The center operates with a Five-Star license and an 18-month compliance history score of 97%, effective October 15. 2019. The Secretary of State website was checked and MRS. CHRIS PLAY AND LEARN INC was current and active. The restrictions include: First and Second Shift; Meets all En. Requirements and reduced ratios.; Meets enhanced space. Upon arrival, I was greeted by K. DeLeon, Director. I discussed the Annual Compliance Monitoring guidelines with Ms. DeLeon. We completed the walkthrough of the center together and Ms. DeLeon assisted me with the visit. The following items were posted within the center/ classrooms: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/ departure procedures, summary of law and menus. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. Children were observed during indoor/ outdoor free play and transitions. There were five (5) children present today. There are ten (10) children enrolled in the center. The center is only providing care for first shift currently. Outdoor Learning Environment: The outdoor learning environment was monitored today and in compliance. Inspections: The outdoor inspections and fire drills were monitored. The last fire drill was conducted on 8/18/25. The centers last shelter in place/ lockdown drill was conducted on 8/5/25. The last monthly playground inspection was conducted on 7/14/25. The center last had a sanitation inspection on 7/22/25. The center received two (2) demerits and received a superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 4/1/25. Transportation: The center does not provide transportation. Nutrition: Meals met nutritional value during today’s visit. Files: Children records were monitored today and in compliance. Staff files were monitored today and in compliance. There was one (1) violation cited today. Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. On the OLE, I observed a broken wooden border/ considered a tripping hazard. 10A NCAC 09 .0604(p) Violations must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 9/10/2025, stating how each violation was corrected and how compliance will be maintained in the future include the violation number, statement of compliance, license ID and your signature in the letter or e-mail. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Please email me a signed copy of the letter to Lashawnda.Berry@dhhs.nc.gov Technical Assistance: 1. The center has had a wall extension added, they have had approval from sanitation and fire. They have submitted all required documents to building and zoning and they are waiting for approval. Once they receive approval from building and zoning, they will submit all documents to me. 2. Inspections: After each sanitation and fire inspection you must send a copy of the report to you Child Care consultant within one week. *I encouraged the Administrator to stay up to date with the Division of Child Development and Early Education by visiting www.ncchildcare.ncdhhs.gov. This center has a five-star license and an 18-month compliance history score of 96%. The center’s license and NC child care law were prominently posted. I was greeted by staff member, K. Hill, who informed the Director, K. DeLeon, that I was present. I discussed with you, Ms. DeLeon, the Annual Compliance Monitoring guidelines. Ms. DeLeon assisted me on my walk through of the center. Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. The NC Secretary of State website was viewed before the visit and MRS CHRIS PLAY AND LEARN, INC. was current and active. The center’s Emergency Drill Log and Report was reviewed. Fire drills were monitored and was in compliance. Shelter-in-place and Lock-down drills were reviewed and in compliance. The playground safety inspections were reviewed and documented. The center last had a sanitation inspection on 1/12/2023. The center received six (6) demerits and a superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 3/28/2023. There are currently eighteen (18) children enrolled in the center and there were (3) children in attendance today. Children were observed engaged in teacher directed activities, indoor and outdoor free play. This facility currently does not provide care for second shift. The center does not provide transportation. This center has a NC- PreK program, all documents were monitored and in compliance. Staff files and children records were monitored today and in compliance. The playground was monitored today and in compliance. There were three (3) violation cited. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. While monitoring the outdoor learning environment, I observed the steps to the play structure were peeling and two of the small play structures were rusted/ peeling paint. 10A NCAC 09 .0601(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Today I observed three bottles of paint that said keep out of reach of children on a shelf, accessible to children. .2820(b) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The mulch surfacing on the outdoor learning environment measured under the required six inches (6"). .0605(k)(1-4) Violations must be corrected immediately. Rated License: The hold harmless legislation was extended and signed into law on Monday June 12th, 2023. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024 and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each include a preparation year and a reassessment year. While the first reassessment year won’t begin until July 1, 2024, the preparation year for cohort one began July 1, 2023. Your facility is in cohort two.
