The Secretary of State database was monitored, and LITTLE PEOPLE’S PLAYHOUSE LLC. is current and active. I was greeted by you, P. Vann owner/lead teacher and D. Williams, teacher. There were 7 children present during the walkthrough of the facility. I conducted a walkthrough of the facility. I observed the children participating in free play and teacher directed activities. The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required. Shelter and place/lockdown drills were documented as required. The playground safety inspections were reviewed and documented. The center last had a sanitation inspection on 8/7/25. The center received 8 demerits and received superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 11/14/25. The inspection was completed by the Mecklenburg County Fire Department. During today’s visit, I monitored the facilities vehicle used to transport children. The playground was monitored today. All staff files were monitored today. Ten percent of children’s files were monitored. Ms. Vann completed the Operator’s Statements of Responsibility form during the visit. Ms. Vann is the only individual residing at the home. There were 3 violations cited during today’s visit. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The playground has a double seated chair and the paint is peeling. G.S. 110-91(6); .0601(b) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A Lysol in an aerosol container was accessible to children. .2820(b) 1110 Vehicles used to transport children enrolled in the child care center did not comply with all applicable State and federal laws and regulations. There was no documentation of current registration on file for the Ford Transit 150 that is used to transport children. .1002(b) Technical Assistance: -I shared with Ms. Vann to add mulch to the playground. - QRIS Modernization Update: -Today, I had the QRIS conversation with Ms. Vann. This center has a four star license and an 18 month compliance history score of 93%. The license and NC child care law summary were prominently posted. Upon arrival, I was greeted by P. Vann, owner. I conducted a walkthrough of the facility. There were 12 children present during the walkthrough of the facility. During the visit, I observed the children participating in story time, music and free play. The following items were monitored today: supervision, staff/child ratios, permit restrictions, license posted, approved space, storage of hazardous products, storage of medication, staff records and program records. There have been no new staff since my last visit. The staff and training worksheets were reviewed to confirm existing staff were still current with CPR, First Aid, ITS-SIDS training and criminal background qualifying letters. The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required. Shelter-in-place and Lock-Down drills were documented quarterly as required. The playground safety inspections were reviewed. Reports were documented monthly as required. There were 2 violations cited during today’s visit. The violation was corrected during the visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. There were 2 electrical outlets not covered. 10A NCAC 09 .0604(c) 830 A First Aid information sheet was not posted in a place for referral and/or the information sheet did not include all the required information. The first aid information sheet was not posted. .0802(h) Technical Assistance: --I discussed the ABCMS Portal with the administrator. -I provided Ms. Vann with rules regarding increase in capacity. - QRIS Modernization Update: The Division of Child Development and Early Education is preparing a summary of the rules, an online training module for Moodle, and an implementation plan. In the crib/nap area there was a torn plastic climbing equipment. On the playground, there was a cracked wagon. .0601(d) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. There was a foot cream medication on the van use to transport children. 15A NCAC 18A .2820(d) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. T. Garrison medical was completed after employment. 10A NCAC 09 .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks.
