Name of Operation: SMITHFIELD CHURCH ROAD ELEMENTARY ASEP Facility ID: 60000378 Consultant: DENISE WATSON Operation Type: Center Case Number: Visit Date: 11/4/2024 Number Present: 32 Completed Date: 11/4/2024 Age: From 5 To 9 Total Minutes: 45 Time In: 12:25 PM Time Out: 01:10 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Compliance Follow-Up Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor violations cited during compliance during an annual compliance visit on 10/14/2024.I was let into the school by the front office staff. I walked to the cafeteria where the afterschool program was located. Violation 1124 was cited. Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. A photograph was not including with children's information for each child being transported. This violation has not been corrected and was cited again today. The following was observed posted: Emergency Care Plan, Safe Drop off/Pickup, Current License, Summary of Law, Menu, Activity Plan, and Schedule. Children had just come in from outside play and were observed having free play. Group two (2) left to do homework in the approved classroom trailer. The following violation was cited: Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. A photograph was not including with children's information for each child being transported. This is a repeat violation. 10A NCAC 09 .1003(d) Compliance -Violations cited today must be corrected immediately. Please send me a letter documenting how violations cited today were corrected by November 18, 2024. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Ms. Wolfe stated that she only needs to collect five (5) more pictures of children. She has communicated to parents the need and the importance of receiving a picture. Parents were asked to bring in a picture or add it to the database ASEP uses for her to print and use in the transportation file. Ms. Wolfe stated that she will take the remaining pictures herself and print them to place in the file to correct the violation. The last annual compliance visit was made on 10/18/2023. The current 5 star rated license was issued 03/27/2019. The March 2024 version of the Child Care Center Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 97% as of today. I was let into the school by the front office staff. They let site coordinator L. Wolfe know that I was present. She accompanied me on the walk through. Program records, staff records, and children's records were made accessible to me. The following was observed posted: Emergency Care Plan, Safe Drop off/Pick up, Current License, Summary of Law, Menu, Activity Plan, and Schedule. A Sanitation Inspection was completed on 12/20/2023. A Fire Inspection was completed on 9/20/2024. Fire Drills, Outdoor Playground Inspections, and Emergency Preparedness Drills were monitored today. Attendance and Sign In/Out was monitored. An Offsite Staff Verification form, Off site transportation form, and Offsite Children's Verification form was monitored. All used licensed spaces, outdoor play area, and bathrooms used by the children were monitored today. Children arrived at the cafeteria as school let out. Group one will use the restroom, then have outside play. Group two will go to the restroom and then have snack. The following violations were cited during todays visit: Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. A photograph was not including with children's information for each child being transported. 10A NCAC 09 .1003(d) 1898 Staff did not complete the health and safety training within one year of employment. All health and safety training topics were not complete within one (1) year for two employees. .1102(a) Compliance -Violations cited today must be corrected immediately. Please send me a letter documenting how violations cited today were corrected by October 16, 2024. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Staff and training worksheets should be kept current and on file. I suggest completing them annually when you're completing the off-site verification forms.
