Upon arrival I was greeted by you, Ms. Sheila Beasley, I shared the reason for the visit, and you were able to assist me. Permit Information: The program currently operates with a Five-Star center license effective 12/2/25. The license was posted, with the following restrictions: - 1st shift - 2nd shift - meets enhanced ratios Ownership: The ownership information was confirmed as Kingdom Kidz Learning Center LLC with SOS #2977893 and was observed to be “Current-Active” on the NC Secretary of State website. As a reminder, if any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. I monitored the following items: supervision, staff/child ratio, licensed spaces, space capacity, staff and children’s records, health, safety, and program records. Inspections: The program's most recent fire inspection on file with DCDEE was completed on 9/29/25. This is due again on or before 9/29/26, requirements were discussed. The program's recent sanitation inspection was conducted on 11/18/25. The program received seven (7) demerits and received a superior classification. The last playground inspection was completed on 5/1/26. Indoor Learning Environment: I observed children participating in free play. The spaces used for childcare had several materials and learning centers, I gave suggestions on supplementing the areas with materials that align with your lesson plan/curriculum. During the visit, a tour also took place. Children’s Records: One (1) children’s file was reviewed, the health care needs section was left blank, requirements were discussed. If items do not apply, “none” or “N/A” is sufficient. Outdoor Learning Environment: The outdoor learning environment was monitored, suggestions were given to enhance the learning area such as adding books, art materials and music items in prop boxes. Program Records: The last fire drill was completed on 5/4/26 and the last emergency drill was completed on 3/2/26. Staff Records: There were no new staff files to review. I reviewed and verified the operator’s verification form with you. The operator’s statements of responsibility were completed during the visit. I reviewed the following items out of compliance and corrective steps: - Health and Safety Training – Admin of Medication - CBC letter on file for J. Nutrition: The facility was in compliance with child care meal pattern requirements. Transportation: Transportation is not provided. Transportation interested was shared, requirements were reviewed and will be sent to refer to via email. Medication: Medication is not administered. Weapons: It was reported that the facility was in compliance with child care requirements regarding firearms. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. I observed a deteriorated box in the outdoor learning environment with items inside. I also observed a cable with damaged prongs and warning labels. 15A NCAC 18A .2832(a) 1329 Application for enrollment did not include all required information. The health care needs section for one (1) file was not completed to it's entirety. .0801(a)(1-7) 1757 A valid qualification letter was not on file and available to review at the facility. The CBC letter for J.B was not on file. G.S. 110-90.2(b) & (d) & .2703(e) 1898 Staff did not complete the health and safety training within one year of employment. The training "Administration for Medication" was not on file. .1102(a) Corrective Action Plan – Compliance Letter: The violations that were not corrected during the visit must be corrected immediately. On or before 6/12/26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. A compliance letter template was shared with you please ensure that all these components are completed: • Name of your facility • Your facility ID number • Date the visit made • Date that you are submitting the compliance letter • Provide each cited violation number • Describe when and how each violation was corrected • Sign with your signature Please email the information to: abigail.avalos@dhhs.nc.gov Compliance History: The program’s compliance history was ninety-eight percent (98%) prior to today’s visit. Rated License Information: The QRIS rulemaking process has been completed, this means that hold harmless is no longer in place. We are actively planning for the implementation of the new QRIS requirements. Your facility is not due for re-assessment until 2028. If you are interested in applying, you can apply at any time. Lead in Water, Lead-Based Paint and Asbestos Testing: As of today, your facility has not completed all sections. Enrollment has been started for lead-based paint and asbestos. During the visit, I observed the lead in water testing, please connect with the Clean Classrooms for Carolina Kids team to upload these. Ms. Sheila Beasley, the Owner and Lead Teacher, was present and caring for children upon arrival. I stated the purpose of today's visit and began my walk-through of the indoor and outdoor learning spaces. Currently this center operates with a Temporary License, issued on June 1, 2025. The restrictions include daytime care only; 1st and 2nd Shift with a capacity of 15 for each shift. Age ranging from 0-12 years of age. The NC Secretary of State website was viewed before the visit and Kingdom Kidz Learning Center, LLC was current/active as of January 16, 2025. If any changes to the company need to be made or you decide to sell your business, then you must notify your consultant of at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. Indoor Learning Environment: There are six (6) children enrolled on first shift. Currently Ms. Beasley does not have any children enrolled for second shift. Out of the six (6) enrolled children, four (4) children were present. During the morning arrival children were engaged in planned teacher-directed table activity, Children later transition into a music activity and into free choice play throughout the learning environment. Upon arrival, the staff/child ratio was observed and was in compliance. I reviewed all program records requirements. The last fire drill was conducted on 9/2/25. The last playground inspection was completed on 9/01/25. The Sanitation inspection that took place on 6/12/25 with 13 demerits and a Superior Rating. I spoke with Ms. Beasley about contacting the Fire Inspector or the last person that conducted the Fire Inspection for the previous owner, Kevin Miller-kevin.i.miller@charlottenc.gov. A fire inspection is needed prior to the temporary license expiration date of 12/1/2025. We also spoke about the a current required Building Inspection prior to the temporary license expiring date of 12/1/2025. Ms. Beasley should contact the information below: - Building/Certificate of Occupancy – This inspection is scheduled through the internet, automated phone line or inspections. Visit http://webpermit.mecklenburgcountync.gov/webpermit/ for details. Non-receipt of the building inspection will require me to place the facility on a Provisional License Status until receipt. Please make sure you are adhering to deadlines to complete any requirement requested for an approved fire and building inspection. Outdoor Learning Environment: Was monitored and in compliance with Child Care Requirements. Records: No new staff have been hired since the last Temporary Time Period Visit. I monitored two (2) Children’s records during today’s visit. Records were organized and presented with all documents that are required to be on file. Nutrition: Your facility was in compliance during today’s visit with Child Care Meal Patterns Requirements. Bacon, egg, whole grain toast, strawberries and Milk. Weapons: Your facility reported that they were in compliance during today’s visit with Child Care Requirements regarding firearms. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The smoke detector presented a constant chirping between 30 to 60 seconds. This signifies a low-battery and may require a change. This can be a potential risk of this alarm system not working in the case of a real fire. 10A NCAC 09 .0601(a) The following violation were cited and corrected during today’s visit. Compliance History: North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Rated License: We discussed the two-component rated license process during today’s visit. The owner, Sheila Beasley, stated they will continue to remain in the harmless hold due to completing the Change of Ownership June 1, 2025, before it was no longer available. I explained to Ms. Beasley that I will keep the previous score and education from their last ERS assessment/Rated License. It is important to make sure staff are continuing to take EDU course work or work towards completion of an AA degree in Early Childhood. I suggest researching T.E.A.C.H and other community college resources to continue courses in early childhood education. I explained and discussed enhanced ratios and enhanced space with you. I explained that if you receive a four or five-star license, then you will be required to have an approved curriculum for children four years of age. As discussed, at the end of your temporary license period you must be at least a three (3) star facility in order to continue to provide care for children who receive subsidy. You must have earned enough points for 3-5 stars to continue receiving subsidy dollars. To help childcare providers meet the new training requirements, the Division of Child Development and Early Education has Moodle Trainings set up and can be accessed through the Divisions website: ncchildcare.ncdhhs.gov. General Reminders (in compliance today): You are currently meeting minimum ratio requirements. The following staff/child ratios and maximum group sizes apply to your facility: 0 to 12 Months ~ 1 staff / 5 children ~ 10 group 1 to 2 Years ~ 1 staff / 6 children ~ 12 group 2 to 3 Years ~ 1 staff / 10 children ~ 20 group 3 to 4 -Years ~ 1 staff / 15 children ~ 25 group 4 to 5 Years ~ 1 staff /20 children ~ 25 group 5 Years and Older ~ 1 staff / 25 children ~ 25 group Technical Assistance: Please make sure you have activities such as books, tinker toys, cause and effect toys for the area near the kitchen when preparing meals. This can help keep children occupied while preparing plates for mealtimes. Due to the structure of the home, supervision may pose a concern if in the kitchen area and children are left in the play area. We discussed making the adjacent area child friendly and keeping items that are considered a possible violation out of reach of children. I also discussed changing the battery in the alarm system or having the fire inspection offer an alternative solution in removing the beeping sound. You, Sheila Beasley, Operator, assisted me with the visit today. Ms. Beasley had two (2) thirteen-year-old volunteers today. I spoke to Ms. Beasley about their responsibilities and that they cannot care for the children without her being present. During the observation, I observed five (5) preschool age children; two (2) one-year olds, one (1) three-year-old, one (1) four-year-old and one (1) five-year-old present. Ms. Beasley stated she has a total of eight (8) children enrolled in the first shift only. Currently she does not have any children enrolled in the second shift. I used the Center Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. During the visit, I monitored the indoor and outdoor environment, staff and children's records, outdoor inspection checklist, fire drill reports and required posted items. The most recent fire drill and playground inspections were conducted on 6/3/25. You last completed CPR on 1/7/2025, and will expire on 1/7/27 and First Aid, 1/7/25 and will expire on 1/7/2027. The SIDS certification was received on 2/08/25 and will expire on 2/08/28. Indoor Learning Environment: The program has designated rooms in front of the home. The rooms have minimally developmentally appropriate materials for all ages. Lunch consisted of chicken nuggets, mac and cheese, spinach, apples slices, wheat roll, and milk. Ms. Beasley is currently working with the food program to help her with her menus and meal reimbursements. Weapons: Your facility is in compliance with Child Care Requirements regarding firearms. Records: Three (3) children’s records were reviewed during today's visit. All required paperwork was kept in an orderly fashion in a binder. No violations were observed concerning children’s records during today’s visit. Violation Number Comment Rule 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic covering brand new puzzles were observed accessible to children under the age of three in the learning environment. .0604(q) Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to administrative action. License Status: Currently, you hold a temporary child care license. Restrictions on your license include daytime care only. The capacity for the program is 15 for the first shift and 15 for second shift. Your temporary license will expire on December 1, 2025. Rated License: We discussed the two-component rated license process. You stated that you are interested in getting the FCCERS assessment completed. During today’s visit, I worked on getting you acclimated with WORKS and how to upload an official transcript for your education to be evaluated. Ms. Beasley stated she has EDU 119 and an Admin Course. I explained that I do not have the authority to evaluate her education but stressed the importance of sending all recommended paperwork to NC WORKS for a full evaluation. As discussed, at the end of your temporary license period you must be at least a three (3) star facility in order to continue to provide care for children who receive subsidy. General Reminders (in compliance today): 1. Although you prepared your child care room in a designated space in your home, for child care use, I explained to you that your entire home and premises are licensed; therefore, a walk-through of the entire home may be conducted to ensure the safety of children and compliance with the laws and requirements. I did not walk through your entire home during today’s visit. 2. You must make sure you are maintaining ratios at all times. 3. Children must be visually supervised while always awake and able to be heard while at rest. You may never leave your home to go outside or behind closed doors from the children. 4. Please make sure you read over the Child Care Law/Rules to familiarize yourself with all rules and regulations. Please visit https://ncchildcare.ncdhhs.gov/. One violation cited and corrected during today’s visit. Technical Assistance: I spoke with Ms. Beasley about what her two (2) young volunteers can contribute while helping at the facility. They can assist in organization and cleaning (removing all plastic from new toys brought in to add to the learning environment). They can engage in play but make sure they are speaking and engaging appropriately with children. They can assist in plating food and cleaning up before and after meals. They should not be the primary caregivers during the day. I also spoke to Ms. Beasley about a possible hire at the facility. I strongly recommend Ms. Beasley find someone with a Child Development background. This will help support her understanding of appropriate age activities for the variety of children she is caring for throughout the day. I spoke with Ms. Beasley about adding additional materials in the block area (cardboard blocks, plastic blocks and cloth blocks, small vehicles, and figurines). Additional manipulatives, puppets, felt board with felt board activities, different types of art supplies (construction paper, plain paper, crayons, markers, colored pencils, glue, paint, paint brushes, playdoh, playdoh shape cutters, safe scissors and paint dotters.
