Today’s visit was with you, Ms. Betty McGriff. This facility currently operates with a three (3) star rated license effective August 4, 2017. I was greeted by Ms. Betty as she opened the door as I walked on the deck. I greeted her and stated the purpose of my visit. Ms. Betty assisted me with todays visit. Ms. Betty currently has two children enrolled full-time and one school-aged child that attends during the school year. Two children were present today and they were watching television when I walked inside of the facility. Children were served spaghetti o’s and meatballs with tomato sauce, applesauce and milk. Transitions from eating, playing, and nap time. Children were observed playing in centers as well. INSPECTIONS: The last documented fire drill was conducted 06-12-2026. The last documented shelter-in-place or lockdown drill was conducted 06-15-2026. The last documented playground inspection was conducted 06-5-2025. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility does not provide transportation. It was stated that there are no fire-arms on the premises. The attached checklist was used to note the requirements we monitored today. During this visit you stated that you are ready to retire and that you are planning on closing the facility when the two children are no longer attending. When you decide to terminate, please contact your consultatnt and send a email or letter stating your intentions and the day you desire to close yoru facility. Rated License: Ms. Betty and I discussed re-assessment of star rated license application. We reviewed and completed the application while I was present. Education was also reviewed. You have decided to do Pathway two, based on the current standards and education you will remain a three-star facility. Violation Number Comment Rule 543 Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. Children were watching tv upon arrival. .1718(c) 908 Health questionnaire was not completed annually. last completed on January 2025 .1703(a)(1) 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. Started but has not completed all required trainings. .1703(d)(2) Violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. REQUIRED RESPONSE: You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. Today’s visit was with you, Ms. Betty McGriff. This facility currently operates with a three (3) star rated license effective August 4, 2017. Upon arrival I was greeted by Ms. Betty outside. I stated the purpose of my visit and walked around to the interest of the child care area. Ms. Betty stated that she will not have children until later in the morning. Ms. Betty currently has five children enrolled. When I first arrived, there were no children present. During the visit, two children arrived. I observed greeting and arrival of children, Children were served yogurt and oatmeal for their morning snack and spaghetti and meatballs with tomato sauce, backed beans, mixed fruit, and milk. The infant was served carrots, potatoes and milk. Transitions from eating, playing, and nap time. Children were observed playing in centers, eating breakfast and lunch, outdoor gross motor play. Diaper changing was observed and handwashing. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last documented fire drill was conducted 07-3 -2025. The last documented shelter-in-place or lockdown drill was conducted 05-12 -2025. The last documented playground inspection was conducted 07-3 -2025. Last compliance history was 93%. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility does not provide transportation. Rated License: Ms. Betty and I discussed re-assessment of star rated license application that was received and dated for June 2025. If your education has not changed you will remain a three-star facility. We reviewed the self-assessment today and potential points received. Violation Number Comment Rule 806 Operator did not wash his/her hands before and after handling food and feeding the children. After provider placed children in the play area, she immediately started preparing their lunch and this requirement was not observed. .1725(a)(7)(A) 816 Each child's hands were not washed before and after eating. Children were playing in the play area, and when lunch was prepared they were sat at the table an served their meal. Handwashing was not observed. .1725(a)(7)(B) 1804 Safe sleep policy and poster about safe sleep practices was not posted in a prominent place in the infant sleeping room or area where parents and caregivers are able to view daily. there was no policy posted, only the poster was posted. .1724(b) 1953 Operator did not ensure child's hands were washed upon arrival at the home each day. Children arrived and sat down to eat morning snack. .1725(a)(4) 1983 Children's special diets or food allergies were not posted in the food preparation area and/or in the child's eating area. Today’s visit was with you, Ms. Betty McGriff. This facility currently operates with a three (3) star rated license effective August 4, 2017. Upon arrival I was greeted by Ms. Betty who let me in the facility. Ms. Betty had an additional caregiver with her today, who met requirements. Ms. Betty currently has six children enrolled. When I arrived, There were two children present today. Children were observed playing in centers, watching television, and eating. Children were served chicken nuggets, mixed vegetables, and milk. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last documented fire drill was conducted 07-08 -2024. The last documented shelter-in-place or lockdown drill was conducted 07-08 -2024. The last documented playground inspection was conducted 06-14 -2023. Last compliance history was 92%. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility does not provide transportation. The provider stated that there are no weapons on the premises. A one year old was watching television. .1718(c) 716 Electrical outlets not in use were not covered. there were three uncovered outlets in the room where children sleep. 10A NCAC .1719(a)(27) 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). Providers background check expired in 2021, provider had another letter on file that belonged to someone else with a similar name but the address and birthdate was different. Upon arrival I was greeted by Ms. B. McGriff. I shared the reason for the visit and she assisted me during today’s visit. Your program currently operates with a three (3) Star Family Childcare License effective 8/4/2017. The license was posted, with restrictions of first (1st) shift/daytime care, children on ground level only, a maximum of five (5) preschool children at any time, fuel burning stove not used during operating hours and no more than two (2) children under age one. The license was posted and the restrictions were in of compliance. Ownership: The facility owner Ms. Betty McGriff. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The compliance history is ninety-one (91%) percent as of today’s visit and this information was reviewed with the family child care homes operator. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The following items above were found out of compliance. The following inspections are below. •Last fire drill was conducted on 1/5/2024 and is out of compliance. •Last Shelter in Place/Lockdown Drill was conducted 2/9/2024 and is in compliance. •Last Playground Inspection was conducted in January of 2024 and is out of compliance. The children have access to a play area designed for childcare, restroom, and the outdoor learning environment. Indoor Learning Environment: The children had finished breakfast which consisted of peaches, corn flakes, and milk and were transitioning to screen time, restroom and outside learning. Outdoor Learning Environment: The children have access to the upper deck which is supplied with a small climbing structure and several different types of learning toys. The children do not use any other part of the yard. Program Records: I reviewed all the required records. Children’s Records: The facility is licensed for 1st shift and has four (4) children enrolled. None of the children enrolled reside in the home. There were two (2) children present during the visit. Medication: No medication is administered. Transportation: Transportation is not provided. Weapons: Your facility reported that they were in compliance during today’s visit with Child Care Requirements regarding firearms. Three (3) violations were observed today. One (1) was corrected during visit. The violations are as follows: Violation Number Comment Rule 716 Electrical outlets not in use were not covered. In the smaller room that serves the children older than two, there were two (2) outlets uncovered. 10A NCAC .1719(a)(27) 1544 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. There last playground inspection was conducted in January of 2024. GS 110-91 On or before 3/18/24 I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. Family child care homes are required to test water for lead once. o Child care rule 10A NCAC 09 .1725(a) for FCCH, to assure the health of children through proper sanitation, the family child care home operator shall: (2) collect and submit samples of water from each water outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. Results of the analysis shall be on file in the home. water samples shall be collected by the operator and tested within 12 months of the effective date of this Rule. o DCDEE encourages you to utilize the Clean Classrooms for Children’s program. You can take the pre-enrollment webinars which is available at www.cleanwaterforUSkids.org/carolina then enroll – At www.cleanwaterforUSkids.org/carolina completing the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos. Keep mindful that once you evaluate and potential hazards and receive results if there are recommendations and/or water mitigation support is needed they will provide direction in how to mitigate and ways to fund said mitigation. •There is a free virtual Training for Family Child Care Centers Providers on March 13th at 1:00 pm. Go to teaching strategies and click on the registration website to register in advance. •The NCAEYC annual conference is September 20th to the 21st. Must be a NACEYC member to attend. •Resources such as Child Care Resource and Referral and Smart Start can be valuable when trying to increase the quality of programming in your FCCH. •On the NCRLAP website, there are training videos, sample questions and clarifying information regarding the assessments you may find helpful. •For the latest information on child care rules and regulations, please visit the Division of Child Development and Early Education's website at https://ncchildcare.ncdhhs.gov. •There is a new Summary of Laws on the DCDEE website, please make sure it is updated. •As a reminder all screen time is to be limited to thirty minutes (30) daily and no more than two and one half (2.5) hours per week. Upon arrival I was greeted by Ms. B. McGriff. I shared the reason for the visit and she assisted me during today’s visit. Your program currently operates with a three (3) Star Family Childcare License effective 8/4/2017. The license was posted, with restrictions of first (1st) shift/daytime care, children on ground level only, a maximum of five (5) preschool children at any time, fuel burning stove not used during operating hours and no more than two (2) children under age one. The license was posted and the restrictions were in of compliance. Ownership: The facility owner Ms. Betty McGriff. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The compliance history is ninety-one (91%) percent as of today’s visit and this information was reviewed with the family child care homes operator. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The following items above were found out of compliance. Today’s visit was with you, Ms. Betty McGriff. This facility currently operates with a three (3) star rated license effective August 4, 2017. Upon arrival I was greeted by Ms. Betty who let me in the facility. Ms. Betty currently has seven children enrolled. When I first arrived, I waited a couple of minutes for the provider to open the door. When she opened the door she stated she was getting the infant. There were three children present and later two additional children arrived. A child’s mother was present during the visit and played games with the children. Children were observed playing in centers, eating breakfast which consisted of oatmeal, peaches, and milk. Diaper changing was observed and handwashing. The attached checklist was used to note the requirements we monitored today. INSPECTIONS: The last documented fire drill was conducted 05-112 -2023. The last documented shelter-in-place or lockdown drill was conducted 05-12 -2023. The last documented playground inspection was conducted 06-14 -2023. Last compliance history was 85%. I monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management, children’s files, and staff files. The facility does not provide transportation. Ms. Betty and I discussed re-assessment for star rated license IN 2022, I printed the application for her to complete and she gave her self-assessment. I did not receive the application, so I am evaluating the points to administer a new star rating. If your education has not changed you will remain a three-star facility. We completed the application today. Violation Number Comment Rule 542 When screen time was provided, it was not offered to stimulate a developmental domain in accordance with NC Foundations for Early Learning and Development. when I arrived the television was on and all children were under three years of age. .1718(b)(1) 811 Baby food, snack items, and meal items sent from home, for each individual child, were not dated and labeled with the child's name.
