At arrival I knocked on the door and once opened, I greeted Mrs. Haskins, owner, and stated the purpose of my visit. I conducted a walk-through of the facility partially assisted. This facility currently only operates one out of the two approved shift and has a five (5) star license. During the visit Ms. Haskins and I discussed that she was ready to apply and have her environmental rating scales scheduled. Due to the readiness this visit was changed to a rated License visit and the scales will be ordered once you complete your three month self-assessment. The following centers were available today, blocks, fine motor, dramatic play, and library. Children were observed playing with blocks and several staff child interactions about materials in the class, counting, and circle time. Children participated in outdoor gross motor play as well. Ms. Haskins gave the children several words of encouragement and praise. I conducted a walk-through of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. Rated License: Ms. Haskins last Annual Compliance visit was conducted on November 11, 2025. You currently have five stars in education. The Family and Community Engagement Standards Family Child Care Homes were identified, I will order your environmental rating scale as soon as you send me an email with your completed self-assessment. If you obtained a 4.0 or higher on your environmental rating scales you will maintain your five stars. I have received all other documents needed except the self-study. There were no violations observed during this visit. Violation Number Comment Rule 1735 The daily schedule, activity plan, infant feeding plan, allergy posting, SIDS Sleep Chart/visual check were not maintained on file for a minimum of 30 days from the revision or replacement date. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Upon arrival I was greeted by Ms. Haskins, owner/operator. I stated the purpose of my visit and proceeded with the monitoring visit. This facility currently with a five (5) star rated license effective 02-28- 2019. Children were observed actively engaged in activity centers, art, transitions from play to lunch. Ms. Haskins discussed letters, numbers, shapes, and colors during her circle time. Children participated in art/writing while they waited for lunch. Lunch consisted of turkey, pasta, green beans, oranges, bread, and milk. One child had lima beans as a meat substitute. After lunch children transitioned to toileting routines and nap time. INSPECTIONS: The last documented fire drill was conducted 11-1 -2024. The last documented shelter-in-place or lockdown drill was conducted 11-14 -2024. The last documented playground inspection was conducted 11-1-2024. The last compliance history prior to this visit was 97%. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility has been approved for transportation but currently she does not provide transportation. The attached checklist was used to note the requirements we monitored today. Violation Number Comment Rule 816 Each child's hands were not washed before and after eating. Children washed hands but played with toys and art before the food was served. .1725(a)(7)(B) 908 Health questionnaire was not completed annually.
