Prior to today’s visit the compliance history was 97%. Supervision, Staff / Child Ratios, Adequate Approved Space , Permit Restrictions and CPR/First Aid were monitored during today’s visit. Upon arrival, I checked in at the main office and was directed to the cafeteria. Erik Randle, Associate, was setting up the learning centers. I greeted Mr. Randle and asked if Ms. Howard was available. Ms. Howard came to greet me and I explained the purpose of my visit to she and Mr. Randle. Children had not arrived yet for the afternoon, so I was able to review the program board for current CPR/First Aid certifications. One staff member has an expired certification which was due for renewal by June, 2024. We discussed that on September 23, 2024, a CPR and a First Aid violation was observed and cited. The following violations documented during the September 23, 2024, visit were monitored for compliance during this visit: 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One (1) staff member does not have current First Aid Certification. This is a violation of a requirement in .1102(c). This violation has not been corrected. 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. one (1) staff member does not have current CPR certification. This is a violation of a requirement in .1102(d). This violation has not been corrected. Ms. Howard informed me that the staff member is scheduled for CPR/First Aid class this week. She stated that he did not attend the training scheduled for October 28, 2024. I spoke with the staff member and he stated he had a personal conflict and could not attend the training. I explained to Ms. Howard and Mr. Randle that approved training for CPR/First Aid is available regularly giving opportunities to have taken the training. I explained the compliance statement and the timeline for corrections. The compliance requirement is included in the comments section of this visit summary. I explained to Ms. Howard that failure to correct the violations may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. I monitored the arrival of the children in the Media Center. I observed them engaged in reading books and talking with one another. I observed the children transition to use the restroom before coming to the cafeteria for the afternoon. Supervision and ratio were maintained. Permit and approved space were in compliance. The following two (2) violations were cited today: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member does not have current First Aid certification. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member does not have current CPR certification. .1102(d) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Upon arrival, I checked in at the main office and was greeted by Mr. Randle, Associate. Mr. Randle called Shontella Howard, Site Coordinator. Ms. Howard met me in the Main Office and escorted me to the program board located outside of the cafeteria. Children had not arrived yet for the afternoon, so I was able to review staff files, program notebook and talk with Mrs. Howard and Mr. Randle. The facility had a Five-Star License issued March 29, 2018, meeting enhanced space and reduced enhanced ratio. The eighteen-month compliance history score of 99% prior to today’s visit. The last Routine Unannounced visit was completed May 28, 2024. A walk through of the facility was completed. There are two (2) groups of after schoolers. I observed the children arriving to the media center for the afterschool program, attendance taken as children arrived, staff initiating quiet transitional games and children reading books accessible to them on the media center tables. I observed bathroom and handwashing routines. I observed the children eating fruit cups and juice for snack. After snack, the after schoolers washed their hands again and began homework and participated in the different activity centers. Each group had activity areas set up with adequate and accessible materials. The following child care requirements were monitored and observed today: Supervision: Each group of children were adequately supervised during the visit. Nurturing and engaging tones were heard when staff spoke with children. Staff/Child Ratio: The ratios were maintained during today’s visit. Check in was observed and attendance records were reviewed and found in compliance. CPR: One (1) staff member was not current with CPR; a violation was cited. First Aid: One (1) staff member was not current with First Aid; a violation was cited. Special Training: Staff members have completed all required special training. CBC Qualification: Each staff member was current with CBC qualification letter. Emergency Medical Care Plan: The Emergency Care Plan posted up to date and in compliance. Administration and Storage of Medication: There are no children with medication(s).There were no medications stored. Storage of Hazardous Substances: There were no hazardous substances accessible to children. General Safety: There were no general safety concerns observed. Discipline: There were no discipline concerns; appropriate discipline was provided. Adequate/Approved Space: Each group of children were observed in licensed approved space. Program Records: I reviewed all information required to be posted. The last monthly fire drill was August 30, 2024. The last shelter in pace drill was September 3, 2024. I observed a current playground safety inspection dated August 27, 2024, and monthly records meeting compliance. The last Fire Inspection Report posted is dated September 13, 2023. I spoke with Inspector Joshua Cunningham after Ms. Howard contacted him by phone to request a copy of current Fire Inspection Report. He emailed the report to Ms. Howard, and I was able to review it date July 11, 2024.Ms. Howard stated she will print the report and post it on the ASEP bulletin board. License Posted: The license was observed posted. Permit Restrictions: All permit restrictions were observed meeting compliance today. The last Sanitation Inspection was conducted February 21, 2024, with a superior rating and six (6) demerits. Two (2) violations were cited during today's visit. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One (1) staff member does not have current First Aid Certification. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. one (1) staff member does not have current CPR certification. .1102(d) Compliance Statement: Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before Monday, October 7, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Staff and Training Worksheets: I recommend you update the Staff and Training Worksheets to help you track the needed trainings and expirations dates for qualifying letters, health and safety trainings and other required documents to maintain compliance. QRIS: Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions. Visit the NCRLAP’s website, https://www.ncrlap.org/ for more information about updated resources, credit hour trainings, and outreach assessment opportunities to help you become familiar with these tools. For official environment rating scale assessments for a NC Star Rated License, the NCRLAP will use the Revised editions until February 1, 2025.
