The last Annual Compliance visit was made on 3/12/2024. The 5 Star Rated License was issued on 1/13/2020. I was let into the facility by operator Silvia Fernandez. She was present with five (5) children. The November 2024 version of the Family Child Care Home Master Item Number Listing and a checklist was used to note the requirements I monitored today. This facility has a compliance percentage of 94% as of today. Two (2) household members were also present during today's visit. Children were observed having free play. There were toys and activities available. Attendance was monitored. Summary of law, schedule, summary of law, first aid sheet, and emergency numbers were posted. Monthly Fire Drills, Emergency Preparedness Drills, and Playground inspections were monitored. A sanitation inspection was completed in 4/17/2024. Children's files were monitored for Application, Emergency Care Authorization, Medical/Immunization, Discipline Statement, Summary of Law, No smoking statement, Shaken Baby Syndrome policy, and Outside fence permission. The provider's file was monitored for Criminal Qualifying Letters, First Aid and CPR coverage, Health and Safety Trainings, Shaken Baby Syndrome Policy, Special Trainings, On-going Training hours, and Annual Health Questionnaire. This facility does not provide transportation. There are no children enrolled with allergies or medication present. The bathroom and kitchen areas were monitored. The following violations cited during today's visit: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A copy of one (1) enrolled child's health assessment was not on file within 30 days of enrollment. During the visit I observed children in free choice activities, a teacher directed music activity, large group time, and outdoor time. I heard you using nurturing tones when speaking with the children. You were engaged with all the children, sitting with them on their eye level, and participating in play with them. Supervision and staff/Child ratio’s were maintained. General safety requirements were monitored and found meeting compliance. You stated, currently you do not have any medications to administer. Cleaning supplies were observed locked, outlets were observed covered. I reviewed your file. You and all members of your family living in the home have a current DCDEE criminal record qualifying letter. You have current CPR/FA certification and have completed all the health and safety trainings within the last 5 years. You completed EPR training on 8-29-16. Information required to be posted were observed posted on the bulletin board, as you enter the home. The daily schedule was reviewed and found meeting compliance. The current activity plan was reviewed, it does not meet compliance with 10A NCAC 09 .1817(8). FCCH Capacity Increase – you inquired about increasing your capacity. A blast email was sent out in October, I have printed that out for you today, regarding what is required in order for family child care home providers increase capacity. When increasing capacity you are changing from and R3 (residential 3) to and R4 (residential 4), which means you must meet the requirements for an R4. FCCH providers must provide the following to their child care consultant, along with a signed written request to increase capacity. 1) Building permit (application submitted at local inspection office) 2) Building Inspection and Fire Inspection (building inspection form will be revised to include FCCH) 3) Fire Alarm System that will notify a professional offsite monitoring service (acceptable alternate method would be a Honeywell Home System, ADT Security, SimpliSafe, CPI Security and Brinks Home, etc.) 4) Carbon Monoxide Detectors 5) Portable Fire Extinguishers (type 2-A: 10-B-C) placed every forty (40 ft) feet within the child care area. 6) Minimum of two means of egress (exits) required (this does not include windows), one exit is required to be a ramp or the exit must be at grade. 7) Exits must having lighting. 8) Interior walls and ceilings must be made of noncombustible material (ex: no plaster or gypsum wallboard or wood paneling) I explained today would be my last visit, as I will be retiring from my position next friday. I will send and email out the day before I leave providing you with information on who to contact until a new consultant is assigned to the territory. You, Silvia Fernandez, Owner/Operator assisted me with the visit. Your program currently operates with a 5 Star Rated License effective January 13, 2020. The center’s 18 month compliance history, prior to today’s visit 99%. Upon my arrival, you and the four children present were outdoors. Once I entered the home, we went back outside so you could finish your outdoor time. I observed children running in the yard and playing on the small slide/climber. I observed landscaping material sticking out in three areas around the climber; this poses a tripping hazard. I encourage you to trim back or rake mulch over the areas to completely cover the landscaping material. You conducted a teacher directed art activity on the back porch and then came inside. Once inside, I observed you and the children washing hands and then you all went into the room set up for child care to have center play. I observed you sitting with the children and playing with them, assisting with interlocking manipulative, allowing them to pretend do your hair, reading books and playing with blocks. Supervision was observed being maintained. Staff/child ratio was observed being maintained. I monitored the room used for child care, the bathroom and kitchen area today. I observed one outlet uncovered in the bathroom children use during operating hours, you corrected this by using an outlet cover. I observed Lysol disinfectant and Target Brand All Purpose cleaner on kitchen counter, you locked them in a cabinet below the kitchen sink. I also observed Dawn Power Wash by the sink, below 5 feet and it states keep out of reach of children. Currently you do not have any medication to administer to children. I observed your family’s medication in the locked cabinet below the sink. Program records were observed meeting compliance. All information required to be posted was observed posted on the bulletin boards as you enter the home. The License was also posted in the entrance of the home, all permit restrictions were observed meeting compliance. The last sanitation inspection was conducted in April 26, 2023. While reviewing the daily arrival and departure attendance, two of the children present did not have their arrival time documented. This was corrected during the visit. I reviewed your file. You and all members of your family living in the home have a current DCDEE criminal record qualifying letter. You have current CPR/FA certification and have completed all the health and safety trainings within the last 5 years. You have not kept your ITS/SIDS training current. You informed me you don’t care for infants and don’t want to in the future. I will issue a new license with the age range stating you will only be serving children 1-12 year. The following violations were cited during today’s visit: Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. Outdoors I observed landscaping material sticking out above the mulch in three areas by the slide/climber, which is considered a tripping hazard. .1719(a)(1)&(17) 716 Electrical outlets not in use were not covered. I observed one outlet uncovered in the bathroom children use during operating hours. 10A NCAC .1719(a)(27) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. While reviewing the daily attendance, I observed two children present today that did not have their arrival time listed. .1721(e)(6) 1940 Hazardous cleaning supplies and other items that might be poisonous e.g. toxic plants, were not out of reach or in locked storage when children were in care. I observed Lysol disinfectant and a Target Brand All Purpose cleaner on the kitchen counter by the refrigerator. I also observed Dawn Power Wash, which states keep out of reach of children by the sink. .1719(a)(6) Compliance Letter: Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before November 14, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. If you email the compliance letter, it must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Technical Assistance was provided on the following: General Safety - I encourage you to implement a daily opening checklist to use as you check the child care spaces each day prior to the children’s arrival. The checklist could list general safety items such as all outlets covered, material in good repair indoors and outdoors such as not broken, no splinters, no choking hazards, hazardous cleaning supplies locked when not in use, no tripping hazards, outdoors play area for debris, vandalism, broken equipment, and animal waste, in addition to anything else you check prior to children's arrival each day. This may help you maintain compliance with three of the four violations cited during today's visit. I provided a Hazardous Item storage document for you to refence; itis can also be found on the DCDEE website under Provider Documents. Attendance: Two children present did not have their daily arrival listed, you corrected this during the visit. You stated that with it being a holiday today the children and parents were excited upon arrival, and you forgot to document the last two children who arrived. I suggested keeping the sign in/out log in the child care room, maybe seeing it will help you remember on document their times as they arrive on those more hectic days. Other: Rated License: Even though the last license issued was done in 2020, it was changed due to your education level increasing, you did not have the FCCERS-R completed in 2020. The last FCCERS-R conducted were in October 2018. I explained that your license is currently in a hold harmless period until June 30, 2024. You will be reassessed during Cohort 1 because you were last fully assessed in 2018. From July 1, 2023 to June 30, 2024 you are in the preparation year. You will be reassessed during the July 1, 2024 to June 30, 2025 year. At your request, you may choose to have the FCCERS-R conducted during your preparation year, the assessment scores will be applied to a program’s license and program standards points only if this helps maintain or increase a star rating. A program may decide to wait until Year 2 to apply the scores. Resources are available to you during your preparation year. I encourage you to contact CCRI for additional support, utilize the NC Rated License Assessment Project Website (NCRLAP.org) which has many resources regarding the FCCERS-R, and you may contact me for a Technical Assistance Visit where we would concentrate on the FCCERS-R. Age Range Change: During the visit while I was reviewing your file, you did not keep your ITS/SIDS training current. You stated you don't want to care for infants any longer.
