The facility currently holds a Five Star License issued January 16, 2018, and had a compliance history score of 93% prior today’s visit. The facility’s last Annual Compliance Visit was August 8, 2022. I was greeted by the owner/operator, Mrs. Vantoinette Savage. Mrs. Savage's husband was present in the home during the visit. One (1) child was present during the visit. The following was observed posted: Current License, NC Summary of Child Care Law, Schedule, Activity Plan, and Emergency Numbers. Monthly Fire Drills, Playground Inspections, the Emergency Medical Care Plan, Safe Sleep Poster and Safe Sleep Policy, Handwashing signs, and the Emergency Preparedness and Response (EPR) Plan was observed posted; the EPR Plan was last updated August 2022. Mrs. Savage is using an approved curriculum, Creative Curriculum. The daily attendance and arrival and departure times were reviewed and met compliance. Upon arrival there were no children present, and one (1) child arrived during the visit. I observed the child completing a supervised independent activity, and I observed the child eating breakfast: salmon patties on a roll, fruit, and milk; breakfast items were noted on the menu. Handwashing was observed during the child's arrival, and before and after breakfast. The provider's handwashing was observed at appropriate. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit; compliance was met. Soap and toilet paper were available in the bathroom for use, and there were no hazardous materials observed. There were no hazards in the child care room and the toys were in good repair. In the kitchen, there was a working fire extinguisher, working thermometer in the refrigerator, and smoke detectors. The outdoor play area was clean and the toys were in good repair. Mrs. Savage does provide additional toys for the children during outdoor play. The provider file was monitored and observed for Health Questionnaire, Emergency Information, Professional Development Plan, First Aid and CPR, and Criminal Qualifying Letters. Mrs. Savage's three (3) household members, husband, grandson and granddaughter, have current criminal qualifying letters on file. Mrs. Savage has a thirteen year-old granddaughter that resides in the home. Mrs. Savage has a child care file for her granddaughter. In-Service training hours were reviewed, and a violation was cited for the incomplete Ongoing Training Log The children’s files were monitored today using the children's records form. Two (2) children's enrollment date was incomplete on the Discipline Policy, and a violation was cited. A Pet Acknowledgement form was signed for Mrs. Savage's dog. The dog is inaccessible to children. A Pet acknowledgement was signed for each child enrolled, and the dog has current rabies vaccinations on file. There are no children enrolled with allergies, and there are no children on medication or emergency medication. Ms. Savage does not provide transportation, however, she does have a current driver's license and a vehicle in case of an emergency. The Written Plan of care was verified. The Sanitation Report was completed on January 18, 2023, with a superior rating and eight (8) demerits. Two (2) violations was cited during today's visit. One (1) violation was corrected during the visit. Violation Number Comment Rule 916 Operator did not maintain a record of on-going training in which he/she has participated. 10A NCAC 09 .1705(b)(4)(C) 925 Signed and dated statement which attests that a copy of the discipline policy was given to and discussed with a parent/guardian of each child prior to enrollment was not on file in the home. Two (2) children (D.M.) and (G.C.'s) enrollment date was incomplete on the form. G.S.110-91(10); .1727(a)&(b) Corrective Action The child care provider is expected to maintain all applicable child care requirements at all times. The provider will send me a compliance letter explaining how she corrected each of today’s violations and the steps she put in place to ensure on going compliance to me on or before Monday, August 14, 2023 to Deanna.Matthews@dhhs.nc.gov, or to the address listed below: Deanna Matthews P.O. Box 756 Gastonia, NC 28053-0756 Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. The following Technical Assistance (TA) was provided: -Continue to review emails from the Division of Child Development and Early Education (DCDEE) and from your assigned child care consultant. -House Bill 103: Effective July 11, 2022, individuals have five years to renew their criminal record check instead of every three (3) years. -On June 12th, Governor Cooper signed Senate Bill 291, extending the Child Care Hold Harmless legislation until June 30, 2024, and requiring the North Carolina Child Care Commission to make recommendations for modernizing the state’s Quality Rating and Improvement System. -NCDHHS Children’s Environmental Health will be conducting a series of virtual trainings on the recently re-adopted Child Care Sanitation Rules, effective July 1, 2023. The rules have been approved but have not yet been updated in the Administrative Code. Once updated, notification will be sent out via listserv. -The North Carolina Child Care Commission and the North Carolina Department of Health and Human Services Division of Child Development and Early Education are collaborating to modernize NC’s child care quality rating and improvement system (QRIS), commonly known as the Star Rated License. We are developing a plan that will be reviewed by the legislature in Spring 2024. During the next few months, we are collecting information from parents, teachers, administrators, operators, and partners to consider in the plan. -If you are unable to attend these sessions, you may also complete an online survey or write to DCDEE_QRIS@dhhs.nc.gov. -Stabilization Grant Funds paid for Quarters 1-6 must be spent by September 30, 2023. -Ten (10) hours of in-service training hours are required per year for the provider.
