The facility operates with a Five-Star Rated License based on meeting requirements at the five-star level for classroom and instructional quality. The effective date of the license is January 16, 2026. Upon arrival I was greeted by Aisha Owens, Director. We completed a walk-through of the facility. I observed children in the indoor learning and outdoor environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in personal care routines, teacher directed activities, free choice of indoor and outdoor activity areas. Staff members were observed supervising activities, and assisting with personal care routines and teacher directed activities. The NC Secretary of State website was reviewed on July 7, 2026, and Spoiled Rotten Scholars LLC was listed as current-active. A sanitation inspection was completed April 7, 2026, with a “Superior” classification. The last approved fire inspection was received on July 2, 2025. The Emergency Drill Log was reviewed today. A shelter-in-place drill and lockdown drill was conducted May 2, 2026. A monthly fire drill was conducted June 22, 2026. Playground safety checklists were also monitored and are occurring each month as required. The facility does not provide transportation. Files for ten percent of children enrolled were monitored. The Staff and Training Worksheets were received today. The worksheets were also used to verify staff having current criminal background qualifying letters, first aid, CPR training and ITS-SIDS training for staff working in the infant room and BSAC training for school age group leaders. There has been one new staff hired since a temporary time period visit was conducted on December 16, 2026. The personal files and medical files for the new staff member and ten percent of existing staff were monitored. The following violations were observed. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last approved fire inspection was received July 2, 2025. 10A NCAC 09 .0304(a) 721 All equipment and furnishings were not in good repair. A small portion of the plastic border around the mulch on the school age playground and preschool playground was in poor repair. The plastic had cracked and was broken creating an opening on the top of the border. G.S. 110-91(6); .0601(b) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In space #4b, a bottle of Cetirizine was not stored in locked storage. 15A NCAC 18A .2820(d) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. The safe sleep policy was not posted in the infant room. .0606(b) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before July 22, 2026, a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. This emergency medical care information shall be on file in the center on the child's first day of attendance and shall be updated as changes occur and at least annually. Safe Sleep Policy- The policy must be posted in the space where children twelve months and younger are present. Although, the policy was not posted a safe sleep poster was posted instead. I observed children in the indoor learning and outdoor environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in personal care routines, teacher directed activities, free choice of indoor and outdoor activity areas. Staff members were observed supervising activities, and assisting with personal care routines and teacher directed activities. The NC Secretary of State website was reviewed on July 7, 2026, and Spoiled Rotten Scholars LLC was listed as current-active. A sanitation inspection was completed April 7, 2026, with a “Superior” classification. The last approved fire inspection was received on July 2, 2025. The Emergency Drill Log was reviewed today. A shelter-in-place drill and lockdown drill was conducted May 2, 2026. A monthly fire drill was conducted June 22, 2026. Playground safety checklists were also monitored and are occurring each month as required. The facility does not provide transportation. Files for ten percent of children enrolled were monitored. The Staff and Training Worksheets were received today. The worksheets were also used to verify staff having current criminal background qualifying letters, first aid, CPR training and ITS-SIDS training for staff working in the infant room and BSAC training for school age group leaders. There has been one new staff hired since a temporary time period visit was conducted on December 16, 2026. The personal files and medical files for the new staff member and ten percent of existing staff were monitored. The following violations were observed. This is the facility’s third temporary time period visit. The facility’s temporary license expires on January 15, 2026. The facility’s compliance history was 96% prior to today’s visit. The following items were monitored: Posted License & Permit Restriction Supervision, Approved Space, Staff Child/Ratios and Group size, Storage of Hazardous Products, Storage of Medication, General Safety Requirements, New Staff Files/Criminal qualifying letters , Specialized Training, Program records. Upon my arrival I was greeted by Aisha Owens, adminstrator and Ms. Thomasine Wyche. Infants and toddlers were observed playing on floor with the teacher. Safe Sleep Check Charts and feeding schedules were completed and maintained as required. Preschool children were observed in circle time. A variety of materials was present in the activity centers. All required documents were current and posted. Samples of children’s artwork was observed. The teachers were nurturing and engaged with the children. All of the groups were adequately supervised, within staff/child ratio, group size and space capacity requirements. Playground inspections and fire drills were completed and documented as required. The EPR was to be completed by November 4, 2025, however Ms. Wyche is having trouble uploading the map requirements. Two (2) new staff have been hired since my last visit. I monitored new staff files. All staff are current on Criminal qualifying letters and specialized trainings. The last sanitation inspection was conducted on February 26, 2025. The last fire inspection was on July 2, 2025. One (1) violation was cited and discussed with both administrators. Violation Number Comment Rule 1812 The center did not complete an EPR Plan within four months of completing the EPR in Child Care training and/or the Plan was not completed on a template provided by the Division. The EPR plan was to be completed by November 4, 2025 and has not been completed. .0607(c) Corrective Action The childcare provider is expected to always maintain all applicable childcare requirements. The provider will send me corrections to the violation cited during today’s visit on or before Tuesday December 30, 2025. You must send a letter explaining what actions were taken to correct the violations cited during the visit. Failure to correct the violations and/or submit the compliance letter to me by the due date listed about may result in an unannounced follow-up visit being conducted to ensure corrections were made. Technical Assistance and other discussions: The facility has chosen Pathway #2. The facility will follow the Reduced Enhanced Ratio that is required at the Five Star level. The facility has incorporated the Two Star Rated License requirements into their policy as well as the additional four choices listed below as required at the Five Star Level. These policies were verified during today’s visit. The Facility Continuous Quality Improvement Plan and the Individual Continuous Quality Improvement Plan were completed and verified during today’s visit. The facility is using the Creative Curriculum and Formative Assessment for all ages served. The administrators and the two Lead Teachers have been trained on the Curriculum and Formative assessment as required. The facility will be doing the child assessments at the beginning of the school year and then again and at the end of the school year on each child enrolled. The administrator has chosen Rule .3205 (f) (10) (A) for the administrator and (11) (A) for all lead teachers. The facility has two Lead Teaches. The facility has two other staff. The facility meets the Five Star Education level. The administrators are reviewing rule .3224 the Recognition of Quality Initiatives and will send me the initiatives by December 30, 2025. The CBC Provider Portal/Technical Assistance & Notification to the Division of New Hires: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days.
