Upon arrival I knocked on the door and was greeted by Mrs. Owens, I greeted her and stated the purpose of my visit. I walked into space one greeted Ms. B, stated the purpose of my visit and proceeded with the monitoring visit, unassisted. This facility currently operates with a three-star rated license effective February 10, 2020. During the visit Doretta Owens and Ms. B, assistant administrator, assisted me with the visit. Infants and young toddlers were observed eating lunch and holding their bottles, sitting in the bouncy chair, and playing with toys and napping. Children were observed in circle time at the table, playing with manipulatives, two year olds were also engaged in group activity. Today’s menu was posted had ranch chicken and potatoes casserole with veggies, oranges, and milk. Children were served corn dogs, French fries, mixed tropical fruit and milk. Children were observed playing with blocks napping, transitioning and bathroom routines. Naptime transitions were also observed. INSPECTIONS: The last sanitation inspection was conducted 02-24-2026. The last fire inspection was conducted 3-13-25. The last documented fire drill was conducted 04-15-2026. The last documented shelter-in-place or lockdown drill was conducted 04-20-2026. The last documented playground inspection was conducted 04-08-2026. The last compliance score was 94% I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. The attached checklist was used to note the requirements we monitored today. The staff and training worksheet was completed during this visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed in March 2025. 10A NCAC 09 .0304(a) 428 A current activity plan was not posted for each group of children for reference. Activity plan was dated April 2026. GS 110-91(12); .0508(a) 438 Developmentally appropriate equipment and materials were not provided for variety of outdoor activities for vigorous play and large muscle development. The kitchen door was unlocked and open. Today’s menu was posted had ranch chicken and potatoes casserole with veggies, oranges, and milk. Children were served beef stew with vegetables, rice, peaches and milk. Children were observed napping, transitioning and bathroom routines. Naptime transitions were also observed. INSPECTIONS: The last sanitation inspection was conducted 02-18-2025. The last fire inspection was conducted 3-13-25. The last documented fire drill was conducted 05-2-2025. The last documented shelter-in-place or lockdown drill was conducted 03-11-2025. I conducted the walk-though of the facility unassisted, as Ms. R. was covering in a classroom. Children in three one was observed having story time and later participating in art. In space four, children were transitioning from circle time to activity center play. In space five children were observed in circle time participating in a object hunt. In space two children were very active in dramatic play, reading, science, and blocks today. In space one, children were having tummy-time and one was crawling. We discussed the environmental rating scale view worksheets limiting creativity and that if they did more free choice instead of teacher lead, they could receive higher points. Please review and share with teachers the website, (www.ncrlap.org), for examples and videos to help prepare for future environmental rating scales. 2. Contact Your local CCRI(Child care resource and Referral agency), to assist you in preparing for the ERS-3 and best practice. 3. CBC Provider Portal: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new If the new hire or resident. Upon arrival I knocked on the door, Ms. R opened the door. I greeted Ms. R. and stated the purpose of my visit. She made me aware that the Director was out today. I conducted the walk-though of the facility unassisted, as Ms. R. was covering in a classroom. Children in three one was observed having story time and later participating in art. In space four, children were transitioning from circle time to activity center play. In space five children were observed in circle time participating in a object hunt. In space two children were very active in dramatic play, reading, science, and blocks today. In space one, children were having tummy-time and one was crawling. Children were served soup, sandwiches, and fruit for lunch. observed eating a snack that consisted of peanut butter cheese crackers and milk The following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure were found in compliance. There are no new staff at the facility. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The last fire drill was conducted 12/05/2024. .0604(t); .0302(d)(5) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. in space three (Toddlers), there were trash bags undeer the sink behind the changing table. .0604(q) 861 Prohibited styrofoam and foam rubber products were accessible to children under 3 years of age and/or approved foam products were used without proper supervision. The teachers lounge/closet was unlocked, styrofam cup was preseent. .0604(q) violations: Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. REQUIRED RESPONSE: You shall correct the violation(s) immediately. Environmental Rating Scales / Best Practice: Several teachers were observed using pre-drawn coloring sheets. I spoke with the teachers about the importance of free choice art. We discussed the environmental rating scale view worksheets limiting creativity and that if they did more free choice instead of teacher lead, they could receive higher points. Please review and share with teachers the website, (www.ncrlap.org), for examples and videos to help prepare for future environmental rating scales. 2. Contact Your local CCRI(Child care resource and Referral agency), to assist you in preparing for the ERS-3 and best practice. 3. CBC Provider Portal: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new If the new hire or resident. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster.
