Amy Italiano, lead consultant accompanied me on today's visit. The last Annual Compliance visit was made on 8/21/2024. The current 5 star license was issued on 08/23/2024. The April 2025 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 98% as of today. I was let into the facility by teacher Maddie Perkins. Assistant director Kayla McKoy and director Natasha Kimber was present. Facility files were left accessible to me. Ms. Kimber accompanied me on the walk through of the facility. Eight (8) classrooms were monitored on today for: Sign in/out, allergy list, medical action plans (if applicable), menu, schedule, activity plan, and staff/child ratio chart. Storage of medication and permission to administer was monitored. Supervision, and storage of hazardous products was monitored. Children were observed having indoor/outdoor free play, teacher directed activities, and having routine needs met. There were toys available. The following was observed posted: current license, Summary of Law, Safe Arrival and Departure procedures, Menu, Emergency Care Plan, and no smoking signage. A Sanitation inspection was conducted on 4/15/2025. A Fire inspection was conducted on 9/11/2024. Monthly Fire Drills, Playground Inspections, and EPR Drills were monitored. Staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. A sample of children's files were monitored for Application, Emergency Care authorization, Summary of Law, Medical/Immunization, Discipline Statement, No smoking statement, Safe Sleep Policy, Infant Feeding Schedule and Shaken Baby Syndrome Policy (if applicable). This facility does not provide transportation. The following violations were cited during today's visit: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One (1 staff was observed eating Lays potato chips in the hallway. .0901(i) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One (1) staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) 1898 Staff did not complete the health and safety training within one year of employment. One (1) staff did not complete the health and safety training within one year of employment. .1102(a) Compliance Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Technical Assistance The holes in the turf on the two (2) year old playground and preschool playgrounds have been patched but they will wear again. The general manager present today stated that you are in the final stages of approving the quote and construction should start next month. Children will utilize whatever playground is not being worked on. Please keep me updated on when the repairs are complete and children resume using the the appropriate playground. We discussed that sippy cups can be brought in from home as long as they have a lid and have the child's name and date on the cup. We discussed that Emergency Medical Information needs to be update annually. They do not need to have a separate paper, they can review the original form, date and initial that the information is still accurate. The last Annual Compliance visit was made on 06/2/24. The current 5 star license was issued on 11/22/2017. The March 2024 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 96% as of today. I was let into the facility by director Lucia Huber. Ms. Huber and Ms. Kimber accompanied me on the walk through of the facility. Facility files were made accessible to me. Nine (9) classrooms were monitored on today for: Sign in/out, allergy list, medical action plans (if applicable), menu, schedule, activity plan, and staff/child ratio chart. Storage of medication and permission to administer was monitored. Supervision, and storage of hazardous products was monitored. A sample of staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. A sample of children's files were monitored for Application, Emergency Care authorization, Summary of Law, Medical/Immunization, Discipline Statement, No smoking statement, Safe Sleep Policy, Infant Feeding Schedule and Shaken Baby Syndrome Policy (if applicable). This facility does not provide transportation. The following violations were cited during today's visit: Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Three (3) authorization forms for diaper cream dates had expired. 10A NCAC 09 .0803(1)(a & b) Compliance -Violations cited today must be corrected immediately. Please send me a letter documenting how violations cited today were corrected by September 4, 2023. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Rated License The Environment Rated License Assessment results were discussed during today's visits. Items scoring a four or below and/or that could be a licensing violation were discussed. Staff's education was made available to me. Upon reviewing and confirming education, the Rated Licensing Packet will be processed. Technical Assistance There are a few holes in the turf on the preschool playground, and the toddler/two playground. The steps leading up to the climbing structure on the preschool playground are becoming warped. We discussed a plan for maintenance. It was stated that the holes will be filled and sealed over as well as the steps and platform on the climbing structured before it gets worst and becomes aa hazard. It was also stated that the the playground will be remodeled in the near future. Please continue to check the DCDEE website weekly as new information is added as well as emails from me.
