The last Annual Compliance visit was made on 09/20/23. The current 1 star license was issued on 5/27/2020. The March 2024 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 95% as of today. I was let into the facility by Ms. Hoffman. She let Ms. Humphries know that I was present. She accompanied me on the walk through of the facility. Facility files were made accessible to me. Eleven (11) classrooms were monitored on today for: Sign in/out, allergy list, medical action plans (if applicable), menu, schedule, activity plan, and staff/child ratio chart. Storage of medication and permission to administer was monitored. Supervision, and storage of hazardous products was monitored. Children were observed having indoor and outdoor free play, teacher directed activities, and having routine needs met. There were ample toys and activities available. The following was observed posted: current license, Summary of Law, Safe Arrival and Departure procedures, Menu, Emergency Care Plan, and no smoking signage. A Sanitation inspection was conducted on 07/11/2024 . A Fire inspection was conducted on 03/19/2024. Monthly Fire Drills, Playground Inspections, and EPR Drills were monitored. A sample of staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. A sample of children's files were monitored for Application, Emergency Care authorization, Summary of Law, Medical/Immunization, Discipline Statement, No smoking statement, Safe Sleep Policy, Infant Feeding Schedule and Shaken Baby Syndrome Policy (if applicable). This facility does not provide transportation. The following violations were cited during today's visit: Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Three (3) diaper creams, and one (1) sunscreen did not have written authorization and instructions from a child's parent or authorized health professional. 10A NCAC 09 .0803(1)(a & b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One (1) Vaseline authorization and one (1) diaper cream authorization had expired. .0803(12) Compliance -Violations cited today must be corrected immediately. Please send me a letter documenting how violations cited today were corrected by September 26, 2024. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Rated License An application and documentation to apply for higher stars was received from this facility. Once violations from today's visit have been corrected and the pathway chosen has been verified, a rated license packet will be submitted. The last Annual Compliance visit was made on 10/03/22. The current 1 star license was issued on 05/27/20. The June 2022 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 93% as of today. I was let into the facility by Kristina Hoffman. I was introduced to owner Prachi Ghadi, and director Blenda Humphries. A walk through of the facility was completed. Eleven (11) classrooms were monitored on today for: Sign in/out, allergy list, medical action plans (if applicable), menu, schedule, activity plan, and staff/child ratio chart. Children were observed having indoor/outdoor free play and having routine needs met. There were ample toys and activities available. A sample of staff files including seven (7) new staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. A sample of children's files were monitored for Application, Emergency Care authorization, Summary of Law, Medical/Immunization, Discipline Statement, No smoking statement, Safe Sleep Policy, Infant Feeding Schedule and Shaken Baby Syndrome Policy (if applicable). This facility does not provide transportation. Storage of medication and authorization to administer medication was monitored. The kitchen, bathrooms, and outdoor play areas used by children were monitored today. The following violations were cited during today's visit: Violation Number Comment Rule 1054 Documentation of staff's on-going training was not on file and/or was not current .One (1) child care provider did not have documentation of on-going training on file. 10A NCAC 09 .1106(a) Compliance -Violations cited today must be corrected immediately. Technical Assistance Some of your staff have taken the NCEC equivalency exam on Moodle. Testing out does not count towards education for ongoing training. If an individual tests out and does not have a degree or credentials they still need 20 hours of ongoing training unless they have 10+ years experience. The ongoing training chart specifies North Carolina Early Childhood Credential for 10 clock hours, NOT EQUIVALENCY. Therefore, if someone tests out of credentials, their training hours would be based on hours worked per week unless they have 10 years’ experience.
