The last annual compliance visit was made on 10/12/2023. The current 5 star rated license was issued 11/03/2020. The March 2024 version of the Child Care Center Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 99% as of today. I was let into the school by the front office staff. Site coordinator J. Udechukwu accompanied me on the walk through. Program records, staff records, and children's records were made accessible to me. The following was observed posted: Emergency Care Plan, Safe Drop off/Pick up, Current License, Summary of Law, Menu, Activity Plan, and Schedule. A Sanitation Inspection was completed on 12/20/2023. A Fire Inspection was completed on 3/25/2024. Fire Drills, Outdoor Playground Inspections, and Emergency Preparedness Drills were monitored today. Attendance and Sign In/Out was monitored. An Offsite Staff Verification form, Off site transportation form, and Offsite Children's Verification form was monitored. All used licensed spaces, outdoor play area, and bathrooms used by the children were monitored today. Children arrived at the cafeteria as school let out. Both groups used the restroom, then went outside for outdoor play. Once returning inside they will have snack, then group one will have free play in the gym, and group two will have center time. Then it will be homework and TDA until closing. The following violations were cited during todays visit: Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. One (1) substitutes file was not available for review. G.S. 110-91( 9) 1757 A valid qualification letter was not on file and available to review at the facility. A valid qualification letter was not on file and available to review at the facility for one (1) substitute. G.S. 110-90.2(b) & (d) & .2703(e) Compliance -Violations cited today must be corrected immediately. Please send me a letter of compliance of how the violations cited were corrected by October 22, 2024. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. The March 2024 Child Care Center Item number listing was use to monitor this facility. I was let into the school by the front office staff. Upon my arrival I stated to them the reason for my visit. Site coordinator Jullia Udechukwu and Group Leader Carolyn Marcus were in the cafeteria setting up. Files for this facility were made available to me. The following was observed posted: Current license, Summary of Law, and Safe Arrival and Departure procedures, Staff Child ratio chart, and Medical Care Plan. The current schedule, menu, and lesson plans were observed posted. Monthly Playground Inspections and Fire Drills were monitored. Emergency Preparedness Drills were monitored. A Sanitation Inspection was completed on 12/20/23 and a Fire Inspection was completed on 3/25/24. Three (3) staff files were monitored for Criminal Record Checks, Emergency Info/Health Questionnaire, First Aid/CPR training, and BSAC training. An Offsite Staff Record Verification form was observed. The cafeteria, gym, and bathroom, area used by the children was monitored. Children arrived at the cafeteria as school let out. Attendance was taken, then they went outside. After outside play, the children come in and have snack. After snack, homework and TDA is completed, then free play until dismissal. Attendance/Sign in and out was monitored. The following violations were cited during today's visit: Violation Number Comment Rule 1043 All staff records, except financial records, were not made available for review. Records for one (1) staff was not available for review. G.S. 110-91( 9) Compliance -Violations cited today must be corrected immediately. Remember all staff are required to be listed on the staff off site varication form. It was stated today that you have been prepping for the SACERS-U. You are in Co-hort one (1) who is scheduled to begin completing rated license assessments this year, however it is voluntary, and can be completed at any time before that. The Environment Rating Scale request form will be made available to you. Once you fill it out and return it to me, I will submit it to request to have the ERS completed.
