Name of Operation: MAJOR CARES LEARNING CENTER Facility ID: 60004109 Consultant: LASHAWNDA BERRY Operation Type: Family CC Home Case Number: Visit Date: 3/12/2025 Number Present: 5 Completed Date: 3/12/2025 Age: From 1 To 2 Total Minutes: 135 Time In: 01:00 PM Time Out: 03:15 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance during an annual compliance visit. This family child care home has a five-star license and an 18-month compliance history score of 93%. The center’s license and NC child care law were prominently posted. I was greeted by Brittany Major, Owner. I discussed with you, Ms. Major, the Annual Compliance Monitoring guidelines. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. The children were observed during naptime. There were five (5) children present today. There are six (6) children enrolled in the center. Inspections: The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required, the centers last fire drill was documented on 2/26/25. Shelter-in-place drills were in compliance and documented on 1/15/25. The playground safety inspections were reviewed, the last playground inspection was documented on 3/12/25. The facility last had a sanitation inspection on 8/12/24. The inspection was completed by the Mecklenburg County Health Department. Outdoor Learning Environment: The outdoor learning environment was monitored today and in compliance. Safety: The center is in compliance with the firearm safety requirements. Transportation: The center does not provide transportation currently. Nutrition: Meals met nutritional value during today’s visit. Files: Children records were monitored today and in compliance. Staff files were monitored today and in compliance. There were six (6) violations cited today. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. I observed the cabinet underneath the bathroom sink not lock, this is where hazardous products/ creams are stored/accessible to children. .1719 (a)(7) 716 Electrical outlets not in use were not covered. I observed electrical outlets in the operator office (where children were asleep) and in the kitchen, that were accessible to children, they were not in use and no safety covers. 10A NCAC .1719(a)(27) 729 Appliances with heating elements, such as bottle warmers, crock pots, curling irons, and electrical cords were accessible to children. I observed a heated blanket on the couch in the office of the operator, it was not plugged in but the cords were accessible to children. 10A NCAC .1719(a)(28) 1917 Children were not cared for in space designated as the caregiving area on the floor plan provided by the operator. Upon arrival, I noticed that children were asleep in the Operators office, this space in not approved to provide care for children. .1707(7) 1920 Indoor and outdoor area was not checked daily for debris, vandalism, broken equipment, and animal waste. On the outdoor learning environment, I observed a broken blue children's chair on the children's play space. .1719(a)(17) 2055 An individual sleeping space for resting such as a bed, crib, play pen, cot, mat, or sleeping bag with individual linens was not provided for each pre-school age child who was in care for four hours or more, or for all children if overnight care is provided. Upon arrival, I observed four children asleep in the operators office. There were two children asleep on the floor with no individual mat/cot and two children asleep on the couch. 10A NCAC 09 .1718(a)(2) Violations must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 3/26/25, stating how each violation was corrected and how compliance will be maintained in the future include the violation number, statement of compliance, license ID and your signature in the letter or e-mail. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Please email me a signed copy of the letter to Lashawnda.Berry@dhhs.nc.gov Please note that any violations cited today or during future visits will negatively impact your compliance history. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Technical Assistance: 1.Ms. Major stated that she would like to provide transportation in the future. Ms. Major agreed to contact me when she is ready to have her vehicle inspected and approved to transport children. 2.Today we discussed options for the Ms. Majors office/adding the space on the center's floor plan to provide care for children. This family child care home has a four-star license and an 18-month compliance history score of 90%. The center’s license and NC child care law were prominently posted. I was greeted by Brittany Major, Owner. I discussed with you, Ms. Major, the Annual Compliance Monitoring guidelines. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. Children were observed engaged in multi-play opportunities. The caregiver was observed during nurturing and engaging interactions with children. There were three (3) children present today. There are five (5) children enrolled in the center. Inspections: The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required. Shelter-in-place and Lock-down drills were reviewed and in compliance. The playground safety inspections were reviewed and documented. The facility last had a sanitation inspection on 7/2023. The inspection was completed by the Mecklenburg County Health Department. Outdoor Learning Environment: The outdoor learning environment was monitored today and in compliance. Safety: The facility is in compliance with the firearm safety requirements. Transportation: Today I the provider and I discussed using her vehicle for transportation. During today’s visit, I monitored the providers Chevrolet Tahoe, license plate: JDG-957, the vehicles registration expired on May 2022. The vehicle will be approved to transport children once Ms. Major is able to provide documentation of a copy of the vehicle’s current registration. Nutrition: Meals met nutritional value during today’s visit. Files: Children records were monitored today and not in compliance. Staff files were monitored today and not in compliance. There were five (5) violations cited today. I observed diaper cream in a drawer under the diaper changing table, accessible to children. .1719 (a)(7) 920 Records were not made available for review. One children's file was not available for review. G.S. 110-91(9) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. One children currently enrolled did not have documentation of departure times for the week of 3/4/24. .1721(e)(6) 938 Individuals who provide care for 5 hours or more a week, during planned absences of the operator, had not completed a health questionnaire. MJ, caregiver's health questionnaire expired on 6/1/23. .1729(a)(4) 1945 Items that are prohibited to be an infants sleep space, eg. pillows, wedges, blankets, toys, bumper pads, quilts, loose bedding, towels, and wash cloths were present in the crib with a sleeping infant. I observed an infant asleep in a bassinet with a blanket. .1724(a )(3) Technical Assistance: 1. I encouraged the Administrator, to stay up to date with the Division of Child Development and Early Education by visiting www.ncchildcare.ncdhhs.gov. This website enables you to view the entire Law and Child Care Requirements for North Carolina as well as download required forms. I encourage you to click on the “What’s New” tab for important updates impacting childcare in North Carolina. 2. Today Ms. Major and I discussed relocation. The provider will need to do the following for FCCH relocation: a. Notify Care Consultant via email atleast 30 days prior to moving with the new address. b. Obtain the following inspections: Fire, Building and Sanitation * Send a copy of all inspections to your child care consultant. Violations must be corrected immediately.
