections, the Emergency Medical Care Plan, Incident log, and the Emergency Preparedness and Response (EPR) Plan was observed posted; compliance was met. A menu was posted and compliance was met for nutrition requirements. The Safe Sleep Policy and Safe Sleep Poster were observed posted. A violation was cited for the Safe Sleep Documentation. The daily attendance and arrival and departure times were reviewed and met compliance. Mrs. Campbell's file was monitored and observed for Health Questionnaire, Emergency Information, CPR and First Aid, ITS-SIDS Training, and a Criminal Background Check (CBC); Mrs. Campbell's husband has a current CBC; no one else resides in the home. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit. Soap and toilet paper were available in the bathroom for use. The toys and equipment were in good repair. In the kitchen, there was a working fire extinguisher, working thermometer in the refrigerator, and working smoke detectors. The outdoor play area was clean and the toys were in good repair. There were non-toxic garden plants in the outdoor area. The children enrolled do not have any medications; there are no medications stored; no children with allergies. Mrs. Campbell does not provide transportation, and Mrs. Campbell does have a vehicle in case of an emergency. The sanitation inspection was conducted January 16, 2024 , with a demerit score of six (6). Mrs. Campbell's file was monitored and observed for Health Questionnaire, Emergency Information, CPR and First Aid, ITS-SIDS Training, and a Criminal Background Check (CBC); Mrs. Campbell's husband has a current CBC; no one else resides in the home. Mrs. Campbell met the required number of ongoing training hours. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit. Soap and toilet paper were available in the bathroom for use. The toys and equipment were in good repair. In the kitchen, there was a working fire extinguisher, working thermometer in the refrigerator, and working smoke detectors. There were two (2) outlet covers uncovered and a violation was cited. The outdoor play area was clean and the toys were in good repair. The storage shed was unlocked and a violation was cited. The child enrolled does not have any medications; there are no medications stored. Mrs. Campbell does not provide transportation, and Mrs. Campbell does have a vehicle in case of an emergency. The sanitation inspection was conducted January 5, 2023, with a demerit score of six (6). The Emergency Preparedness and Response Plan (ERP) was reviewed; last annual review by provider was conducted on March 30, 2023. Three (3) violations were cited. Violation Number Comment Rule 716 Electrical outlets not in use were not covered. Two (2) outlet covers were not posted. 10A NCAC .1719(a)(27) 1886 Required locked storage was not secure with combination locks, electronic, or magnetic devices, key, or equivalent locking devices. The outdoor storage shed was not locked. A storage shed was unlocked that contained a hand saw. .1719 (a) (7) 1919 A First Aid Information sheet, complete with required information, was not posted for quick referral by staff. The First Aid Information sheet was not posted. .1719(a )(14) The following Technical Assistance (TA) was provided: -Continue to review emails from the Division of Child Development and Early Education (DCDEE) and from your assigned child care consultant. -House Bill 103: Effective July 11, 2022, individuals have five (5) years to renew their criminal record check instead of every three (3) years. -On June 12th, Governor Cooper signed Senate Bill 291, extending the Child Care Hold Harmless legislation until June 30, 2024, and requiring the North Carolina Child Care Commission to make recommendations for modernizing the state’s Quality Rating and Improvement System. -NCDHHS Children’s Environmental Health has conducted a series of virtual trainings on the recently re-adopted Child Care Sanitation Rules, effective July 1, 2023. -Make sure that all outlet covers are covered. Refer to Child Care Rule 10A NCAC 09 .1719(a)(27) Requirements For A Safe Indoor/Outdoor Environment: (a) The operator of a family child care home (operator) shall provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of children in care, including but not limited to the following: (27) cover all electrical outlets not in use and remove cracked, or frayed cords in occupied outlets; -The First Aid Information Sheet must be posted. Refer to Child Care Rule 10A NCAC .1719)(a)(14) Requirements For A Safe Indoor/Outdoor Environment: (a) The operator of a family child care home (operator) shall provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of children in care, including but not limited to the following: (14) have a First Aid information sheet posted in a place for quick referral by staff members. The information sheet shall include First Aid guidance regarding burns, scalds, fractures, sprains, head injuries, poisons, skin wounds, stings and bites. A child care operator may request a First Aid information sheet from the North Carolina Child Care Health and Safety Resource Center at 1- 800-367-2229; -Keep Outdoor Storage Sheds Locked. Refer to Child Care Rule 10A NCAC 09 .1719 (a)(7) REQUIREMENTS FOR A SAFE INDOOR/OUTDOOR ENVIRONMENT: (a) The operator of a family child care home (operator) shall provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of children in care, including but not limited to the following: (7) keep all corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product that is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled shall be kept in its original container or in another labeled container, used according to the manufacturer's instructions, and stored in a locked area when not in use. Locked areas shall include those that are unlocked with a combination, electronic, or magnetic device, key, or equivalent locking device. These unlocking devices shall be kept out of the reach of a child and shall not be stored in the lock. Toxic substances shall be stored below or separate from medications and food. Any product not listed in this Paragraph that is labeled "keep out of reach of children" without any other warnings shall be kept inaccessible to children when not in use, but is not required to be kept in locked storage. The product shall be considered inaccessible to children when stored on a shelf or in an unlocked cabinet -Your facility has been assigned to Cohort 2: Prep year 7/1/24-6/30/25, Reassessment year 7/1/25-6/30/26.
