This center has a three-star license and an 18-month compliance history of 100%. The NC child care law summary was prominently posted. I was greeted by M. Moore, site coordinator. I conducted a walkthrough of the facility. Staff/child ratio, Supervision, capacity, group size and space were observed in compliance today. Group 1 there were 13 children present with the site coordinator. Group 2 there were 13 children present with a group leader. The center’s Emergency Drill Log and Report was reviewed. Fire drills were reviewed. Shelter in place and Lock down drills were monitored. Playground inspection reports were monitored. The playground was not monitored during today’s visit. Ms. Moore shared with me that they are currently using the black top area as the playground needs mulch. The facility last fire inspection was conducted on 5/28/24. There was no documentation of a current fire inspection on file. The facility’s last sanitation inspection was conducted on 3/31/2025, superior with 0 demerits. Staff and Children’s DPI verification forms were monitored today. The children were observed participating in arrival, handwashing, snack and free choice. I used the staff and training worksheet to monitor staff files. There was 1 violation cited during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. There was no documentation of a current fire inspection on file. 10A NCAC 09 .0304(a) Technical Assistance: - --- QRIS Modernization Update: -Today, I had the QRIS conversation with Ms. Moore. A QRIS conversation report was completed, signed by Ms. Moore and she was given a copy. Please submit a written, signed and dated statement to me at the email address below detailing the steps taken to correct each violation. Your letter must include the following: 1. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation using the item listing in your visit summary, you must state how each violation was corrected and are now in compliance. 5. Name of Operation: Lincoln Heights Montessori ASEP Facility ID: 60004299 Consultant: MICHELLE CUTHBERTSON Operation Type: Center Case Number: Visit Date: 1/10/2024 Number Present: 17 Completed Date: 1/10/2024 Age: From 4 To 10 Total Minutes: 105 Time In: 03:00 PM Time Out: 04:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of my unannounced visit was to monitor for compliance with all applicable childcare requirements for a temporary time period. Currently this center operates with a Temporary License, issued on October 9, 2023. The restrictions include daytime care only. This center is on a temporary license due to a location change. Upon arrival, I was greeted by I. McNeil, group leader. I stated the reason for the visit. Ms. McNeil stated that Ms. Moore, site coordinator, would not be present for today’s visit. Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. The last fire inspection was made on 10/2/2023. The last sanitation inspection was made on 12/14/23. The center received a Superior classification with 7 Demerits. The outdoor inspections and fire drill report forms were reviewed during today’s visit. The forms were current and up to date. I monitored the outdoor learning environment. I reviewed the center’s DPI verification forms. I monitored staff files. There were 17 children in attendance. They were observed participating in arrival, snack and free play. For snack, the children were eating string cheese, graham crackers and 100% apple juice. The snack menu was not current with what the children were eating. This was corrected during the visit. There was 1 violation cited during today’s visit. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. The menu was not current with what the children were having for snack. 10A NCAC 09 .0901(b) Technical Assistance: Star Rating: Prior to the change of location, the facility was a 3 star Rated License. I discussed with Ms. McNeil that the facility is currently in hold harmless and that we would be using the rated license paperwork from the old location to put the facility back on a 3 star license. I shared that the facility would be following the old location timeframe for the next rated license evaluation period.
