Your program currently operates a Four Star Rated License with an effective date of July 24, 2023. The program’s compliance history before today’s visit was 97%. Upon my arrival, I checked into the school’s main office. The program’s Program Coordinator, L. Love, greeted me in the office. I stated the reason for the visit to Ms. Love. The following items were monitored: supervision, staff/child ratio, CPR, First Aid, BSAC training, Playground Safety training, criminal background qualifying letters, EMC plan, administering medication, storage of hazardous products/medications, general safety, discipline, adequate/approved space, program records, license posted and permit restrictions. The program uses the cafeteria as the primary space. Activity areas were set up in the cafeteria today. Activity areas included science, math, writing, career development, arts, global world culture, career development, homework and books. Today, children were observed participating in arrival procedures, personal care routines, snack and free choice of indoor activities. Snack consisted of chocolate simply Chex snack mix and 100% apple juice. Staff were observed supervising and interacting with the children in a caring and nurturing manner. There have been no new staff hired since the last annual compliance visit conducted on January 8, 2025. The staff and training worksheet was used to confirm staff were current with First Aid, CPR, and criminal background checks. The worksheet indicated a staff member’s criminal background qualifying letter expired February 2025 however, I reviewed the staff member’s file, and a qualifying letter was received February 5, 2025. A fire inspection was conducted on August 6, 2024. A sanitation inspection was conducted November 27, 2024, and the facility received a “Superior” classification. A lockdown drill was conducted on January 6, 2025. The last fire drill was conducted on April 7, 2025. There was one violation observed today. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last shelter-in-place drill or lockdown drill was conducted January 6, 2025. .0604(u);.0302(d)(8) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. The facility currently operates with a four-star license, issued July 24, 2023, earning seven points in the education component, four points in the program standards component and one quality point by offering a staff benefits package, infrastructure of parent involvement and having approved enhanced policies. Today, I observed children participating in arrival procedures, routines, homework activities, and snack. Snack consisted of whole grain Cheez-Its, mozzarella string cheese and 100% apple juice. Upon arrival of the children, the Program Coordinator was by herself with thirty-two children present from 2:52PM to 3:01PM. The children were five years of age to ten years of age. The children use the cafeteria as their primary space. Activity areas such as reading, writing, art, math, science, career development and global world culture were set up in the cafeteria. A sanitation inspection was conducted November 27, 2024, with a “Superior” classification. The last fire inspection was conducted on August 6, 2024. A fire drill and lockdown drill were conducted January 6, 2025. Playground safety checklists were monitored and occurring monthly as required. Public School Off-Site Records Verification forms for staff and children for the 2024-2025 school year were reviewed today. The School Principal was notified and allowed the substitute to leave her afternoon duties to assist with the after-school program. -DPI Verification Forms- A discussion was held with the Program Coordinator on how to document when children are enrolled or disenrolled on the Children’s Records Verification form. Children’s names may be crossed out with the date of their last attendance. -ABCMS (Criminal Background System): North Carolina child care administrators can now view and edit their facility’s staff roster in ABCMS, the DCDEE’s criminal background check system. Provider Access to ABCMS allows administrators to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. The facility’s 18 month compliance history, prior to today’s visit was 100%. A checklist was used to monitor the facility. Upon my arrival, I met with LaTanya Love, Site Coordinator. I explained the purpose of today’s visit. You provided me with a notebook you have organized with information required to be maintained for licensing purposes. The current Public School Off Site Verification Form for Staff Records, Children's Records and Transportation was on file. Current staff and children, per groups, are listed on the appropriate forms. I completed the staff and training worksheet today, while reviewing staff files today, no violations were observed. Monthly playground inspections were reviewed and found meeting compliance. The current incident log was reviewed and found meeting compliance. The last fire inspection was conducted on 7/10/23. The last sanitation inspection was conducted on 10/12/23 with a superior rating. The emergency drill log was reviewed and found meeting compliance. I monitored the ASEP bulletin board. Information required to be posted was observed posted, except for current activity plans. The activity plan posted for group 1 and group 2 was dated January 4, 2024. For outdoor time, you use the patio area outside the cafeteria, you have a container of gross motor material you take with you for outdoor time. Children can choose what gross motor material to use while outdoors and your provide a teacher directed gross motor activity. I monitored children's arrival from day school. Daily attendance was reviewed and found meeting compliance. After arrival, children wash hands and are provided a snack. After snack, hands are washed again and children can play freely in centers and have homework time. They go outdoors towards the later part of the afternoon. Material and and equipment in each center were monitored per group and found to be in good repair, developmentally appropriate for the age range served. Four centers are set up for each group of children. Supervision and staff ratio were maintained during today's visit. Staff were heard using nurturing tones when speaking with children. Currently, there are no children enrolled required to have medication administered. You have not gone on a field trip but have one planned for January 25th. We discussed the emergency information you will be taking with you on each child enrolled in your program. You will get permission for each child as it gets closer to going on the field trip. A current activity plan was not posted for either group today. The plan posted for Group 1 and Group 2 was dated January 4, 2024. GS 110-91(12); .0508(a) Childcare providers are expected to maintain compliance with all applicable child care requirements at all times. All violations cited today shall be corrected immediately. LaTanya Love, Site Coordinator, will email a compliance letter explaining how each violation cited today has been corrected. The compliance letter must be received by the consultant, andrea.anderson@dhhs.nc.gov, on or before January 30, 2024. Failure to correct the violations and/or submit the compliance letter to me by the date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Technical Assistance was provided on the following: Activity Plan: I believe CMS has a portal with internal documents where you can find activity plans. The ASEP activity plans typically have two pages the TDA page and then a gross motor page and at the bottom of that page you will make the four interest centers you will offer each day of that week and post both pages on the board for easy review. DCDEE has a sample school age activity plan if you are interested in using that plan. DCDEE website is www.https://ncchildcare.ncdhhs.gov Health and Safety Training: Upon hire a new employee has the first year to obtain health and safety training. DCDEE Moodle is not the only place to obtain training, it is free training. Currently NCID is having issues, so you may want to find other approved training sources to obtain the training. Within the first 90 days of hire you must obtain CPR/FA certification if you don't have valid CPR/FA training and you must obtain Recognizing and Responding to Suspicions of Maltreatment from https://www.preventchildabusenc.org/. You will need to make sure to maintain the health and safety training log, keeping the training certificates with the training log. If you are hired and have completed approved health and safety training within a year of hire, you may count those trainings. CCRI,Inc and the Mecklenburg County Community Health Care Nurses provided Health and Safety Trainings, you can find trainings on the CCRI training calendar online at https://www.childcareresourcesinc.org/ and community health care nurse trainings Health@MeckNC.gov or https://healthychildcare.unc.edu/ is another website to find trainings. The facility’s 18 month compliance history, prior to today’s visit was 100%. A checklist was used to monitor the facility. Upon my arrival, I met with LaTanya Love, Site Coordinator. I explained the purpose of today’s visit. You provided me with a notebook you have organized with information required to be maintained for licensing purposes. The current Public School Off Site Verification Form for Staff Records, Children's Records and Transportation was on file. Current staff and children, per groups, are listed on the appropriate forms. I completed the staff and training worksheet today, while reviewing staff files today, no violations were observed. Monthly playground inspections were reviewed and found meeting compliance. The current incident log was reviewed and found meeting compliance. The last fire inspection was conducted on 7/10/23. The last sanitation inspection was conducted on 10/12/23 with a superior rating. The emergency drill log was reviewed and found meeting compliance. I monitored the ASEP bulletin board. Information required to be posted was observed posted, except for current activity plans. The activity plan posted for group 1 and group 2 was dated January 4, 2024. For outdoor time, you use the patio area outside the cafeteria, you have a container of gross motor material you take with you for outdoor time. Children can choose what gross motor material to use while outdoors and your provide a teacher directed gross motor activity. I monitored children's arrival from day school. Daily attendance was reviewed and found meeting compliance. After arrival, children wash hands and are provided a snack. After snack, hands are washed again and children can play freely in centers and have homework time. They go outdoors towards the later part of the afternoon. Material and and equipment in each center were monitored per group and found to be in good repair, developmentally appropriate for the age range served. Four centers are set up for each group of children. Supervision and staff ratio were maintained during today's visit. Staff were heard using nurturing tones when speaking with children. Currently, there are no children enrolled required to have medication administered. You have not gone on a field trip but have one planned for January 25th. We discussed the emergency information you will be taking with you on each child enrolled in your program. You will get permission for each child as it gets closer to going on the field trip. The following violation observed during today’s visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current activity plan was not posted for either group today. The plan posted for Group 1 and Group 2 was dated January 4, 2024. GS 110-91(12); .0508(a) Childcare providers are expected to maintain compliance with all applicable child care requirements at all times. All violations cited today shall be corrected immediately. LaTanya Love, Site Coordinator, will email a compliance letter explaining how each violation cited today has been corrected. The compliance letter must be received by the consultant, andrea.anderson@dhhs.nc.gov, on or before January 30, 2024. Failure to correct the violations and/or submit the compliance letter to me by the date listed above may result in an unannounced follow-up visit being conducted to ensure corrections were made. Technical Assistance was provided on the following: Activity Plan: I believe CMS has a portal with internal documents where you can find activity plans. The ASEP activity plans typically have two pages the TDA page and then a gross motor page and at the bottom of that page you will make the four interest centers you will offer each day of that week and post both pages on the board for easy review. DCDEE has a sample school age activity plan if you are interested in using that plan. DCDEE website is www.https://ncchildcare.ncdhhs.gov Health and Safety Training: Upon hire a new employee has the first year to obtain health and safety training. DCDEE Moodle is not the only place to obtain training, it is free training. Currently NCID is having issues, so you may want to find other approved training sources to obtain the training. Within the first 90 days of hire you must obtain CPR/FA certification if you don't have valid CPR/FA training and you must obtain Recognizing and Responding to Suspicions of Maltreatment from https://www.preventchildabusenc.org/. You will need to make sure to maintain the health and safety training log, keeping the training certificates with the training log. If you are hired and have completed approved health and safety training within a year of hire, you may count those trainings.
