N. Murphy, Preschool Lead Teacher, assisted me with today’s visit. Your program currently operates with a Five Star Rated License, issued July 2, 2022. The program’s compliance history before today’s visit was ninety-five percent. The NC Secretary of State website was reviewed on November 5, 2025, and Magnolia Preschool, LLC was listed as current- active. I conducted a walk-through of the indoor and outdoor environment today. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free choice of indoor activity areas, personal care routines, and teacher directed activities. The caregivers were observed interacting with children in a nurturing and caring manner. An approved fire inspection was received on October 1, 2025. The sanitation inspection was completed July 17, 2025, with a “Superior” classification. The last fire drill was completed on October 1, 2025. A lockdown drill was conducted on August 4, 2025. Ten percent of children’s records were monitored today. Staff files were also monitored. A criminal background check employee roster was reviewed. All staff and household members have been entered and connected to the facility. The following violations were observed today. Violation Number Comment Rule 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. There were eight exposed nails on on the wooden fence to the right of the playground. GS 110-91(6); .0605((i) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. One child's emergency medical information has not been updated since 2022. .0802(c) 1422 Potentially hazardous items such as archery equipment, hand and power tools, nails, chemicals, or propane stoves, were not stored in locked areas or with other safeguards, or removed from the premises. There were hand tools such as a pitch fork stored in an unlocked storage building on the playground. .2506(c) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. The violations documented must be corrected immediately. On or before November 19, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Thermometer: Refrigerators used for food and beverage served to the children must have a thermometer and a temperature of 45 degrees Fahrenheit or below. A thermometer was placed in the refrigerator in space #2 during the visit. Medication: Prescription and over the counter medication such as diaper ointments, teething gel, sunscreen etc. must be accompanied by authorization from the parent before administering. Annual Licensing Fee: Invoices will be emailed to your facility address on file by November 30, 2024. Online payments will be due by December 31, 2024. Murphy, Operator/Lead Teacher assisted us with today’s visit. A walk through of the indoor learning environment was conducted. Children were observed participating in teacher directed activities, free play of indoor activities, and transitioning to outside. Caregivers were observed interacting with the children in a nurturing and caring manner. The last fire inspection was conducted on October 13, 2023. The last sanitation inspection was conducted on November 21, 2023 with a “Superior” classification. Fire drill and lockdown drills were conducted on April 3, 2024. Outdoor safety checks were also monitored today and occurring monthly as required. The were two violations cited and one corrected today. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There was an aerosol can of sunscreen and bug repellent stored in a unlocked cabinet .2820(b) 846 Over-the-counter medicines were not in their original containers or administered as authorized in writing by parent, physician or authorized health professional. Two children under two years of age had Motrin on site with parent permission however, the recommended dosage states not for children two and under without doctor's permission.
