    Name of Operation: OAKHURST ELEMENTARY ASEP Facility ID: 60003687 Consultant: MARA BRINTON Operation Type: Center Case Number: Visit Date: 5/31/2024 Number Present: 14 Completed Date: 5/31/2024 Age: From 5 To 11 Total Minutes: 165 Time In: 02:45 PM Time Out: 05:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor for compliance with applicable child care requirements during a Routine Unannounced Visit. Upon arrival at the five-star rated license afterschool program, the program coordinator, Ms. Vanesa Moultrie was present and greeted me in the school office. Ms. Moultrie and I went to her office due to a school talent show taking place during day school. The child care item number listing dated March 2024 was used to identify and determine non-compliance. The following sections were monitored for compliance: A1, B1, C4, C5, C6, E1, E2, F3, G1, G3, G4 and G5. The cafeteria, and the outdoor learning environment were completed with Ms. Moultrie. There was one new staff member hired since the last visit. The previous staff and training worksheets were reviewed. Ms. Moultrie and I reviewed previous staff and training documentation to show existing staff were current with CPR, FA, CMT training. All staff were monitored with current CBC’s. Monthly outdoor inspections, incident log, monthly fire drills and quarterly safety drills were monitored completed and current. Children were monitored eating their PM snack yogurt, vegetable juice, apple sauce and Cheese-Its. The last sanitation inspection was completed March 13, 2024, with zero (0) demerits cited and a Superior classification issued. The last annual fire inspection was completed on March 6, 2024, with two cited violations. Please make sure to follow up with the school office staff to ensure all cited violations have been corrected. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A power tool (floor cleaning machine) was monitored plugged into an electrical socket charging in the cafeteria, accessible to ASEP children. 10A NCAC 09 .0601(a) 1921 Staff working in part-time, full-day or track out school age care programs required to complete BSAC training, did not complete it within three months of employment. A staff person hired February 12, 2024 did not obtain BSAC within three months of employment. .2510(j) Technical Assistance Provided and General Discussion: 1. Ms. Moultrie expressed concern for EC children being dismissed from the cafeteria. I encouraged Ms. Moultrie to speak with the school principal to discuss her observations and concerns with the current daily transition plan. It was observed while the ASEP staff were trying to prepare for afterschool, the EC children and staff were seated at the same tables needed for the ASEP program. The ASEP staff were observed cleaning and disinfecting the tables and EC children seated down afterwards. 2. We discussed and reviewed DPI Verification forms to ensure proper adherence to DCD expectations. 3. Ms. Wise is slated to work in ASEP for summer camp. It is critical that she obtain BSAC before working in any licensed summer camp for ASEP. I spoke with the PA, Leigh Bishop during the visit to express the concern. Corrective Action Plan: All violations must be corrected immediately. You shall submit a written, signed, and dated statement/compliance letter to me, at the address below detailing how each violation has been corrected and when. This information shall be received by Friday, June 14, 2024. You may email me your letter of correction. Mail written documentation to Mara Brinton, 7870 Woodmere Drive, Harrisburg, NC 28075. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time-period.
