The last Annual Compliance visit was made on 5/20/2025. The 3 Star Rated License was issued on 12/1/19. I was let into the facility by operator Lillian Whack. She was present with three (3) children. The April 2025 version of the Family Child Care Home Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 98% as of today. Children was observed having free play and routine needs met. There were toys and activities available. Attendance was monitored. Summary of law, schedule, summary of law, first aid sheet, and emergency numbers were posted. Monthly Fire Drills, Emergency Preparedness Drills, and Playground inspections were monitored. Children's files were monitored for Application, Emergency Care Authorization, Medical/Immunization, Discipline Statement, Summary of Law, No smoking statement, Shaken Baby Syndrome policy, and Outside fence permission. The provider's file was monitored for Criminal Qualifying Letters, First Aid and CPR coverage, Health and Safety Trainings, Shaken Baby Syndrome Policy, Special Trainings, On-going Training hours, and Annual Health Questionnaire. This facility does not provide transportation. Allergies and storage of medication was discussed. The outdoor, bathroom and kitchen areas were monitored. The following violations were cited during today's visit: Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. A n aerosol can of air freshener was located on a shelf not in locked storage. .1719 (a)(7) 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The 18 month compliance history, prior to today’s visit was 98%. Upon my arrival I was greeted by Lillian Whack, Owner/Operator. Supervision was maintained during today’s visit. The current licensed capacity is 8 children on first shift, with a maximum of 5 preschool children at any time, serving no more than 2 children less than 1 year of age. You may also care for 5 children on second shift, you stated you do not have any children enrolled on second shift at this time. Today, four children were present. One infant was present, one toddler was resent and two preschool children were present. During the visit I observed the preschool children and toddler were observed in free choice activities and in a teacher directed activity. The infant was observed during a bottle feeding and having tummy time. I reviewed your filed to monitor specialized trainings. You are current with CPR/FA and ITS/SIDS certification. You completed EPR certification in November 2016. You last completed the health and safety trainings in June 2018 and should have completed them all by June 2023 to maintain compliance with 10A NCAC 09 .1703(d)(2) completing health and safety training as part of on-going training so that every five years all 11 topic areas are covered. Your current DCDEE qualifying letter and your husband’s were reviewed and found meeting compliance, there are no other family members in the home 16 years or older. I reviewed program records during today’s visit. All information required to be posted were observed posted in the home. I reviewed visual safe sleep documentation, daily attendance, monthly playground inspections, and the emergency drill log and found them all meeting compliance. I requested to review your incident log and you stated you haven’t had any incidents to log, but had a notebook with reports ready, just in case. You do not have any children required to have medication. All cleaning supplies were observed stored properly. No hazards were observed today. Medications were monitored and observed to be stored properly. The following violations were observed today: Violation Number Comment Rule 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. You have not completed seven (7) of the health and safety topic areas since June 2018. .1703(d)(2) Compliance Letter Childcare providers are expected to maintain compliance with all applicable child care requirements at all times. All violations cited today shall be corrected immediately. Lillian Whack, owner/operator, will email a compliance letter explaining how each violation cited today has been corrected. The compliance letter must be received by the consultant, andrea.anderson@dhhs.nc.gov, on or before December 19,2023.
