The center currently has a Five-Star rated license. The center license and Summary of the Law was prominently posted. I was greeted by T. McCrory, Administrator. I completed the walkthrough of the center alone and Mr. McCrory assisted me with the rest of the visit. Permit information-the program operates with a Five- Star license, issued February 14, 2020. The center has six (6) points for education, seven (7) points for program standards and one (1) quality point. License restriction include: First Shift; Meets enhanced ratios; Meets enhanced space. The program is still located at: 4262 Freedom Drive, Charlotte, NC 28208. The phone number is (704) 392-8850. The center had thirteen (13) children present during today’s visit and fifteen (15) children enrolled. The following items were monitored: Supervision, S/C Ratio, Storage of Hazardous Substances, Storage of Medication, Adequate Approved Space, Program Records, License Posted and Permit Restrictions. The center does not provide transportation. The center has not hired any new staff since their last visit. The center’s last fire drill was documented on 4/28/25. The center’s last lockdown drill was on 5/5/25. The last sanitation inspection was completed on 2/3/25 with zero (0) demerits, receiving a superior rating. The center playground inspections were monitored and in compliance. There was one (1) violation cited during todays visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. Some children enrolled were did not have documentation of daily departure times from the center. 10A NCAC 09 .0302(d)(4) Violations must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 5/29/25, stating how each violation was corrected and how compliance will be maintained in the future include the violation number, statement of compliance, license ID and your signature in the letter or e-mail. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. The center operates with a Five-Star license and an 18-month compliance history score of 95%, effective February 14, 2020. The restrictions include: Daytime care only; Meets enhanced ratios; Meets enhanced space. The Secretary of State website was checked and the status of LITTLE EAGLES, INC was current and active. Upon arrival, I was greeted by K. Branch, staff member. I discussed with Ms. Branch the Annual Compliance Monitoring guidelines. Director, N. McCrory informed me that she would be in late to the center today and that L. Owens, staff member, would be able to assist me with the visit. I completed the walkthrough of the center alone and Ms. Owens soon arrived and assisted me with the rest of the visit. The following items were posted within the center/ classrooms: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/ departure procedures, summary of law and menus. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. Children were observed engaged in nurturing interactions with caregivers, teacher directed activities and multi-play opportunities. There were fifteen (15) children present today. There are sixteen (16) children enrolled in the center. Outdoor Learning Environment: The outdoor learning environment was monitored and not in compliance. Inspections: The center was closed from 6/7/24-8/30/24 for summer break. The outdoor inspections and fire drills were monitored. The last fire drill was conducted on 6/27/2024. The centers last documented lockdown was conducted on 5/21/24. The last monthly playground inspection was conducted on 8/2/2024. The center last had a sanitation inspection on 9/16/2024. The center received 3 demerits and received superior classification. The center last had a fire inspection on 7/8/2024. Transportation: The center does not provide transportation. Nutrition: Meals met nutritional value during today’s visit. Files: Children records and staff files were monitored today and in compliance. Staff files were monitored today and in compliance. There were three (3) violations cited during today's visit. Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. The wall panels on the OLE have three holes in them. 15A NCAC 18A .2825(a) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Today I observed the following on the OLE: a rusted metal crate, table and two chairs not in good repair, toy containers not in good repair, muddy/ dirty foam mats and rugs (trash). 10A NCAC 09 .0604(p) 1757 A valid qualification letter was not on file and available to review at the facility. One (1) therapist that was present during today's visit, was alone with a student doing therapy. The provider did not have a qualification letter on file. The center operates with a five-star license. The restrictions include: Daytime care only; Meets enhanced ratios; Meets enhanced space. The centers 18-month compliance history is 96%. The center’s license and NC child care law were prominently posted. The NC Secretary of State website was viewed before the visit and LITTLE EAGLES, INC was current and active. I used the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to this facility. In addition to monitoring for health and safety, I reviewed center files and required postings. I observed staff to child and child to child interactions. I was greeted by K. Branch, staff member. Ms. Branch notified Ms. McCrory (via telephone) that I was present. Ms. McCrory was not present during today’s visit, Latoya Owens, staff member, assisted me with today’s visit. The following items were posted within the center/ classrooms: Daily schedules, activity plans, staff/child ratio worksheets, emergency numbers, emergency procedures, arrival/ departure procedures, summary of law and menus. Indoor Learning Environment: Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. Children were observed engaged in multi-play opportunities. All teachers were observed in nurturing and engaging interactions with children. There were fourteen (14) children present today and there are fifteen (15) children enrolled. Outdoor Learning Environment: The center’s outdoor learning environment was monitored today. Inspections: The outdoor inspections, fire drills, shelter-in place/ lock-down drills were monitored. The documents were current and up to date. The last fire drill was conducted on 6/9/23.The center was closed for the summer and reopened again on August 30, 2023. The last monthly playground inspection was conducted on 8/31/2023. The center last had a sanitation inspection on 8/31/2023. The center received 4 demerits and received superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 6/21/2023. Transportation: The center does not provide transportation. Nutrition: Meals met nutritional value during today’s visit. Files: Staff files and children records were monitored during today's visit and in compliance. There were two (2) violations cited during today's visit. Violation Number Comment Rule 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. During the walkthrough of the outdoor learning environment, I observed 2 storage bins that were full of molded rotted toys at the bottom of the stairs, along with 1 black storage bin full of water. 10A NCAC 09 .0604(p) 9995 A violation was found for which there is no item number. During the walkthrough of the center, I noticed that there was an unlocked door in space #1 near the cozy corner, that leads to space #2. Space #2 leads to the kitchen, and during today's visit, the kitchen door was wide open/ not locked/ accessible to chidlren. Violations must be corrected immediately.
