This center has a five star license and an 18 month compliance history score of 96%. The license and NC child care law summary were prominently posted. Upon arrival, I was greeted by T. Conway, director. It was shared with that Environmental health was also present during the visit. Ms. Conway accompanied me during the walkthrough of the facility. There were 53 children present during the walkthrough of the facility. During the visit, I spent time observing space #1, the classroom for infants. I observed the infants enjoying outdoor play. I spent time observing space #5, the classroom for children three years of age. I observed the children playing in centers. I observed a small participating in an art activity with a teacher. The following items were monitored today: supervision, staff/child ratios, permit restrictions, license posted, approved space, storage of hazardous products, storage of medication, staff records and program records. There have been 4 new staff since my last visit. I monitored all new staff files. The staff and training worksheets were reviewed to confirm existing staff were still current with CPR, First Aid, ITS-SIDS training and criminal background qualifying letters. The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required. Shelter-in-place and Lock-Down drills were documented quarterly as required. The playground safety inspections were reviewed. Reports were documented monthly as required. There was 1 violation cited during today’s visit. The violation was corrected during the visit. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. In space #2, there was torn padding on a teacher's chair. The facility currently has a five-star license and an 18 month compliance history of 97%. The license and NC childcare law summary were prominently posted. The Secretary of State database was monitored and PLESANT GOVE UNITED METHODIST CHURCH is active and current. I was greeted by T. Conway, director. Ms. Conway accompanied me during the walkthrough of the facility. Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required. Shelter in place and/or lockdown drills were documented as required. The playground safety inspections were reviewed and documented. The center last had a sanitation inspection on 1/13/25. The center received 4 demerits and received superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 12/13/24. There were 65 children in attendance today during the walkthrough. I observed the children participating in free play, eating, holding, routines and transitions. The playground was monitored during today’s visit. The facility does not provide transportation. I monitored all new staff files since my last visit and 10 percent of existing staff files were monitored today. Ten percent of the enrolled children’s files were monitored at random today. There were 2 violations cited today. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. In space #6, a child's medication has expired permission to give dates. 10A NCAC 09 .0803(1)(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. There was no documentation on file that a staff member completed the training within the required timeframe. .1102(g) Technical Assistance: -Hold Harmless has been extended until the new QRIS is implemented (SB 425). -Use this time for staff to update their information in the WORKS database. Teri Conway, administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Conway and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 904 Child was handled roughly. On July 21, 2023, a teacher aggressively grabbed a two-year-old child and threw the child onto a mat causing the child’s lip to bust and bleed. The teacher then kicked the mat the child was laying on and it slid across the floor. .1803(a)(1) 1810 There was a substantiation of child maltreatment. Pursuant to its investigation, the Division has confirmed sufficient information to determine child maltreatment. GS 110-105.6(a) Violations must be corrected immediately.
