Name of Operation: Little Angels Preschool Facility ID: 60004394 Consultant: RASHEKA WHEELER Operation Type: Center Case Number: Visit Date: 8/13/2025 Number Present: 27 Completed Date: 8/13/2025 Age: From 3 To 8 Total Minutes: 145 Time In: 09:00 AM Time Out: 11:25 AM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of my unannounced visit was to monitor for compliance with all applicable child care requirements for a second temporary time period. You, Marlaina Williford, assisted me with the visit today. Upon arrival, I met with Ms. Williford to discuss today’s visit and to go over Pathway #1, Pathway #2 and Pathway #3 option for her to make the best decision for her program. Ms. Williford stated that she is currently not interested in any of the pathways to the stars to remain on a license. She was made aware that she would receive a one-star. Ms. Williford stated she may find interest after she complete the temporary stage in license to focus on the requirements needed to successfully work towards a pathway that will meet the needs of the program. Currently this center operates with a Temporary License, issued from 4/07/25 to 10/07/25. The restrictions include daytime care only, with the building capacity of 39 for first shift; Age ranging from 3-12 years of age. Indoor Learning Environment: There were (27) twenty-seven children present during the visit and (30) thirty children enrolled in building. Upon arrival, children were engaging in story time. The fire drill report was discussed today and was observed in compliance with the latest date of 8/11/2025. The playground was observed upon arrival, but the program administrator will have until October 7, 2025, to receive her playground training. The most recent sanitation inspection was completed on 4/25/25; the most recent fire inspection took place on 6/25/25; the building inspection was completed on 7/23/25. During the visit, the Child Care Center Item Number Listing (DCD 0357) was used as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to this facility. In addition to monitoring for health and safety, three (3) children’s records (10%), center files and required postings. Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, arrival/departure procedures, summary of law, current menus, and the Temporary License. Records: Staff and training worksheet was completed prior to today’s visit. No new staff have been hired since the last visit. Nutrition: All children have an opt out form due to everyone bring in their own meals. Weapons: Your facility is in compliance with Child Care Requirements regarding firearms. One violation was cited during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. During the visit, I observed several students who was not signed in on Bright Wheel. 10A NCAC 09 .0302(d)(4) Your temporary license will expire on October 7, 2025, and no extension of time will be given. Technical Assistance: Please make sure that you are using the most up-to-date forms to post and our provide for families to complete upon enrollment and when changes occur with their child. I also discussed making sure that all classrooms have the correct space capacity for each classroom. Space #3/4 is allowed for 24 children. Space #2 is allowed for 8 children. Space #1 is allowed for 9 children. The hallways are not considered spaces for children to receive care. Noted: I spoke with Ms. Williford about the following information. The program completed a Change of Ownership on April 7th, 2025, and could request a harmless hold by taking the previous owner's rated star level which included the previous' program ERS Score and Education points. The administrator stated she is interested in that opportunity to remain in the hold harmless. I stated that our conversation about the harmless hold will be documented within this visit summary, stating that I will keep the previous ERS score and education from the previous’ owners last ERS assessment/Rated License. Ms. Williford stated that this will give her the opportunity to either gather additional information regarding the pathways to the stars or accept the one-star status. We also discussed making sure she has a back-up version of attendance and sign-in on paper just in case Bright Wheel is not operating accordingly during the visit.
