You, Anyia Holloway, Operator, assisted me with the visit today. I used the Family Child Care Home Item Number Listing (DCD 0375) as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. During the visit, I monitored the indoor and outdoor environment, staff and children's records, outdoor inspection checklist, fire drill reports and required posted items. When I arrived, I stated the purpose of today’s visit and began my monitoring visit. You have three (3) enrolled children and all three (3) were present during today’s visit. Children engaged in free play throughout the learning environment; a planned art activity and a large group playdough exploration activity with the provider. The fire drill took place on 11/10/2205 and playground inspection was dated 11/01/2025. were conducted on11/10/2025. You last completed CPR on 1/17/2025, and will expire on 1/17/27 and First Aid, 1/7/25 and will expire on 1/17/2027. The provider’s ITS SIDS certification will expire on 6/4/2027. Her annual health questionnaire will expire on 9/1/2026. Indoor Learning Environment: The child care room contained dramatic play materials, blocks materials, manipulative materials, books, musical instruments, art supplies, and games. The area was set up and accessible for the child to engage in varies play opportunities. Weapons: Your facility is in compliance with Child Care Requirements regarding firearms. Transportation: The provider does desires to participate in transportation. Register, Insurance, and Driver’s license was reviewed during today’s visit. Registration: Insurance Policy: Approved Vehicle: White Ford Flex: License Plate: LDP 1240 Children’s Records: Three (3) children’s files were monitored during today’s visit. The outdoor learning environment where children play was observed in compliance. I spoke to Ms. Holloway about other areas that could be used for outdoor play, will need all the Christmas Decorations/cords inaccessible to children prior to use. Violation Number Comment Rule 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. One child present was not signed in on the documentation sheet for morning arrival. .1721(e)(6) 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has. Applications of enrolled children were missing this information (left blank) on the applications monitored during today's visit. 10A NCAC 09 .1721 (a)(3)(A)(B)(C)(E) Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. License Status: Currently, you hold a temporary child care license. Restrictions on your license include daytime care for first and second shift. There is a maximum of five (5) preschool children and three (3) at any time. Your temporary license will expire on May 1, 2026. Rated License: We discussed Pathway #1 and Pathway #2 during today’s visit. I stressed the importance of Ms. Holloway visiting the Division’s website (ncchildcare.ncdhhs.gov) and select the “What’s New” drop down. Select QRIS Modernization page and review information. There is an available power point to reference prior to selecting and printing the pathway of your desired preference. The rated license application and the three (3) month study for pathway #1 should be completed and sent to me by February1, 2026. and Safety Trainings: The Child Care Development Block Grant (CCDBG), also called the Child Care Development Fund (CCDF), is a federal grant which provides funding for child care subsidies for low income working families and improvements in child care quality. The recent reauthorization of CCDBG, Child Care and Development Block Grant Act of 2014, brought for the first time in 18 years new state requirements. These requirements promote the health and safety of children in child care, create continuity of access to subsidies for low-income families, better educate parents and the general public about the child care choices available to them, and improve the overall quality of early care and education programs. States must have pre-service or orientation and on-going minimum health and safety training requirements in place for child care providers. The trainings must cover the following content areas: * Administration of medication * Appropriate precautions in transporting children * Building and physical premises safety * Pediatric First Aid and CPR appropriate for the ages of children in care * Prevention and control of infectious diseases, including immunizations * Prevention of and response to emergencies due to food and allergic reactions * Prevention of shaken baby syndrome and abusive head trauma * Prevention of SIDS and use of safe sleeping practices * Recognizing and Responding to child abuse and neglect * Handling and storage of hazardous materials and the appropriate disposal of bio-contaminants * Emergency preparedness and response planning for emergencies To help child care providers meet the new training requirements, the Division of Child Development and Early Education has provided this training on Moodle on our website, providing FREE online training modules. Click on the Course Catalog tab at the top of the page on the website to login to your training. If you have obtained an individual NCID, then you can use it with you log into Moodle. General Reminders: 1. You may care for no more than five (5) preschool children at any time. Your capacity must never exceed five (5) preschool children and 3 school-aged children. 2. Children must be visually supervised while awake at ALL times and able to be heard while at rest. You may never leave your home to go outside or behind closed doors from the children. 3. Please make sure you read over the Child Care Law/Rules to familiarize yourself with all rules and regulations. Please visit https://ncchildcare.ncdhhs.gov/. 4. Please make sure all bedroom doors are kept locked to avoid privacy being affected. 5. Please make sure you review and check over already submitted children’s applications for full completion. There were several important pieces of information missing from children’s application. Please contact families as soon as possible to obtain needed information. 6. Create a transportation binder with the following information: A copy of Driver License, current Insurance Policy, current Registration, a picture of each child with their emergency information, and their signed transportation authorization. 7. Please remind all three families about the deadline for a completed physical examination. As of today’s date, and according to the children’s records, they all have until 1/8/2026 to be in compliance with this child care rule. 8. We spoke about reusing menu items on a monthly basis. The provider will have to remember to change the dates and/or make substitutions if a food item is not available. The following violations were cited today. REQUIRED RESPONSE: You shall correct the violation(s) immediately. Send a signed corrections letters to the address below or an email by 12/25/25.
