This facility operates with a five star facility. They operate year round with a Meck PreK and after school. I was greeted by Ms. C, the supervisor of Y-kids who opened the door an I walked to Ms. McCallum office. I greeted her, stated the purpose of my visit and proceeded with my walk through the facility unassisted. Children were observed actively engaged in activities centers. Teachers were actively engaged with the children. The following centers were available: science, manipulatives, dramatic play, blocks, art to name a few. Today, children were served milk, fresh vegetables, rice, Salisbury steak, and bread. Children were also observed transition to the gym for indoor gross motor play. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure, and all required posted documents. This facility also provides transportation. The outdoor environment was not observed today due to active precipitation. The items posted on the menu meet the required nutritional components. All applicable requirements were monitored today. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last sanitation inspection was conducted 2 /24/2026 The last fire inspection was conducted on 01 /31/2025. The last documented fire drill was conducted 02/05/2026. The last documented shelter-in-place or lockdown drill was conducted 12-22-2025. The last documented playground inspection was conducted 02-09-2026. The last Annual Compliance was conducted 03/04/2025. Compliance History prior to today’s visit is 96% Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Fire inspection was not completed, despite emails sent to the fire inspectors. 10A NCAC 09 .0304(a) 428 A current activity plan was not posted for each group of children for reference. This facility currently holds a five star license. Permit information-the program operates with a five-star license, issued June 11th, 2018, earning seven (7) points for education, seven (7) points for program, and one (1) quality point by meeting one programmatic option. License restrictions include daytime care only; meets all enhanced requirements and reduced ratios; meets enhanced ratios and meets enhanced space. At arrival I monitored bus drop off and children transitioning to the building. A group leader opened the door for me. I stated the purpose of my visit and I walked to met with Natalya McCallum, Assistant Administrator. After greeting and restating the purpose of the visit, I who assisted me with the visit. Later we were joined by Veronie Gamble. I observed the children engaged in the following activities: art, reading, and working on homework. I observed children getting off the elementary school bus into the building and transitioning to handwashing and center/table play. Children were served a snack which consisted of oranges, graham crackers and water. Dinner consisted of milk, rice, ground beef, tortilla, and black beans. During the visit I approved additional spaces to be used by children through out the year, as well as changing the age range from five to 12 years of age to three years of age to 12 years of age. This facility would like to start having two Mecklenburg Pre-K classrooms during the day. The space for Meck Pre-k are approved, however, do not use the space until you receive the additional materials discussed in the technical assistant section. Once materials are received, please email me a picture off the materials on the shelves. Also be aware that to be in compliance with having preschool age children, staff qualifications will change. (Pages 24- 25, CHAPTER 09 -CHILD CARE RULES EFFECTIVE NOVEMBER 1, 2024) INSPECTIONS: The last sanitation inspection was conducted on 11/4/2025 . The last fire inspection was conducted on 02-13-2025. The last documented fire drill was conducted 02-06-25. The last documented shelter-in-place or lockdown drill was conducted 2-06-25. The last documented playground inspection was conducted 02-07-25. The last compliance history was 90% I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management, transportation, and all required postings and requirements. The attached checklist was used to note the requirements we monitored today. The new spaces moonitored are Space 7 (Dojo room): Children, Space 8, (room 325): children, Space 9 (237-old cycle room): 8 children, Space 10: children, Space 11: children These spaces are preexisting spaces that were used temporarily during covid 19 pandemic and measured on 8/20/2020. These spaces are now license space for ages four through twelve years of age. Violation Number Comment Rule 508 Special diet or food allergy information was not posted where they can be seen in food preparation and eating areas. In space two, and space four, there was no evidence of this reqirement. .0901(g) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. The bus used to pick up children did not have the required identifying information. 10A NCAC 09 .1003(d) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services.One child did not have the required information with their application. .0801(b) 1850 Signage was not posted regarding the smoking and tobacco restriction at the entrance of the center and/or in vehicles used to transport children. There was no signage available to monitor. .0604(i) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. REQUIRED RESPONSE: You shall correct the violation(s) immediately. I was greeted by a group leader/camp counselor who opened the door an I walked to Ms. McCallum Ms. McCallum was talking to a child and group leaders when I arrived to her office. I greeted her, stated the purpose of my visited and proceeded with my walk through of the facility unassisted. Children were observed actively engaged in gross motor play, playing basketball, and eating snack. Snack consisted of popcorn and apples. Many of the group leaders were actively engaged with the children. Children were also observed departing from the facility with there parents. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure, and all required posted documents. New Staff files were monitored today. This facility does not provide transportation. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival, and departure, and all required posted documents. New Staff files were monitored today. Violation Number Comment Rule 707 Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. The top of a grey shelf was broken in group five, in group 4 the was a drawer system that the top was off. .0601(d) 1301 Center did not maintain a record of daily attendance. Upon arrival I observed children getting off of the school bus and being escorted in the building by staff. I walked to the side door and a teacher greeted me and said Ms. N. McCollum was in her office. I walked to the administrators office and greeted her and stated the purpose of my visit. I proceeded with the visit unassisted. Full enrollment wass not captured during this visit, however their were 41 children present. Violation Number Comment Rule 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. There are also concerns of inadequate supervision as it relates to the s/c ratios. Upon arrival, observed children engaged in outdoor gross motor play. I entered the building through the unlicensed space and introduced myself to the attendant. I stated the purpose of my visit and asked some interview questions that will be detailed below. The attendant then stated that the Senior Program Director will be able to assist me with more detail’s information. Ms. Gamble appeared and I greeted her and restated the purpose of my visit. I reviewed the allegations with the administrators, and they were given an opportunity to respond to the concerns. I conducted a walkthrough of the facility partially assisted by Ms. Gamble. The following items were monitored: supervision, staff/child ratio general safety, discipline, adequate/approved space, permit restrictions, most recent violations and all required posted documents. Enrollment was not captured during this visit; however, all classes were in ratio. Observation: During the visit, children were observed participating in outdoor gross motor activities. The following areas were monitored for compliance: supervision, ratios, space, general safety, and required postings. Enrollment was not captured during this visit. Lunch consisted of grapes, chicken alfredo, watermelon, bread, and milk. Space four was in the pool with N. McCallum, family Service director, there were nine children in the pool with her and the remaining four were sitting on the side of the pool or in chairs. Lifeguards were on duty. Handwashing and transitions were observed, and Snack consisted of sun-butter and pretzels. Staff were engaged with children and placed around the playground and classroom to observe and interact with children. During the visit Staff ages were verified, attendance for both regulated and unregulated programs were monitored as well as the subsidy attendance due to supplemental information given and needing to be verified. Complaint: Allegations: The report alleges that There are concerns of staff/child ratios being out of compliance. Findings: Prior to this visit I attempted to contact the reporter for more information and was unsuccessful. Administration and Staff were interviewed. When asked about ratios, staff were aware of ratios based on their age group. All classrooms were observed in ratio today. Four of the five spaces being used had a minimal of two staff. Space one: 2:18, Space two: 2:17, Space three: 1:18, Space four: closed, Space five: 2:22. There were a total of 72 children present, seven teachers, and one additional staff. Staff ages were verified in compliance with rules and it was stated that any staff under 18 years of age is paired with older staff in both the regulated and unregulated areas. It was also stated that in the unregulated area, there are more camp counselors and could have a minimum of three or more per group and activity. Incident logs were reviewed, their were several minor incidents but none that appeared to be a result of lack of supervision. -Allegation regarding concerns of staff/child ratios being out of compliance is deemed unsubstantiated due to findings, reports and observations. -Allegation regarding concerns of inadequate supervision as it relates to the s/c ratios, were deemed unsubstantiated due to findings, reports, and observations. Violation Number Comment Rule 103 The number of children present was not within permit capacity. The space approved for the size of the classroom was not followed. Space two/group three has a max of 15 children, 19 children were using that space. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Technical assistance: Ms. Gamble and I discussed having other spaces approved for future use and possible increase of capacity. Once you review and decide on the below items, please contact the appropriate agencies as discussed and used before to approve spaces. Once spaces have been approved, forward the approvals and I will set up a time to remeasure the space. 1. Division shall issue a new license to reflect the increase in licensed capacity. (2) If a licensee wishes to increase the center's licensed capacity by using space that is already approved for child care, the Division shall, upon request, issue a new license showing the increase in accordance with applicable requirements of G.S. 110, Article 7 and this Chapter. 2. (f) For centers with a licensed capacity of 200 or more, there shall be a second administrator on-site for a minimum of 20 hours per week who shall have the Level I North Carolina Early Childhood Administration Credential or its equivalent. (d) For programs with a licensed capacity of 200 or more school-age children, there shall be two program coordinators on site, one of whom shall not have concurrent group leader responsibilities. The additional program coordinator shall have completed the applicable staff requirements in Rule .2510(b) of this Chapter. 3. If you increase you facility capacity, make sure the playground can accommodate enhanced space. 4. Review CHILD CARE RULES Effective July 1, 2023 to read any updated changes that may prohibit your capacity increase and enhanced space requirements.
