Name of Operation: Movement Northwest Early Education Facility ID: 60004412 Consultant: MICHELLE CUTHBERTSON Operation Type: Center Case Number: Visit Date: 10/23/2025 Number Present: 36 Completed Date: 10/23/2025 Age: From 3 To 5 Total Minutes: 165 Time In: 09:15 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of my unannounced visit was to monitor for compliance with all applicable childcare requirements for a temporary time period. Currently this center operates with a Temporary License, issued on September 8, 2025. The restrictions include daytime care only. This center is on a temporary license due to a location change. Upon arrival, I was greeted by L. Crump, director. I stated the reason for the visit. Ms. Crump accompanied me during the walkthrough of the facility. There were 36 children present during the walkthrough of the facility. I observed the children participating in transitions, teacher directed activities, and outdoor play. Supervision, staff/child ratio, group size and space were observed in compliance today. The last fire inspection was conducted on 8/6/25. The last sanitation inspection was made on 9/29/25. The center received a Superior classification with 6 Demerits. Fire drill report forms were reviewed and were current and up to date. I monitored the outdoor learning environment. The facility does not provide transportation. I monitored all staff files. I monitored a percentage of children files. There were 2 violations cited today. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In space 211, the space for children 3 years of age there was no activity plan posted. GS 110-91(12); .0508(a) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. A child's file was missing the medical exam. GS110-91(1) Technical Assistance: I shared with Ms. Crump to complete the playground safety training by March 2026. I shared with Ms. Crump to complete the EPR plan by September 2026. Star Rating: Due to a change of location, the previous ERS scores will be used. The facility received a score of 5.64 for ECERS that was conducted on 2/9/2023. The facility will earn 7 points in Program Standards; they will follow highest enhanced ratios and enhanced space. The facility will earn 4 points in Education. The administrator is 6 points in education. The lead teachers are 4 points in education. The teacher is 7 points in education. The facility will earn 1 quality point for staff benefits package and infrastructure of parent involvement. The facility will receive a total of 12 points. A four star license will be recommended. The facility uses creative curriculum, which is an approved curriculum. QRIS Conversation: -Today, I had the QRIS conversation with Ms. Crump. A QRIS conversation report was completed, signed by Ms. Crump and she was given a copy. Compliance Plan: All violations cited must be corrected immediately. Please submit a written, signed and dated statement to me at the email address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation using the item listing in your visit summary, you must state how each violation was corrected and are now in compliance. 5.
