Your program operations with a Four Star Rated License with an effective date of January 27, 2022. Upon my arrival, I checked into the school's main office. Program Coordinator, D. Gentry assisted me with today's visit. The program operates before and after school. There are a total of fourteen children enrolled before school. The cafeteria is used as the primary space for the children. Activity areas were set up in the cafeteria. Today's activities included social studies, language arts, math, science, reading and board games. I observed the children participating in arrival procedures and free choice of indoor activities. Staff were observed interacting with the children in a nurturing and caring manner. The Staff and Training Worksheet was reviewed today to confirm existing staff were current with criminal background qualification letters, First Aid and CPR training and had completed BSAC training. There has been one new staff member hired since the annual compliance visit conducted on November 5, 2025. Records for the new staff member was reviewed. Violation Number Comment Rule 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One new employee with a hire date of November 20, 2025 did not have verification on file that Recognizing and Responding to Suspicions of Child Maltreatment training had been completed. .1102(g) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violation documented must be corrected immediately. On or before April 28, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. The 18 month compliance history, prior to today's visit, was 99%. A checklist was used to monitor the facility today. I met with David Gentry, Site Coordinator during today's visit. I was provided a notebook that had all the paperwork I needed to review during the annual compliance visit. The current Public School Off Site Verification Form for Staff Records, Children's Record and Transportation was signed and on file with each current staff listed and all current children enrolled listed for before school and after school. I reviewed the current ASEP bulletin board outside the cafeteria and observed all information required to be posted, except for the current activity plan. Each group leader had the teacher directed activity plan in their notebook on their activity care, however the second page of the activity plan, which lists the gross motor and the centers you will offer daily was not available for review. The last fire inspection was conducted on 6-12-23. The last sanitation inspection was conducted on 9-15-23. The emergency drill log was reviewed and found meeting compliance. I observed you and the other group leader set up centers for both groups in the cafeteria. Each group had six centers set up. As children entered they placed their belongings down, washed hands sat down and worked on homework. Snack was provided and children were allowed to play in centers. You showed me the outdoor environment that is used for outdoor time. You have two areas available for use. Currently the stationary equipment was observed with adequate depth of resilient surfacing. You have two children that have medication required for food allergies. Permission is on site and the medication is stored in the nurses office. You have access to the nurses office. You stated during field trips you will keep the medication and permission to administer in your travel back and keep the bag on your person. The following violation was observed today: Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan was observed in each group leader's notebook on the activity cart, it only had the teacher directed activity and not have page two which lists the gross motor activity and the centers listed that would be offered daily.
