I was greeted by Owner/operator, Ms. Shana Rushing, and I explained the purpose of the visit. Ms. Rushing's husband was present in the home during the visit. The program currently operates with a Five-Star License issued July 18, 2022. The NC Secretary of State website was reviewed on March 1, 2024, and S & E Child Development Center, Inc., was listed as current-active. The last annual compliance visit was conducted March 8, 2023. The center's compliance history was reviewed with the operator. The program’s compliance history was one-hundred (100) percent as of February 29, 2024. I observed five (5) children upon arrival, and Ms. Rushing and the children were observed having music and movement. I observed Ms. Rushing reading to the children. Nurturing tones were heard when Ms. Rushing spoke with the children. The children were observed eating the following for lunch: chicken, peas, applesauce, rolls, and milk; drinking water was available. The program was monitored with the Family Child Care Home Item Number Listing, and the Annual Compliance Monitoring Checklist for Family Child Care Homes. A copy of the monitoring checklist was provided to Ms. Rushing after the visit. The following was observed posted: Current License, NC Summary of Child Care Law, Schedule, Activity Plan, Monthly Fire Drills, Shelter-In-Place/Lockdown Drills, Playground Inspections, Safe Sleep Policy and Poster, Incident log, and the Emergency Preparedness and Response (EPR) Plan was observed. The Emergency Numbers were not posted, and a violation was cited. The program adhered to permit restrictions. There are no infants currently enrolled and Ms. Rushing is current with Infant Toddler Safe Sleep (ITS-SIDS) training. The daily attendance and arrival and departure times were reviewed and met compliance. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit. Soap and toilet paper were available in the bathroom for use. The toys and equipment were in good repair. In the kitchen, there was working thermometer in the refrigerator, and working smoke detectors; there was a working fire extinguisher. Ms. Rushing's file was monitored and observed for Health Questionnaire, Emergency Information, First Aid and CPR, and Criminal Background Check (CBC), Qualifying Letters; Mr. Rushing has a current CBC letter on file. Ms. Rushing met the requirements for ongoing training hours. No one else resides in the home. The children's records were reviewed and compliance was met. There are no children enrolled with allergies, and there are no children on medication or emergency medication. The Written Plan of Care was verified. The outdoor play area was clean and the toys were in good repair. Balls, a basketball goal, riding cars, a parachute, and a tunnel, were observed items used to support gross motor outdoor play. Ms. Rushing does not provide transportation, however, she does have a vehicle in case of an emergency. The Sanitation Inspection was completed September 7, 2023 with a total demerit score of zero (0). The Emergency Preparedness and Response Plan was reviewed. The plan was last updated August 15, 2023. One (1) violation was cited; corrected during the visit. Violation Number Comment Rule 1931 Telephone numbers for the fire department, law enforcement office, emergency medical service, and poison control were not posted in a location visible in the home. The emergency numbers were not posted. .1719(a )(12) Reminders: -Continue to review emails from the Division of Child Development and Early Education (DCDEE) and from your assigned child care consultant. -Your facility has been assigned to Cohort 3: Prep Year: 7/1/25-6/30/26 Reassessment Year: 7/1/26-6/30/27. Please refer to the link for additional information: https://ncchildcare.ncdhhs.gov/Provider/Licensing/Star-Rated-License/Resuming-Star-Rated-License. -On June 12th, Governor Cooper signed Senate Bill 291, extending the Child Care Hold Harmless legislation until June 30, 2024, and requiring the North Carolina Child Care Commission to make recommendations for modernizing the state’s Quality Rating and Improvement System. -Continue to report days that the program will be closed. The following Technical Assistance (TA) was provided: -Emergency numbers, for fire department, law enforcement office, emergency medical service, and poison control must be posted. Refer to Child Care Rule 10A NCAC 09 .1719(a)(12). -If you elect to only complete the self-study instead of the Environment Rating Scale (ERS) assessment, you will only receive two (2) points in Program, for completing the self-assessment for the rated license, which will impact your overall star rating. The self-study must be completed for three (3) months, and I will review the self-study. Even if you go through the Environment Rating Scale (ERS) assessment, you still have to complete the self-study. The visit summary was reviewed, signed, and a copy was provided to you.
