I was greeted by Owner/operator, Mrs. Pamela Withers and I explained the purpose of the visit. Mrs. Wither's daughter was present in the home during the visit. The facility had a Three-Star License issued January 8, 2019, and an eighteen-month compliance history score of ninety-five (95) percent prior to today’s visit. There were five (5) children present upon arrival. I observed Mrs. Withers reading a book to the children, Black Inventers by King Kiel, to the children in the outdoor play area. There are no children enrolled on second shift. The last annual compliance visit was conducted September 12, 2024. The following child care requirements were monitored and observed today using the Family Child Care Home Checklist and the 2025 Master Family Child Care Home Item Number Listing: Supervision: Adequate supervision was provided. Nurturing tones were heard when Mrs. Withers spoke with the children. Staff/Child Ratio: Compliance was met with staff/child ratio. CPR: Mrs. Withers was current with CPR. First Aid: Mrs. Withers was current First Aid. Special Training: Mrs. Withers is continuing to complete required in-service training. ITS-SIDS: Mrs. Withers is current with ITS-SIDS training; no infants are enrolled. The SIDS Policy and SIDS poster were posted. CBC Qualification: Mrs. Withers was current with CBC qualification; and the family members that reside in the home were current with CBC qualification. A violation was cited for the roster that has not been completed in ABCMS. Emergency Medical Care Plan: The Emergency Care Plan was posted. Administration of Medication: There are no children with medication; no children with allergies. Storage of Hazardous Substances: I did not observe any hazardous substances that were accessible, they were observed stored properly during the visit. Storage of Medication: Not applicable. General Safety: There were no safety concerns. Discipline: There were not concerns with discipline; appropriate discipline was provided. Adequate/Approved Space: Not able to monitor. Program Records: I reviewed all information required to be posted and found meeting compliance. The monthly fire drill and emergency drills, the shelter-in-place/lockdown drills were reviewed; a violation was cited for the shelter-in-place/lockdown drills. The playground inspections were up to date; compliance was met. License Posted: The license was observed posted on the information board. Permit Restrictions: Permit restrictions were met. The Sanitation Inspection was conducted August 1, 2024 with a demerit score of six (6). The facility's water was tested January 7, 2025. The Emergency Preparedness and Response Plan was reviewed; lasted updated September 12, 2024. Two (2) violations were cited. The roster was not complete for ABCMS. G.S. 110-90.2 & .2703(r) 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The quarterly lockdown or shelter-in-place drill was not completed at least every three (3) months; completed September 16, 2024, conducted January 3, 2025. .1719(a )(16) & .1721(e )(7) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 2, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. The following Technical Assistance (TA) was provided: -Family child care home operators must conduct a shelter-in-place or lockdown drill, as defined in 10A NCAC 09 .0102, at least every three (3) months, Child Care Rule .1719(a)(16). You must document shelter-in-place and lockdown drills. The record must include the date of each drill, time of day, the length of time to reach the designated location and the signature of the person conducting the drill. Child Care Rule .1721(e)(7). -CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance: • As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. • The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. •Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. • Please note, the hard copy of the Change of Information form will no longer be needed or accepted. The program also reviews child care facilities’ documentation and other information to determine and coordinate any steps needed to identify and eliminate lead-based paint and asbestos hazards (10A NCAC 41C.1001-1007). Licensed child care centers operating within a public school (i.e., afterschool programs, Head Starts, Pre-K, etc.) should coordinate with school staff to complete the lead in water section of the program so the entire school can be tested at once; tested January 7, 2025. Ms. Withers submitted the water samples in December 2024. -Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions. Visit the NCRLAP’s website for more information about updated resources, credit hour trainings, and outreach assessment opportunities to help you become familiar with these tools. For official environment rating scale assessments for a NC Star Rated License, the NCRLAP will use the Revised editions until February 1, 2025. -NCID Password Rule to Change If you have a MYNCID or NCID account, you may have received an email this week announcing changes to the password rules. As part of ongoing efforts to protect state systems and data, the N.C. Department of Information Technology will be changing the minimum length of NCID passwords from eight to 14 characters. The change will affect state agency staff as well as external users like citizens, providers, and state and local county staff. There is no action you need to take on or before February 23rd. When you change your current password after Feb. 23, however, you will be required to create a new one that must: • Be a minimum of 14 characters. • Consist of at least one numeric character, at least one uppercase letter, at least one lowercase letter, and at least one special character such as #, &, * etc. • Passwords shall not contain number or character substitutes to create dictionary words (e.g., d33psl33p for deep sleep). I was greeted by Owner/operator, Mrs. Pamela Withers and I explained the purpose of the visit. Mrs. Wither's daughter was present in the home during the visit. The facility had a Three-Star License issued January 8, 2019, and an eighteen-month compliance history score of ninety-five (95) percent prior to today’s visit. There were five (5) children present upon arrival. I observed Mrs. Withers reading a book to the children, Black Inventers by King Kiel, to the children in the outdoor play area. There are no children enrolled on second shift. The last annual compliance visit was conducted September 12, 2024. The following child care requirements were monitored and observed today using the Family Child Care Home Checklist and the 2025 Master Family Child Care Home Item Number Listing: Supervision: Adequate supervision was provided. Nurturing tones were heard when Mrs. Withers spoke with the children. Staff/Child Ratio: Compliance was met with staff/child ratio. CPR: Mrs. Withers was current with CPR. First Aid: Mrs. Withers was current First Aid. Special Training: Mrs. Withers is continuing to complete required in-service training. ITS-SIDS: Mrs. Withers is current with ITS-SIDS training; no infants are enrolled. The SIDS Policy and SIDS poster were posted. CBC Qualification: Mrs. Withers was current with CBC qualification; and the family members that reside in the home were current with CBC qualification. A violation was cited for the roster that has not been completed in ABCMS. Emergency Medical Care Plan: The Emergency Care Plan was posted. Administration of Medication: There are no children with medication; no children with allergies. Storage of Hazardous Substances: I did not observe any hazardous substances that were accessible, they were observed stored properly during the visit. Storage of Medication: Not applicable. There were no children present during upon arrival. Five (5) children arrived to the facility, due to an early school release day. Upon my arrival, I observed the children completing an afternoon snack. After snack, I observed the children washing their hands. I observed Mrs. Withers-Streeter assisting schoolage children with homework. Mrs. Withers-Street does not have any children enrolled on second shift. The facility currently holds a Three Star License issued January 8, 2019. The last annual compliance visit was conducted September 18, 2023. The sanitation inspection was completed August 1, 2024 with a Demerit score of two (2). The Emergency Preparedness and Response Plan (EPR) was reviewed; plan updated August 21, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight (98) percent as of September 11, 2024. The NC Secretary of State website was reviewed on September 11, 2024. The facility is not incorporated, and Mrs. Withers-Streeter is an "individual" owner. I completed a walkthrough of the indoor and outdoor areas of the home. The facility was monitored today with the Family Child Care Home Monitoring Checklist and the Family Child Care Home Item Number Listing. A copy of the checklist was provided to Ms. Withers-Streeter after the visit. The following was observed posted: Current License, NC Summary of Child Care Law, Schedule, Activity Plan, Emergency Numbers, Emergency Medical Care Plan, First Aid Information Sheet, and the Incident log. The Monthly Fire Drills, Shelter-in-Place/Lockdown Drills, and the Playground Inspections were reviewed; a violation was cited for the Shelter-in-Place/Lockdown Drill. Mrs. Withers-Streeter is using an approved curriculum, Creative Curriculum. The daily attendance and arrival and departure times were reviewed and met compliance. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit. Soap and toilet paper were available in the bathroom for use. The toys and equipment were in good repair. In the kitchen, there was a working fire extinguisher, working thermometer in the refrigerator, and smoke detectors. A violation was cited for air freshener. The outdoor play area was clean and the toys were in good repair. Two (2) outdoor sheds were unlocked and a violation was cited. Mrs. Withers-Streeter file was monitored and observed for Health Questionnaire, Emergency Information, First Aid and CPR, and Criminal Qualifying Letters. Mrs. Withers-Streeter met the required number of training hours; compliance met. Mrs. Withers-Streeter's husband and daughter have a current criminal qualifying letter on file. The children’s files were monitored today, and the children's records met compliance. Mrs. Withers-Streeter does have pets (dogs); compliance was met. There are no children enrolled with allergies, and there are no children on medication or emergency medication. Mrs. Withers-Streeter does provide transportation. Both vehicles used to transport children are insured. One (1) vehicle that is used to transport children was not available to monitor. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight (98) percent as of September 11, 2024. The NC Secretary of State website was reviewed on September 11, 2024. The facility is not incorporated, and Mrs. Withers-Streeter is an "individual" owner. I completed a walkthrough of the indoor and outdoor areas of the home. I observed Mrs. Withers-Streeter assisting schoolage children with homework. Mrs. Withers-Street does not have any children enrolled on second shift. The facility currently holds a Three Star License issued January 8, 2019. The last annual compliance visit was conducted September 18, 2023. The sanitation inspection was completed August 1, 2024 with a Demerit score of two (2). The Emergency Preparedness and Response Plan (EPR) was reviewed; plan updated August 21, 2024. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety eight (98) percent as of September 11, 2024. The NC Secretary of State website was reviewed on September 11, 2024. The facility is not incorporated, and Mrs. Withers-Streeter is an "individual" owner. I completed a walkthrough of the indoor and outdoor areas of the home. The facility was monitored today with the Family Child Care Home Monitoring Checklist and the Family Child Care Home Item Number Listing. A copy of the checklist was provided to Ms. Withers-Streeter after the visit. The following was observed posted: Current License, NC Summary of Child Care Law, Schedule, Activity Plan, Emergency Numbers, Emergency Medical Care Plan, First Aid Information Sheet, and the Incident log. The daily attendance and arrival and departure times were reviewed and met compliance. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit. Soap and toilet paper were available in the bathroom for use. The toys and equipment were in good repair. In the kitchen, there was a working fire extinguisher, working thermometer in the refrigerator, and working smoke detectors. The Safe Sleep Log was reviewed and compliance was met. The Safe Sleep Poster and Safe Sleep Policy was not posted and a violation was cited. The Infant Feeding Chart was reviewed and compliance was met. The outdoor play area was clean and the toys were in good repair. Ms. Wither's file was monitored and observed for Health Questionnaire, Emergency Information, First Aid and CPR, ITS-SIDS Training, and Criminal Qualifying Letters; Ms. Wither's husband and daughter has a current CBC letter; no one else resides in the home. Ms. Withers met the required number of ongoing training hours. The children’s files were monitored today, and a violation was cited for a child's file. A violation was cited for Ms. Wither's dog's expired vaccination. There are no children on medication or emergency medication; children with allergies were posted. Ms. Withers does provide transportation, and the vehicle was monitored for Child Safety Restraints and Seat Belts, Vehicle Safety Inspection and Liability Insurance, First Aid Kit in Vehicle, and Emergency Information on Children; compliance was met. The Written Plan of care was verified. The Sanitation Inspection was conducted July 31, 2023 with a demerit score of six (6). Five (5) violations were cited.
