This facility currently operates with a three (3) star license, meets all enhanced ratios minus one and serves three-five year old children. Upon arrival I was greeted by front receptionist. I stated the purpose of my visit and waited for someone to meet me in the front office. While walking to the classrooms, I saw Ms. Edwina Desravines and we greeted each other and I restated my purpose. Ms. Edwina Desravines assisted me with the. The last day of school is today, June 13, 2026. The first day of the school year was September 2, 2026. The North Carolina Secretary of State Corporation website was viewed prior to the visit and Movement Charter School was active. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be brought sole, subleased, transferred to another person or location or inherited. Today was the last day of school and graduation day. Both classes were preparing for graduation, and putting on cap and gowns, I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, required trainings, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records and files, permit restrictions, safe arrival and departure, and all required posted documents. This facility currently does not provide transportation for preschool classrooms. Staff and Training worksheets were completed prior to arrival. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last sanitation inspection was conducted 02/20/26 The last fire inspection was conducted 03-16 -25. The last documented fire drill was conducted 6-9-26. The last documented shelter-in-place or lockdown drill was conducted 4-15-26 The last documented playground inspection was conducted 6-10-26 Last compliance history was 90% prior to today’s visit. During this visit, Ms. Edwina, made me aware that this is her last school year at this location and that she will be relocating to another school. The teacher in space two will be transitioning to teach in the kindergarten room. Rated License: We discussed the pathways and you all have decided on Pathway two. You currently use Creative Curriculum We went over and identified the Family and Community Engagement that you all will be using. Education: at the routine Unannounced visit I reviewed the staff's education. If staff remain the same, Violation Number Comment Rule 1301 Center did not maintain a record of daily attendance, several children were missing from the attendance but were present. Do not forget to use the facility NCID: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. At arrival I rang the door bell , greeted the administrative assistance and I stated the purpose of my visit and asked for the administrator of the preschool classes. I was allowed to walk back to the classrooms. Before I arrived to space 1, Ms. Guy, teacher in charge in the administrators absence, greeted me. I restated the purpose of my visit and she made me aware the Ms. Des Ravines, administrator was at another location. Ms. Guy partially assisted with the visit due to her being in the classroom. Children were observed napping and quietly playing on their cots. Transitions were monitored and children played in activity centers after nap. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure, and all required posted documents. Star preparation: during this visit I looked in Works to review staff education. It is imperative that teachers submit their transcripts into Works. If you applied for Stars today you would receive a one star based on education. I spoke with some teachers and made them aware of what is needed. This facility currently operates with a three (3) star license, meets all enhanced ratios minus one and serves four-five year old children. Upon arrival I was greeted by front receptionist. I stated the purpose of my visit and waited for someone to meet me in the front office. Ms. Guy, teacher, met me in the office and I restated my purpose. assisted me with the visit in the director’s absence. The receptionist escorted me down the hall where I was greeted by Ms. De, who assisted me with today’s visit. Ms. Edwina Desravines arrived later and assisted me with the rest of the visit. The last day of school was June 18, 2025. The first day of the school year was September 2, 2024. Children have only been in school for four days and are all new to the facility. The North Carolina Secretary of State Corporation website was viewed prior to the visit and Movement Charter School was active. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be brought sole, subleased, transferred to another person or location or inherited. Children in space one were engaged in small group learning activities and puzzles. Children in space two and three were observed in circle time. Lunch was monitored and the children were served turkey sandwiches, applesauce, carrots, and milk. Space two had 13 children present and one teacher, the second teacher assisted me during the walk through the facility. While I was in the office typing my report a teacher from space three pushed the lunch cart in the office, which made the ratio 1:13. Prior to today’s visit, the facility notified me that they would like to lower their age range. It was discovered that Zoning needs to come out and re-inspect because their prior approval stated that they will serve children four and older. Facility is still waiting on Zoning. The first day of school was on September 2, 2025, and they allowed the three-year-olds to attend. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, required trainings, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records and files, permit restrictions, safe arrival and departure, and all required posted documents. This facility currently does not provide transportation for preschool classrooms. Staff and Training worksheets were completed prior to arrival. The attached checklist was used to note the requirements we monitored today. Children's first day of school was September 2, 2025. Staff and the rest of the school had a fire drill on August 22, 2025. The school and the preschool typically do their drills at the same time and is controlled by the elementary school. INSPECTIONS: The last sanitation inspection was conducted 02/05/25 The last fire inspection was conducted 03 -17 -25. The last documented fire drill was conducted 6-3-25. The last documented shelter-in-place or lockdown drill was conducted 6-3-25. The last documented playground inspection was conducted 9-2-25. Last compliance history was 92% prior to today’s visit. The North Carolina Secretary of State Corporation website was viewed prior to the visit and Movement Charter School was active. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be brought sole, subleased, transferred to another person or location or inherited. Children in space one were engaged in small group learning activities and puzzles. Children in space two and three were observed in circle time. Lunch was monitored and the children were served turkey sandwiches, applesauce, carrots, and milk. While I was in the office typing my report a teacher from space three pushed the lunch cart in the office, which made the ratio 1:13. This facility currently operates with a three (3) star license, meets all enhanced ratios minus one and serves four-five year old children. Upon arrival I was greeted by front receptionist. I stated the purpose of my visit and waited for someone to meet me in the front office. Ms. Guy, teacher, met me in the office and I restated my purpose. assisted me with the visit in the director’s absence. The receptionist escorted me down the hall where I was greeted by Ms. De, who assisted me with today’s visit. Ms. Edwina Desravines arrived later and assisted me with the rest of the visit. The last day of school was June 18, 2025. The first day of the school year was September 2, 2024. Children have only been in school for four days and are all new to the facility. The North Carolina Secretary of State Corporation website was viewed prior to the visit and Movement Charter School was active. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be brought sole, subleased, transferred to another person or location or inherited. Children in space one were engaged in small group learning activities and puzzles. Children in space two and three were observed in circle time. Lunch was monitored and the children were served turkey sandwiches, applesauce, carrots, and milk. Space two had 13 children present and one teacher, the second teacher assisted me during the walk through the facility. While I was in the office typing my report a teacher from space three pushed the lunch cart in the office, which made the ratio 1:13. Prior to today’s visit, the facility notified me that they would like to lower their age range. It was discovered that Zoning needs to come out and re-inspect because their prior approval stated that they will serve children four and older. Facility is still waiting on Zoning. The first day of school was on September 2, 2025, and they allowed the three-year-olds to attend. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, required trainings, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records and files, permit restrictions, safe arrival and departure, and all required posted documents. This facility currently does not provide transportation for preschool classrooms. Staff and Training worksheets were completed prior to arrival. The attached checklist was used to note the requirements we monitored today. Children's first day of school was September 2, 2025. Staff and the rest of the school had a fire drill on August 22, 2025. The school and the preschool typically do their drills at the same time and is controlled by the elementary school. This facility currently operates with a three (3) star license. Upon arrival I was greeted by front receptionist. I stated the purpose of my visit, and she contacted Ms. Edwina Desravines, director. The receptionist escorted me down the hall where I was greeted by Ms. Desravines, who assisted me with today’s visit. The first day of the school year was on September 3, 2024. Children have only been in school for four days. The North Carolina Secretary of State Corporation website was viewed prior to the visit and was as active. If any changes to the corporation need to be ade or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be brought sole, subleased, transferred to another person or location or inherited. Children in space two were observed transitioning from large group circle time into smaller group activities and center play. Another group were engaged in active gross motor play on the playground. choose. I conducted a walkthrough of the facility, and the following items were monitored: supervision, staff/child ratio, CPR/first aid, required trainings, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records and files, permit restrictions, safe arrival and departure, and all required posted documents. This facility currently does not provide transportation for the preschool classrooms. Staff and Training worksheets was completed prior to arrival. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last sanitation inspection was conducted 01/17/24 The last fire inspection was conducted 0 - -24. The last documented fire drill was conducted 6-3-24. The last documented shelter-in-place or lockdown drill was conducted 6-3-24. The last documented playground inspection was conducted 6-3-24. Last compliance history was 93% prior to today’s visit. Violation Number Comment Rule 707 Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. There was a foam ball that had missing chunks. .0601(d) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. there were two outlets exposed in the block center in space 2(168). 10A NCAC 09 .0604(c) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last documented inspection was on June 3, 2024. .0605(q) Comments: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. REQUIRED RESPONSE: (for any violations) It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. Upon arrival I was greeted by Ms. Wade and shared the reason for the visit. Ms. Wade assisted me with today’s visit until Ms. Polk was able to step in and complete the visit. Your program currently operates with a three (3) Star License. The license was posted, with restrictions to first (1st) shift care, children in care on ground level only, and meets all enhanced requirements minus one (1). The center's compliance history was reviewed with the operator. The program’s compliance history was ninety two (92%) percent as of today’s visit. Ownership: The facility owner is Movement Foundation. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The fire inspection was conducted on 11/7/23 and passed. The facility did not receive the correct documentation for day care fire inspection visits, so a copy was provided. Please keep in mind that North Carolina law states all yearly fire inspections that have passed must be submitted to the Child Care Consultant within seven (7) days of the inspection. The sanitation visit was conducted on 1/17/24 with a Superior rating and three (3) demerits, this was updated in the system. Please alert your Child Care Consultant with any Sanitation Inspections upon completion. The following items above were found in compliance. The following inspections are below. •Last fire drill was conducted on 3/21/2024. •Last Shelter in Place/Lockdown Drill was conducted 3/19/2024. •Last Playground Inspection was conducted on 2/21/2024. The facility keeps all incident reports on file, but does not have an incident report log, one was provided during this visit. Please keep in mind that all incidents need to be logged on the form and if applicable shared with the Consultant. The children in classroom one sixty nine (169) were observed to be participating in group activities through movement counting and adding. They were also transitioning into teacher led good morning/circle time activities. The children in classroom one hundred and sixty eight (168) and one hundred and sixty seven (167) had transitioned from breakfast to self-guided learning center activities including dramatic play, playdough fine motor work, sensory stations and self-care routines. The facility is in compliance with nutrition standards with breakfast consisting of milk, yogurt, strawberries, and granola. Each classroom has an allergy list posted. Staff Records: The staff-training worksheets was completed at the time of the visit. There were three (3) new staff since the last visit. All files were organized. The medical information was not kept separately and was corrected during the visit. The Shaken Baby Head Trauma policy was not reviewed or signed on or before the date of employment and this is a violation. As a reminder encourage all staff to completely fill out the Emergency Information form. Medication: Medication was monitored and was in compliance. One suggestion for the epi-pen in room one hundred and sixty nine (169) was to keep the medication administration permission form with the medication. Also as a reminder the permission form for the epi-pen in classroom one hundred and sixty eight (168) expires on 3/28/24. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. There were three (3) violations were observed today and two (2) were corrected during today’s visit. New staff members M.J. and J.B. did have proof showing review and signed acknowledgement of the Shaken Baby Syndrome and Abusive Head Trauma policy but the date reviewed and signed was after caring for children ages zero (0) to five (5). .0608(d)(1-4) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. New staff members M.J. and J.B. medical report, proof of tuberculosis and health questionnaire were kept in the staff file and not in a separate file. .0701(d) On or before 4/14/2024 I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. You can take the pre- enrollment webinars which is available at www.cleanwaterforUSkids.org/carolina then enroll – At www.cleanwaterforUSkids.org/carolina completing the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos. Keep mindful that once you evaluate potential hazards and receive results if there are recommendations and/or water mitigation support is needed they will provide direction in how to mitigate and ways to fund said mitigation. •As stated above and as a reminder, any time there is a fire or sanitation inspection please share those results with your Child Care Consultant. At arrival I rang the doorbell, greeted the administrative assistance and we signed the visitor’s log. I stated the purpose of my visit and asked for the administrator of the preschool classes. I was told that the Site Director was in a meeting, and I was guided to the preschool hall and began my visit. As I entered each classroom, introduced myself and stated the purpose of my visit. I was briefly accompanied by Ms. Wade who is in charge during the Site director’s absence. I was later accompanied by Ms. Des Ravines, site director to complete the visit. During this visit children in space #169 were observed having circle time which consisted of a recall of the week and a music and movement. In space 168 children were engaged in circle time, which consisted of a story time. In space #167 children were also engaged in circle time which consisted of a story time. Children were served chicken nuggets, carrot sticks, blueberries, pasta salad, and milk for lunch. The attached checklist was used to note the requirements we monitored today. Supervision, Staff/child ratio, health, safety, program and management, Staff and children’s records were also reviewed today. Transportation is not provided by this facility. INSPECTIONS: The last sanitation inspection was conducted 10/4/2022. The last fire inspection was conducted 6-9-22. The last documented fire drill was conducted 05/31/23. The last documented shelter-in-place or lockdown drill was conducted 5/31/23. The last documented playground inspection was conducted 09/01/23. The last compliance prior today is 90%. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The last day of school was in June 2023. The last fire drill was conducted on 5/31/2023. .0604(t); .0302(d)(5) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. in space 168 there was an uncovered outlet in the book center. In space 167 there was a missing outlet cover in dramatic play center. 10A NCAC 09 .0604(c) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last day of school was in June 2023, and school started September 5th, 2023. The last drill was conducted in May 2023. .0604(u);.0302(d)(8) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. REQUIRED RESPONSE: You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address below by September 22, 2023. Your facility meets these criteria and therefore will. retain your current star rating until six months after Governor Cooper issues an order to rescind Executive Order No. 116 Declaration of a State of Emergency to Coordinate Response and Protective Actions to Prevent the Spread of COVID-19. You are in Cohort three and will not be due to reapply until 2025-2026. You may reapply prior if you desire. *The Pandemic ended in August 2022, This February 2023 a plan will be revealed on how to get all facilities with current license. You had your environmental rating scale done in February 2023, you have decided to not use your scores and will remain in a “hold harmless state”. Technical assistance: *Shelter in place must be conducted every three months and should be documented on the form provided on the DCDEE website. This form does not have to be posted and can be kept in your office to assist you with remembering. Because of the timing of your annual compliance, It is suggested that you conduct a fire drill and shelter in place during the last week of school. *Ask the Janitor or Principal to ask all teachers to assist with litter pickup on the playground. It will assist with the PreK teachers not having to take full responsibility for cleaning after other classrooms before they take the children to the playground.
