The last annual compliance visit was made on 10/16/2024. The current 5 star rated license was issued 12/16/2022. The April 2025 version of the Child Care Center Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 97% as of today. I was let into the school by the front office staff. I walked down to the cafeteria. Program records, staff records, and children's records were made accessible to me. The following was observed posted: Emergency Care Plan, Safe Drop off/Pick up, Current License, Summary of Law, Menu, Activity Plan, and Schedule. Fire Drills, and Emergency Preparedness Drills were monitored today. Attendance and Sign In/Out was monitored. An Offsite Staff Verification form, Off site transportation form, and Offsite Children's Verification form was monitored. All used licensed spaces, outdoor play area, and bathrooms used by the children were monitored today. Children arrived at the cafeteria as school let out. They washed hands. Group three (3) had snack and are scheduled to go outside afterwards then homework. Groups one (1) and two (2) used the restroom. Then they had snack and will do homework afterwards. They will go outside after group three comes in. The following violations were cited during today's visit: Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. A current menu was not posted. 10A NCAC 09 .0901(b) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One (1) staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) 1921 Staff working in part-time, full-day or track out school age care programs required to complete BSAC training, did not complete it within three months of employment. One staff member did not complete BSAC within three months of employment. .2510(j) Compliance -Violations cited today must be corrected immediately. Please send me a letter documenting how violations cited today were corrected by October 6, 2025. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. Failure to correct the violations and/or submit the compliance letter to me by the due date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Technical Assistance You stated that there are field trips scheduled for this year. Please ensure that photos of the children enrolled are attached to the emergency information you take with you. The last annual compliance visit was made on 11/18/2022. The current 5 star rated license was issued 12/16/2022. The August 2023 version of the Child Care Center Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 100% as of today. I was greeted by Site Coordinator, Sherri McCullough. She accompanied me to the cafeteria. Program records, staff records, and children's records were made accessible to me. The following was observed posted: Emergency Care Plan, Safe Drop off/Pick up, Current License, Summary of Law, Menu, Activity Plan, and Schedule. A Sanitation Inspection was completed on 9/25/2023. A Fire Inspection was completed on 4/27/2023. Storage of medication and permission to administer requirements were monitored for students enrolled with allergies. Fire Drills, Outdoor Playground Inspections, and Emergency Preparedness Drills were monitored today. Attendance and Sign In/Out was monitored. An Offsite Staff Verification form and Offsite Children's Verification form. All used licensed spaces, outdoor play area, and bathrooms used by the children were monitored today. This facility does not use the playground. They have outdoor play in the courtyard. Children were observed washing their hands and having snack. Then Group 1 is scheduled to complete homework or center play while group 2 have outdoor play. Then the groups will switch. The following violations were cited during today's visit: Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. EMC plan was not reviewed with all staff annually. 10A NCAC 09 .0802(a) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. Documentation of the staff's annual review of the EPR plan was not available. .0607(f) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment.
