This facility currently operates with a four (4) star rated license effective 04-01-2019. Upon arrival I knocked on the door and was greeted by Ms. Annee. I introduced myself and explained the purpose of my visit. She stated that the Administrator was not present and would be on her way. I proceeded with the monitoring visit unassisted. Toddlers were observed actively engaged in activity centers which included blocks, dramatic play, art, books, and music. Infants were observed enjoying soft toys, fine motor toys and interacting with staff. Transitions to lunch, diapering/toileting, and napping was also observed. Lunch consisted of wheat bread, apple sauce, peas, chicken and rice. The attached checklist was used to note the requirements we monitored today. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program, and management, children’s files, and staff files. INSPECTIONS: The last sanitation inspection was conducted 07-10-2025. The last fire inspection was conducted 3-25-2025. The last documented fire drill was conducted 7-7-2025. The last documented shelter-in-place or lockdown drill was conducted 7-2-25. The last documented playground inspection was conducted 7-7-25. The last compliance prior to this visit was 93% Violation Number Comment Rule 523 The activity plan did not include a daily gross motor activity which may occur indoors and outdoors. Infant and toddlers did not have gross motor play on the plan. .0508(g)(3) 542 The written feeding plan was not modified as the child's needs changed. Infants and one year old child that are eating table food or baby jar food was not reflected or updated on the plan. 10 NCAC 09 .0902(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Staff that worked for the facility who left and was re-employed in 2024 had a TB test conducted on 2022. .0701(a) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. Staff in the infant room took the Health and Safety infant/toddler sudden death syndrome and not the NC ITS-SIDS that is required. .1102(f) 1203 Operational policies were not discussed with parents on or before the child's first day and/or they were not notified in writing of all changes. There is no evidence of this happening. 10A NCAC 09 .0514(b) 1207 Parent participation plan was not discussed with parents on or before the child's first day of attendance and/or a copy was not given to them or posted in the center. There is no evidence/documentation showing that this happened. 10A NCAC 09 .0515(a) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. There was no staff development plan conducted. 10A NCAC 09 .0514(f) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. Children who were enrolled over a year did not have updated information. .0802(c) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. You could designate the start of the “school year” when you transitioning children up, or set a random date on your calendar with an alarm to remind you. Infant forms should be updated when their eating habits change. Rated License reassessment: “Hold Harmless” regarding reassessment of Stars has ended. Be on watch for the upcoming webinar and administrators meeting updating you on the New “Pathways” to reassessment/Stars. During this interim time I suggested that you take the following steps: *Have all staff update their information in the WORKS database. This facility currently operates with a four (4) star rated license effective 04-01-2019. Upon arrival I knocked on the door and was greeted by Ms. Annee. I introduced myself and explained the purpose of my visit. She stated that the Administrator was not present and would be on her way. I proceeded with the monitoring visit unassisted. Toddlers were observed actively engaged in activity centers which included blocks, dramatic play, art, books, and music. Infants were observed enjoying soft toys, fine motor toys and interacting with staff. Transitions to lunch, diapering/toileting, and napping was also observed. Lunch consisted of wheat bread, apple sauce, peas, chicken and rice. The attached checklist was used to note the requirements we monitored today. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program, and management, children’s files, and staff files. INSPECTIONS: The last sanitation inspection was conducted 07-10-2025. The last fire inspection was conducted 3-25-2025. The last documented fire drill was conducted 7-7-2025. The last documented shelter-in-place or lockdown drill was conducted 7-2-25. The last documented playground inspection was conducted 7-7-25. The last compliance prior to this visit was 93% Violation Number Comment Rule 523 The activity plan did not include a daily gross motor activity which may occur indoors and outdoors. Infant and toddlers did not have gross motor play on the plan. .0508(g)(3) 542 The written feeding plan was not modified as the child's needs changed. Infants and one year old child that are eating table food or baby jar food was not reflected or updated on the plan. 10 NCAC 09 .0902(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Staff that worked for the facility who left and was re-employed in 2024 had a TB test conducted on 2022. .0701(a) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. Staff in the infant room took the Health and Safety infant/toddler sudden death syndrome and not the NC ITS-SIDS that is required. .1102(f) 1203 Operational policies were not discussed with parents on or before the child's first day and/or they were not notified in writing of all changes. There is no evidence/documentation showing that this happened. 10A NCAC 09 .0515(a) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. There was no staff development plan conducted. 10A NCAC 09 .0514(f) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. Children who were enrolled over a year did not have updated information. .0802(c) 1323 Each child was not immunized as per Article 6 of Chapter 130A and an immunization record was not on file before or within 30 days after admission. Child did not have any immunizations in their file or a statement saying they do not get immunized. 10A NCAC 09 .0302(d)(2) 1831 At least one child care provider, who has completed ITS-SIDS training was not present in the infant room, while children were in care. Infant forms should be updated when their eating habits change. Rated License reassessment: “Hold Harmless” regarding reassessment of Stars has ended. Upon arrival I was greeted by Ms. B, teacher, I stated the purpose of my visit and proceeded with the walk-though of the facility unassisted. During my walk-through Mrs. C. Thompson, the administrator, arrived. Children were observed sitting at tables and highchairs. The children in highchairs were reading books. Infants were observed napping. Transitioning from nap to lunch was observed, initially children’s hands were wiped with a wet diapering wipe prior to eating. After a conversation with the teacher, children’s hands were washed at the sink. . Children were observed eating pasta raviolis stuffed with ground beef, corn and peas, applesauce, and milk The following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure were found in compliance. During this visit we discussed adding the administrator to the Licenses of both locations. I will look at both facilities applications and email you regarding the type of ownership that is listed and next steps. Depending on the results you may be subjected to completing a new application and going thru the steps of a new facility with a six month temporary license . DCDEE Regulatory system could not print today, so you were given a hand written visit documentation. Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. One child did not have a chart available for review. .0606(g) 9995 A violation was found for which there is no item number. Environmetal Health: 15A NCAC 18A .2803 HANDWASHING Children used wipes instead of washing hands at the sink. This facility currently operates with a five (5) star rated license effective 10-20-2019. Upon arrival I knocked on the door and was greeted by Ms. Annee. I introduced myself and explained the purpose of my visit. She stated that the Administrator was not present. I proceeded to conduct the visit. When I walked in the facility there was no one present in the first classroom. I asked Ms. Annee if she was here alone, and she said yes. She proceeded to tell me that she has six children and was going to call the administrator. At 10:43, the teacher/floater arrived to operate the toddler classroom. At 10:54 the Kitchen staff entered with lunch for the children. Lunch consisted of wheat bread, peaches, peas, milk, water, and macaroni and cheese with ground beef Children were observed in cribs. Later during the visit toddles were observed playing, eating lunch and napping. The attached checklist was used to note the requirements we monitored today. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program, and management, children’s files, and staff files. INSPECTIONS: The last sanitation inspection was conducted 04-13-2023. The last fire inspection was conducted 2-14-2023. The last documented fire drill was conducted 7-5-2023. The last documented shelter-in-place or lockdown drill was conducted 7-5-23. The last documented playground inspection was conducted 7-6-23. The last compliance prior to this visit was 96% Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. Consultant did not receive the ADULT DAY CARE & CHILD CARE FIRE INSPECTION REPORT within one week. The inspection was conducted on 2/14/2023 10A NCAC 09 .0304(a) 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met.
