I was greeted by Mrs. Candice Thompson, administrator who assisted me with today’s visit. This facility currently operates with a three (3) star rated license effective November 07, 2017. Space one was closed today and children were combined in space two. The following centers were available books, dramatic play, music, art, books, and fine motor toys. Children were served Salisbury steak, mashed potatoes, peas, mixed fruit, and milk for lunch. The children then transitioned to nap after toileting and handwashing. Children were not observed participating in outdoor gross motor play today. INSPECTIONS: The last sanitation inspection was conducted 01/29/2026. The last fire inspection was conducted on 01/18/2026. The last documented fire drill was conducted 02/05/2026. The last documented shelter-in-place or lockdown drill was conducted 01-03-2025. The last documented playground inspection was conducted 2/03/2025. Your compliance history was 94 % prior to today’s visit. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program and management. The outdoor environment was not observed today due to active precipitation, consultant will monitor at the next visit. The facility does not provide transportation. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. The facility does not have any new staff. Violation Number Comment Rule 705 Equipment and furnishings were not sturdy, stable and free of hazards. all metal structures has peeling paint. .0601(c) 721 All equipment and furnishings were not in good repair. The steps on the climbing structure a chipping, broken near the joint that holds them to the structure. G.S. 110-91(6); .0601(b) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. the gate measures at 45 inches and 47 inches in the back right where the section of the fence is broken. GS 110-91(6); .0605((i) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Staff file monitored only received 14 of the 15 hours of training needed. .1103(a) 1867 The depth of the loose surfacing was not based on critical height of the equipment. mulch measures at 4 inch in some spots. .0605(k)(1-4) Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. I proceeded with the walk through of the facility unassisted as the administrator was not onsite. During the monitoring, the administrator arrived and assisted me with the remainder of the visit. At arrival children were observed napping, after nap children transitioned to cleaning up, handwashing and snack which consisted of Cheese crackers, and apple juice. Children were then engaged in outdoor gross motor play. The following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure. The facility does not have any new staff. During today’s visit we discussed rating scales and accreditation options, and possibilities to increase or decrease in stars, (see Technical Assistance). We also revisited the conversation of adding the administrator to the license as the owner. We discussed that the license cannot be brought, sold or transferred to another entity without the proper documentation. The last fire inspection was conducted on 01/18/2025. The last documented fire drill was conducted 02/04/2025. The last documented shelter-in-place or lockdown drill was conducted 01-03-2025. This legislation allows facilities to remain at their current star level without a reassessment until June 30, 2024 and when reassessed it reduces the education evaluation requirements for lead teachers from 75% to 50% until June of 2026. To prepare for the reassessment process, DCDEE has developed a cohort system. There are three cohorts, and each include a preparation year and a reassessment year. While the first reassessment year will not begin until July 1, 2024, the preparation year for cohort one begins July 1, 2023. Your facility is in cohort one. Beginning in July and throughout the next few months I recommend you consider completing the following tasks and activities that will help prepare you for your reassessment year, which will be sometime between July 1, 2024 and June 30, 2025. I was greeted at the door by Ms. V. Hood who asked me to go the the second door where I was greeted by Ms. S. Bolton. I restated the purpose of my visit, asked if the administrator was present and then conducted a walk-through of the facility unassisted as the administrator not onsite. Later during the visit, Candance Thompson, administrator joined me. Upon arrival children were observed transitioning from lunch time to nap time. Space one had seven children present but six enrolled. I asked the teacher where the seventh child came from and she stated that she walked two children down from the other building. And tw children do not belong too her. I looked at the enrollment/sign-in sheet and looked at all applications. This classroom is a two year old class. There was one child that was over one years old which brought the ratio down to 1:6. In the second classroom there were five children present. I was greeted at the door by Ms. V. Hood who asked me to go the the second door where I was greeted by Ms. S. Bolton. I restated the purpose of my visit, asked if the administrator was present and then conducted a walk-through of the facility unassisted as the administrator not onsite. Later during the visit, Candance Thompson, administrator joined me. Upon arrival children were observed transitioning from lunch time to nap time. Space one had seven children present but six enrolled. I asked the teacher where the seventh child came from and she stated that she walked two children down from the other building. And tw children do not belong too her. I looked at the enrollment/sign-in sheet and looked at all applications. This classroom is a two year old class. There was one child that was over one years old which brought the ratio down to 1:6. In the second classroom there were five children present. I conducted a walkthrough of the facility and the following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit The purpose of today’s unannounced visit was to monitor for compliance with child care requirements. restrictions, safe arrival and departure, and all required posted documents. Staff records were not observed as the facility does not have any new employees. The facility has added a fairly new playground structure and mulch to their outdoor environment. Violation Number Comment Rule 614 Bed, cribs, playpens, cots or mats were not placed at least 18" apart or separated by partitions when in use. In space one and two children were laying down cots. That cots were placed less than 18" apart and was measuring at 13 inches. 15A NCAC 18A .2821(e) 1756 Enhanced staff/child ratios and group sizes were not met.Two children were walked over from the second facility. One of the child that wear present were only 1 years old the ratio is 1:6. When I arrived the ratio was 1:7. 10A NCAC 09 .2818 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address November 3, 2023, 223. The format to follow is: -Name of your facility, -ID number, -date of the visit, -date you are submitting the letter, -citing each violation number, -when AND how it was corrected AND how you will prevent the reoccurrence of this violation, -Close with YOUR SIGNATURE REQUIRED RESPONSE: You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Rated License: We discussed that the State of Emergency has been lifted as of August 15, 2022, therefore the Division of Child Development and Early Education is preparing a plan on how to proceed with a plan of how to ensure the facilities that participate in the Voluntary Rated License program have up to date licenses. If you are interested in re-assessment of stars you will need to do so before February 2023. I will email a copy of the application to the administrator. In preparation I encourage that your program participates in the following tasks: *Have all staff update their information in the WORKS database. Have all teachers and staff print out their WORKS letter so you can see if their education is updated, also if any teachers have a BSAC certificate have them upload the certificate into their account. * Access the resources for your upcoming assessment at www.ncrlap.org. On the first page there is a section that allows you to access resources classrooms serving children birth through thirty (30) months ITERS-R and classrooms serving children thirty-one (31) months to five (5) years ECERS-R. *Review the materials lists for each classroom. This list is to be used as a resource reminding you of some of the required items to promote quality in programming. *Contact CCRI to have them assist you in providing guidance in preparation for the scales.
