Today’s visit was conducted with Ms. R. a teacher and assistant when the director is out. I greeted her, announced the purpose of my visit, and proceeded with the walk-through partially assisted. Ms. R stated that Mrs. Tonya was out for the day.. This facility currently operates with a four (5) star rated license effective October 2025. Facility is still accredited by the National Accreditation Commission and is a Five Star under Pathway 3. Later during the visit Tonya Reid, administrator arrived to confirm that it was a routine unannounced and that I, the consultant did not need anything. I stated to Mrs. Reid that Ms. R and the staff had everything together and that Ms. R. new where everything was located and I have received all that was needed. Children aged one to four years of age were observed engaged in quite play, infants were observed napping, engaged in play, and tummy time. The school aged children were cleaning up from center play and the children that were finished cleaning up was engaged in an activity. The following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure. There were no new staff hired since the last visit. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. Children were feed chicken nuggets, green beans, pears, milk and bread. The posted menus stated something different. 10A NCAC 09 .0901(b) Violations: All violations were corrected on site. Any Violations observed today were discussed with you and documented on the visit summary and left with you at the conclusion of this visit. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be incompliance with all the applicable laws and rules. Today’s visit was conducted with you, Tonya Reid. This facility currently operates with a four (5) star rated license effective October 2025. Facility is still accredited by the National Accreditation Commission and is a Five Star under Pathway 3. Prior to the visit the NC Secretary of State website was reviewed, and this facility was in current and in compliance. Children were observed engaged in indoor activity centers, transitioning to eat, nap, and quietly playing or resting on their cots. Children were served flatbread cheese pizza, salad, craisins, and milk. INSPECTIONS: The last sanitation inspection was conducted 1-07-26. The last fire inspection was conducted 4-22-25. The last documented fire drill was conducted 04-01-26. The last documented shelter-in-place or lockdown drill was conducted 3-3-26. The last documented playground inspection was conducted 4-01-26. Last Compliance prior to this visit was 90%. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program, and management. Transportation was also monitored and all requirements including insurance and registration was found in compliance. The back playground with the large climbing structure was not monitored today as Mrs. Reid stated the playground is not being used by staff or children and is closed. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. Violation Number Comment Rule 707 Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. A shelf in space 4 had peeling plastic edges and several pieces was missing and the plywood is exposed. .0601(d) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. There was a diaper cream that expired on 2-2025. .0803(12) 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. Today’s visit was conducted with you, Tonya Reid. This facility currently operates with a four (4) star rated license effective September 10, 2021. Prior to the visit the NC Secretary of State website was reviewed, and this facility was in current and in compliance. Children aged one to four years of age were observed engaged in indoor activity centers, and transitioning to and from lunch to nap. Infants were observed napping, engaged in active play with books and play mat. Children were severed barbeque chicken, corn, peas, bread roll, and milk. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. The facility has hired four new staff. INSPECTIONS: The last sanitation inspection was conducted 1/21/25. The last fire inspection was conducted 4/15/25. The last documented fire drill was conducted 04-02-25. The last documented shelter-in-place or lockdown drill was conducted 3-04-25. The last documented playground inspection was conducted 4-02-25. Last Compliance prior to this visit was 94%. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program, and management. Transportation was also monitored and all requirements including insurance and registration was found in compliance. The back playground with the large climbing structure was not monitored today as Mrs. Reid stated the playground is not being used by staff or children and is closed. The facility had a work order in place for several violations that were found and maintenance arrived during the visit to repair the items. Rated Licensed: Facility initially wanted to apply for their Environmental rating scales in 2023. Since the new options to include accreditation has been created, the facility has obtained a certification in the National Accreditation Commission. Mrs. Reid, gave me the application today. After reviewing the application and accreditation information the facility should remain as a four star facility. Consultant will contact you after completion of the staff education and entire Rating re-application with the final star points. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. One water cup in space three did not have the appropriate name or date. 15A NCAC 18A .2804(d) 612 Each child's bed, cot, or 2" mat was not individually assigned and identified. Cots were individually labeled with name or a number that corresponded to a sleep chart, however several children were given the incorrect cot. 15A NCAC 18A .2821(b) & (c) 705 Equipment and furnishings were not sturdy, stable and free of hazards. Two riding toys had plastic that was chipped with ragged edges. .0601(c) 707 Equipment and furnishings not meeting the requirements outlined in child care .0601(b) and (c) were not removed or made inaccessible. The playground had three broken trucks and several riding toys that were damaged. In Space three there were two toilets that were broken and one had a broken handle. .0601(d) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Space one had clear tape over the outlet covers, Space three Air conditioner was not placed in the electrical outlet. 10A NCAC 09 .0604(c) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. There was cream that belonged to a child that withdrew from the facility, paperwork was also expired. Technical assistance: For the classroom that had tape covering the electrical outlet, I suggest getting the complete outlet cover that covers the air conditioner outlet similar to the threes though school-ager classrooms or one that will be harder for children to remove from the outlets. Do a search for types of outlet covers. As outlined in Session Law 2021-127 - Senate Bill (SB); facilities who have been unable to hire/retain staff with the same educational levels in the current star rating are in a hold harmless state. The facility had a work order in place for several violations that were found and maintenance arrived during the visit to repair the items. Rated Licensed: Facility initially wanted to apply for their Environmental rating scales in 2023. Since the new options to include accreditation has been created, the facility has obtained a certification in the National Accreditation Commission. Mrs. Reid, gave me the application today. After reviewing the application and accreditation information the facility should remain as a four star facility. Consultant will contact you after completion of the staff education and entire Rating re-application with the final star points. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Today’s visit was conducted with you, Tonya Reid. This facility currently operates with a four (4) star rated license effective September 10, 2021. Prior to the visit the NC Secretary of State website was reviewed, and this facility was in current and in compliance. Children aged one to four years of age were observed engaged in indoor activity centers, infants were observed napping and two were engaged in play. Toddlers were observed participating in gross motor play. Children were served Grilled chicken bites, green beans, pears, bread; and milk. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. INSPECTIONS: The last sanitation inspection was conducted .1-11-24 The last fire inspection was conducted 4-9-24. The last documented fire drill was conducted 04-04-24. The last documented shelter-in-place or lockdown drill was conducted 3-06-24. The last documented playground inspection was conducted 4-02-24. Last Compliance prior to this visit was 96%. I monitored the following items today: Supervision, Staff/child ratio, health, safety, program, and management. Transportation was also monitored and all requirements including insurance and registration was found in compliance. The back playground with the large climbing structure was not monitored today as Mrs. Reid stated the playground is not being used by staff or children and is closed.
