I was greeted by Group Leader, Ms. Roslyn Bradley, and I explained the purpose of the visit. The Program Assistant Ms. Janet Harrison arrived during the visit. The facility had a Five-Star License issued September 10, 2019, and an eighteen-month compliance history score of ninety-seven (97) percent prior to today’s visit. The last annual compliance visit was conducted September 3, 2025, I completed an indoor walkthrough of the program with Ms. Bradley. Upon arrival, I observed the following activity centers: Manipulation, Dramatic Play, Math, Art, and Relaxation. Each center had adequate materials, and materials were in good repair. I observed other children's arrival to the program at 4:15 p.m., in the cafeteria area. After arrival and attendance, the children prepared for snack by washing their hands. I observed the children served Cheez-its, Mozzarella Cheese String, Unflavored Skim Milk, 1% Milk and or 100% Juice 6 oz. After snack, the children washed their hands and returned to their group. There were three (3) groups of children observed with staff during the visit. The program was monitored with the AC Monitoring Centers Checklist and the Child Care Center Item Number Listing. A copy of the AC Checklist was provided after the visit. I observed the following items posted: License, No Smoking Sign, Summary of NC Law, Menu, a current Activity Plan (PBIS ASEP Activity Plan), ASEP Schedule, First Aid Poster, the Emergency Medical Care Plan, Sanitation Procedures (Vomit or Diarrhea Clean-up Procedures), and the CMS Crisis Plan. The children's records (DPI) were reviewed and compliance was met. Staff records were reviewed with the Staff and Training Worksheet, One (1) new staff have been hired; staff's file was reviewed. The program's roster was complete in ABCMS. Ten (10) percent of veteran files were reviewed; a violation was cited for the health questionnaire form and a violation was cited for the emergency information form; Violations were cited for First and CPR. I did not observe and hazardous products and bio contaminants improperly stored; compliance was met. Medications were reviewed, and a violation was cited for an expired Permission to Administer Medication form. Five (5) violations were cited. Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. A health questionnaire was not on file for the new staff member; the facility's DPI form was not signed .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. A new staff member did not have an emergency information form completed; the facility's DPI form was not signed. .0701(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff's (CM) First Aid expired January 2026 .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A staff (CM) CPR expired January 2026 .1102(d) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. On or before April 7, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. •Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The implementation plan will allow time for administrators/operators to learn and understand the different pathways and the changes within our system. These third editions come with a spiral binding at the top, replacing the current revised editions. There are different tracks for participation (Licensing renewal, Program start-up, staff-child interactions and director support); and there are new opportunities this year like updated ERS-3 trainings, Leadership college coursework for administrators and Communities of Practice for both Child Care Directors and Family Child Care providers. They still have opportunities for college coursework and stipends for coursework completion. Technical Assistance was provided on the QRIS Pathway to the Stars. The last annual compliance visit was conducted September 3, 2025, I completed an indoor walkthrough of the program with Ms. Bradley. Upon arrival, I observed the following activity centers: Manipulation, Dramatic Play, Math, Art, and Relaxation. Each center had adequate materials, and materials were in good repair. I observed other children's arrival to the program at 4:15 p.m., in the cafeteria area. After arrival and attendance, the children prepared for snack by washing their hands. I observed the children served Cheez-its, Mozzarella Cheese String, Unflavored Skim Milk, 1% Milk and or 100% Juice 6 oz. After snack, the children washed their hands and returned to their group. There were three (3) groups of children observed with staff during the visit. The program was monitored with the AC Monitoring Centers Checklist and the Child Care Center Item Number Listing. A copy of the AC Checklist was provided after the visit. I observed the following items posted: License, No Smoking Sign, Summary of NC Law, Menu, a current Activity Plan (PBIS ASEP Activity Plan), ASEP Schedule, First Aid Poster, the Emergency Medical Care Plan, Sanitation Procedures (Vomit or Diarrhea Clean-up Procedures), and the CMS Crisis Plan. The children's records (DPI) were reviewed and compliance was met. Staff records were reviewed with the Staff and Training Worksheet, One (1) new staff have been hired; staff's file was reviewed. The last fire inspection was conducted May 9, 2025. I completed an indoor walkthrough of the program with Ms. Neal. Upon arrival, I observed the following activity centers: Manipulation, Dramatic Play, Math, Art, and Relaxation. Each center had adequate materials, and materials were in good repair. I observed other children's arrival to the program at 3:15 p.m., in the cafeteria area. After arrival and attendance, the children prepared for snack by washing their hands. I observed the children Doritos Unflavored Skim Milk, 1% Milk and or 100% Juice 6 oz. After snack, the children washed their hands and returned to their group. The last annual compliance visit was conducted September 6, 2024. The sanitation inspection was completed February 26, 2025, with a “Superior” classification and three (3) demerits. The last fire inspection was conducted May 9, 2025. I completed an indoor walkthrough of the program with Ms. Neal. Upon arrival, I observed the following activity centers: Manipulation, Dramatic Play, Math, Art, and Relaxation. Each center had adequate materials, and materials were in good repair. I observed other children's arrival to the program at 3:15 p.m., in the cafeteria area. After arrival and attendance, the children prepared for snack by washing their hands. I observed the children Doritos Unflavored Skim Milk, 1% Milk and or 100% Juice 6 oz. After snack, the children washed their hands and returned to their group. There were three (3) groups of children observed with staff during the visit. The program was monitored with the AC Monitoring Centers Checklist and the Child Care Center Item Number Listing. A copy of the AC Checklist was provided to Ms. Neal after the visit. I observed the following items posted: License, No Smoking Sign, Summary of NC Law, Menu, a current Activity Plan (PBIS ASEP Activity Plan), ASEP Schedule, First Aid Poster, the Emergency Medical Care Plan, Sanitation Procedures (Vomit or Diarrhea Clean-up Procedures), and the CMS Crisis Plan. Refer to Child Care Rule .0803(6)(a-i) for permission to administer for chronic health conditions: A parent may give a caregiver standing authorization for up to six (6) months to administer prescription or over-the counter mediation to a child, when needed, for chronic medical conditions, such as asthma, and for allergic reactions. The authorization shall be in writing and shall contain: the child's name, the subject medical condition or allergic reactions, the names of the authorized over-the counter medications, the criteria for the administration of the medication, the amount and frequency of the dosages, the manner in which the medication shall be administered, the signature of the parent, the date the authorization was signed by the parent, and the length of time the authorization is valid, if less than six (6) months. -Emergency medications must be on hand for child(ren) prescribed emergency medication(s). I was greeted by ASEP Program Coordinator, Jaslynn Neal, and I explained the purpose of today's visit. The program has a Five-Star Rated License issued September 10, 2019 and has a compliance history score of 95%. The last Annual Compliance Visit was completed on September 21, 2022. The Annual Compliance Monitoring Checklist for Child Care Centers was used to note the requirements I monitored today. A copy of the checklist was provided to Ms. Neal after the visit. Upon arrival, there were no children present. In Space D10, Group 1, I observed the following centers: Art, Math, Writing and Homework, Reading, and Dramatic Play. In Space D9, Group 2, I observed the following centers: Writing and Homework, Math, Art, Science and Social Studies, and Dramatic Play. The materials in the centers were in good repair. At 3:15 p.m. I observed the ASEP staff meeting the children for arrival to the ASEP Program and attendance for each group was noted by ASEP staff. The children used the restrooms and washed hands. The children prepared to go outside for outdoor play. After outdoor play, the children returned for snack with items noted on the menu: Little Squares Graham Crackers, Milk, and Juice. The children's files were reviewed today with the children's records form. The DPI forms were completed for the children's files. Each file was found to maintain all the required documents. Staff files were reviewed and met compliance. Current existing staff files were reviewed and were compliant. The staff files were reviewed using the the staff and training worksheets and staff files. The DPI forms were completed for the staff files. All required items were posted. Fire Drill logs were complete and up to date. A violation was cited for the Shelter-in-Place/Lockdown Drills. Playground Inspections were reviewed, and the logs and inspections were up to date. The outdoor play area area was in good repair; mulch was added. A variety of outdoor play equipment such as balls, jump ropes, and other outdoor games was available for outdoor play on the blacktop area. On the blacktop area, there are basketball goals available for play. All medications were observed properly stored and permissions and medical and actions plans were current; children with allergies were posted. - The last Sanitation Inspection was conducted on May 16, 2023, with a Superior rating and zero (0) demerits. - The last Fire Inspection was completed May 24, 2021, and a violation was cited. Two (2) violations were cited. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection.
