This facility has a 93% compliance history as of today. I was let into the facility by teacher S. A. Abernathy. She let director Shea Fitzhenry know that I was present. She accompanied me on the walk through. Program records were made available to me. Six (6) classrooms were monitored for attendance and sign in/out. Allergy lists, schedule, menu, and staff/child ratio charts were posted. Storage of hazardous products and medication was monitored. Permission to administer medication was monitored. The food prep area, and bathrooms used by children were monitored. The following violations were cited: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. I completed the walk through. Ms. Fitzhenry was relieved by the cook to make program records available to me. Six (6) classrooms were monitored for attendance and sign in/out. Allergy lists, schedule, menu, and staff/child ratio charts were posted. Storage of hazardous products and medication was monitored. Permission to administer medication was monitored. The outdoor area, food prep area, and bathrooms used by children. The following violations were cited: Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One (1) outlet was uncovered in space four (4). 10A NCAC 09 .0604(c) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. The March 2024 Master Center Item Number Listing was used to complete today’s visit. I was greeted by teacher S. Abernathey and director Shea Fitzhenry. Ms. Fitzhenry accompanied me on the walk through of the facility. The facility's records were made accessible to me. Supervision, Staff Child Ratio, First Aid, Special Training, Storage of Hazardous Substances and Medication, Approved Space, Staff Records, Program Records, License Posted, and Permit Restrictions were monitored today. The following violations were cited during today's visit: Violation Number Comment Rule 860 Balloons were accessible to children. I suggested placing rugs or soft mats in other areas of the rooms to foster cozy and comfort when reading or playing having quiet activities. This facility has a 93% compliance history as of today. I was greeted by Pastor Tom Raven and director Shea Fitzhenry. A walk through of the facility was conducted. Six (6) classrooms were monitored for attendance and sign in/out. Allergy lists, schedule, menu, and staff/child ratio charts were posted. Storage of hazardous products and medication was monitored. Permission to administer medication was monitored. There were toys and materials in each classroom. Bathrooms used by children were stocked with paper towels, tissue, and soap. Refrigerator thermometers and storage of food was monitored. All staff were very attentive and engaged with the children on today. Notice of Compliance, N.C. Summary of Law, Emergency Care Plan, and No Smoking Signage, were posted. Playground Inspections and Fire Drills were monitored. A sample of staff files were monitored for First Aid/CPR, Annual Health Questionnaire, Special trainings, and Criminal Background Check Qualifying Letters. A sample of children's files were monitored for Application, Emergency Care authorization, Summary of Law, Medical/Immunization, Discipline Statement, Outside fence permission, and Shaken Baby Syndrome Policy. This facility does not provide transportation. A Sanitation inspection was conducted on 1/2/2024. A Fire inspection was conducted on 03/24/2023. The following violations were cited during today's visit: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. Then when they check them in fifteen (15) minutes, document the new position. -Your last fire inspection was completed 3/24/23. Your annual date is approaching. I encourage you to contact your file inspector to ensure that this is completed within the required time frame. -Remember that off premise permission for enrolled children (for stroller rides, and emergency drills), and emergency information must be updated annually. -There is a gap between the platform and sliding board (left side) on the large playground. It was documented on the playground inspection form for months and observed during today's visit. Please refrain from having children to play on the equipment until clarification can be made to whether it's a hazard or not. The August 2023 Master Center Item Number Listing was used to complete today’s visit. I was greeted by Pastor Tim Ravan. I told him the reason for my visit. He escorted me to the director's office and let Shea Fitzhenry know that I was here. She arrived shortly and accompanied me on the walk through of the facility. The facility's records were made accessible to me. Supervision, Staff Child Ratio, First Aid, Special Training, Storage of Hazardous Substances and Medication, Approved Space, Staff Records, Program Records, License Posted, and Permit Restrictions were monitored today. Two (2) new staff's file was monitored. The following violations were cited during today's visit: Violation Number Comment Rule 303 Children were not adequately supervised at all times. One (1) teacher was supervising eleven (11) children between space three (3) and four (4). .1801(a)(1-5) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. Technical Assistance Share with all staff supervision and ratio requirements 10A NCAC 09 .1801 SUPERVISION IN CHILD CARE CENTERS (a) Children shall be adequately supervised at all times in child care centers. Adequate supervision shall mean that: (1) staff must be positioned in the indoor and outdoor environment to maximize their ability to hear or see the children at all times and render assistance; (2) staff must interact with the children while moving about the indoor or outdoor area; (3) staff must know where each child is located and be aware of the children's activities at all times; (4) staff must provide supervision appropriate to the individual age, needs, and capabilities of each child; and (5) staff must be able to see and hear children aged birth to five years old while the children are eating. Utilize the staff child ratio charts located in each licensed space to ensure that the voluntary enhanced ratio is being followed. If a caregiver is going to another part of the facility, they must bring the children with them or call for an additional caregiver to help.
