I was greeted by Owner/Operator Mrs. Diana Quarles, and I explained the purpose of the visit. Mrs. Quarle's family was present in the home during the visit. I completed a walkthrough of the indoor/outdoor facility. Your program currently operates with a five-star license, issued July 24, 2024, earning six (6) points in the education component, seven (7) points in the program standards component, and one (1) quality point for obtaining an Infant Toddler Certificate. The program uses a developmentally appropriate curriculum (The Mother Goose) that addresses the five domains of development. Your program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year-old children are enrolled). There are currently no four (4) year-olds enrolled. The sanitation inspection was completed January 14, 2025, with a zero (0) Demerit Score. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven (97) percent as of January 4, 2026. The Emergency Preparedness and Response Plan (EPR) was updated January 2, 2026. The NC Secretary of State website was reviewed on January 5, 2026, and Majestic Hearts Childcare LLC was listed as current-active. Upon arrival, I observed three (3) children present. I observed the children having free independent play, and I observed Mrs. Quarles using nurturing tones when she spoke with the children. One (1) child is enrolled on Second Shift. I observed the children served the following items during lunch; meatloaf, cream potatoes, spinach, whole wheat roll, apples, and milk. The facility was monitored today with the Family Child Care Home Monitoring Checklist and the Family Child Care Home Item Number Listing. A copy of the checklist was provided to Mrs. Quarles after the visit. The following was observed posted: Current License, NC Summary of Child Care Law, Schedule, First Aid Sheet, Activity Plan, Safe Sleep Poster, Safe Sleep Policy, and the Emergency Medical Care Plan. Monthly Fire Drills, Shelter-in-Place/Lockdown Drills, and Playground Inspections were observed and compliance was met. The daily attendance and arrival and departure times were reviewed and met compliance. The child care facility, including the bathroom, child care room, kitchen, and outdoor play area was monitored during the visit. Soap and toilet paper were available in the bathroom for use. The toys and equipment were in good repair. In the kitchen, there was a working fire extinguisher, working thermometer in the refrigerator, and smoke detectors, and carbon monoxide detectors. Mrs. Quarles file was monitored and observed for IT-SIDS training, Health Questionnaire, Emergency Information, First Aid and CPR, and Criminal Qualifying Letters; Mrs. Quarles husband and three (3) children have a current Criminal Qualifying Letter on file. Mrs. Quarles training hours were monitored; six and a half hours (6 1/2) were completed. The facility's roster was complete in the Automated Background Check System (ABCMS). The children’s files were monitored today. A violation was cited for off premise forms. Mrs. Quarles does not have any pets. There are no children enrolled with allergies, and there are no children on medication or emergency medication. There were no hazardous materials or substances observed. The outdoor play area was observed, and there were no hazards observed. The outdoor play area was clean and the toys were in good repair; balls, and other toys were available for outdoor play. Mrs. Quarles does not provide transportation and a vehicle was available in case of an emergency. The Written Plan of care was verified. One (1) violation was cited. The implementation plan will allow time for administrators/operators to learn and understand the different pathways and the changes within our system. Mrs. Quarles attended a QRIS Session. The session provided valuable insights on QRIS, also known as Pathways to the Stars, and designed to help you understand and prepare for the transition to the new QRIS system. -Get Ready for the 3’s / QRIS: Starting on February 1, 2025, the ECERS-3, ITERS-3, and FCCERS-3—also known as the "3s"—will be used for DCDEE environment rating scale assessments. The DCDEE and the North Carolina Rated License Assessment Project (NCRLAP) are collaboratively working on preparations for the transition to the third editions. These third editions come with a spiral binding at the top, replacing the current revised editions. Visit the NCRLAP’s website, https://www.ncrlap.org/ for more information about updated resources, credit hour trainings, and outreach assessment opportunities to help you become familiar with these tools. For official environment rating scale assessments for a NC Star Rated License, the NCRLAP will use the Revised editions until February 1, 2025. Quality Every Day (QED) is a quality initiative at Child Care Resources Inc., funded by Smart Start of Mecklenburg County. They are recruiting for the QED Program this year for those that may be interested in coaching and training for teachers and directors. There are different tracks for participation (Licensing renewal, Program start-up, staff-child interactions and director support); and there are new opportunities this year like updated ERS-3 trainings, Leadership college coursework for administrators and Communities of Practice for both Child Care Directors and Family Child Care providers. They still have opportunities for college coursework and stipends for coursework completion. Mrs. Quarles is currently participating in QED. -Technical Assistance was provided on the QRIS Pathway to the Stars. Mrs. Quarles confirmed that the facility would like to apply for a Five (5) Star Rated License through Pathway 2; forms Pathway to the Stars (completed during the visit). They still have opportunities for college coursework and stipends for coursework completion. The facility had a Five-Star License issued July 24, 2024, and has an eighteen-month compliance history score of one-hundred (100) percent prior to today’s visit. The Annual Compliance visit was completed on January 13, 2025. I observed Mrs. Quarles present with two (2) children. The children were observed eating breakfast. The items noted on the menu were served: cheerios, bananas, and milk. The following child care requirements were monitored and observed today using the Family Child Care Home Checklist and the 2025 Master Family Child Care Home Item Number Listing. Supervision: The children were adequately supervised. Nurturing tones were heard when Mrs. Quarles spoke with the children. Staff/Child Ratio: The staff/child ratio was maintained during today’s visit. CPR: Mrs. Quarles is current with CPR. First Aid: Mrs. Quarles is current First Aid. The Annual Compliance visit was completed on January 13, 2025. I observed Mrs. Quarles present with two (2) children. The children were observed eating breakfast. The items noted on the menu were served: cheerios, bananas, and milk. The following child care requirements were monitored and observed today using the Family Child Care Home Checklist and the 2025 Master Family Child Care Home Item Number Listing. Supervision: The children were adequately supervised. Nurturing tones were heard when Mrs. Quarles spoke with the children. I was greeted by Owner/Operator Mrs. Diana Quarles and I explained the purpose of the visit. Mrs. Quarles husband was present during the visit along with her three (3) children that were present in the home during the visit. The facility had a Five-Star License issued July 24, 2024, and has an eighteen-month compliance history score of one-hundred (100) percent prior to today’s visit. The Annual Compliance visit was completed on January 13, 2025. I observed Mrs. Quarles present with two (2) children. The children were observed eating breakfast. The items noted on the menu were served: cheerios, bananas, and milk. The following child care requirements were monitored and observed today using the Family Child Care Home Checklist and the 2025 Master Family Child Care Home Item Number Listing. Supervision: The children were adequately supervised. Nurturing tones were heard when Mrs. Quarles spoke with the children. Staff/Child Ratio: The staff/child ratio was maintained during today’s visit. CPR: Mrs. Quarles is current with CPR. First Aid: Mrs. Quarles is current First Aid. Special Training: Mrs. Quarles is continuing to complete ongoing in-service training. CBC Qualification: Mrs. Quarles (D. Quarles) does not have a current criminal background check (CBC) letter on file, and a violation was cited. Mrs. Quarles husband has a current CBC on file; Mrs. Quarles four (4) children have current CBC letters on file. ITS-SIDS: There are no infants enrolled at this time. Mrs. Quarles is current with ITS-SIDS training. The Infant Safe Sleep Poster and Policy were posted. Emergency Medical Care Plan: The Emergency Care Plan was posted. Administration of Medication: There are no children with medication, and there are no children with allergies. Storage of Hazardous Substances: I did observe a hazardous substance that was accessible to children, and a violation was cited. Storage of Medication: No storage of medication. General Safety: There are no safety concerns. Discipline: There were no discipline concerns; appropriate discipline was provided. Adequate/Approved Space: The children present during the visit were observed in licensed approved space. Program Records: I reviewed all information required to be posted. The monthly fire drill was complete and up to date, and the quarterly drill (shelter-in-place/lockdown drills) were and up to date. The playground inspections were up-to-date. The last Sanitation Inspection was completed January 14, 2025 with a demerit score of zero (0). The water was tested April 2, 2024.
