The last Annual Compliance visit was made on 7/8/2025. The current 5 star license was issued on 6/4/2019. The April 2025 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 100% as of today. Administrator, Ms. Prince accompanied me on the walk through. Program records were made available to me. Four (4) classrooms were monitored on today for: Sign in/out, allergy list, menu, schedule, activity plan, and staff/child ratio chart. Supervision, and storage of hazardous products was monitored. Children were observed having indoor free play, teacher directed activities, and having routine needs met. There were toys available. The following was observed posted: current license, Summary of Law, Safe Arrival and Departure procedures, Menu, Emergency Care Plan, and no smoking signage. A Sanitation inspection was conducted on 6/2/2026. A Fire inspection was conducted on 3/3/2026. Monthly Fire Drills, Playground Inspections, and EPR Drills were monitored. A sample of staff files including two (2) new staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. A sample of children's files were monitored for Application, Emergency Care authorization, Summary of Law, Medical/Immunization, Discipline Statement, No smoking statement, Safe Sleep Policy, Infant Feeding Schedule and Shaken Baby Syndrome Policy (if applicable). Transportation and off premise requirements were monitored. The following violations were cited during today's visit: Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. A three (3) year old was grouped in children between 12 and 24 months of age in space four (4). 10A NCAC 09 .0713(a)(6) 1110 Vehicles used to transport children enrolled in the child care center did not comply with all applicable State and federal laws and regulations. The vehicle used to transport children enrolled in the child care center's current insurance information was not available. .1002(b) 1876 A child was yelled at, shamed, humiliated, frightened, threatened or bullied. A child was yelled at "I said stop", and told that they "would sit in time out until lunch". .1803(a)(9) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 14, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. Because a grouping and discipline violation was cited, an unannounced follow up will me made in the future. We discussed more appropriate ways of interacting with children with challenging behaviors such as discussing the behavior with the parent and possibly getting evaluated by a therapist. The three (3) year old child can be grouped with the preschool four and five year old class if staff child ratio and classroom capacity permits. CCRI's behavior trainings and technical assistance offerings will be shared via email. Let me know if I can be of further assistance. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000). The last Annual Compliance visit was made on 08/01/23. The current 5 star license was issued on 6/4/2019. The March 2024 version of Child Care Center Master Item Number Listing, DCD-0357 and a checklist was used to complete monitoring today. This facility has a compliance history of 95% as of today. I was let into the facility by the director, Ms. Prince. I told her the reason for my visit. She accompanied me on the walk through of the facility. Facility files were made accessible to me. Four (4) classrooms were monitored on today for: Sign in/out, allergy list, medical action plans (if applicable), menu, schedule, activity plan, and staff/child ratio chart. Storage of medication and permission to administer was monitored. Supervision, and storage of hazardous products was monitored. Children were observed having indoor and outdoor free play, teacher directed activities, and having routine needs met. There were ample toys and activities available. The following was observed posted: current license, Summary of Law, Safe Arrival and Departure procedures, Menu, Emergency Care Plan, and no smoking signage. A Sanitation inspection was conducted on 06/12/2024 . A Fire inspection was conducted on 4/2/2024. Monthly Fire Drills, Playground Inspections, and EPR Drills were monitored. A sample of staff files were monitored today for Criminal Qualifying Letters, Medical Statement, TB Test, Health Questionnaire/Emergency Information, Annual on-going Training Hours, and Special Trainings. A sample of children's files were monitored for Application, Emergency Care authorization, Summary of Law, Medical/Immunization, Discipline Statement, No smoking statement, Safe Sleep Policy, Infant Feeding Schedule and Shaken Baby Syndrome Policy (if applicable). This facility does not provide transportation. The following violations were cited during today's visit: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One (1) child care provider's medical report was older than 12 months. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One (1) staff person's TB test or screening was older than 12 months. .0701(a) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Two (2) staff did not complete an annual health questionnaire on or before the first day of work. .0701(a) Compliance -Violations cited today must be corrected immediately. Please send me a letter documenting how violations cited today were corrected by July 30, 2024. Failure to maintain compliance with all applicable requirements may result in an Administrative Action against your license, which could include civil penalties. A walk through of the facility was completed. Three (3) classrooms were monitored on today for: Sign in/out, allergy list, medical action plans (if applicable), menu, schedule, activity plan, and staff/child ratio chart. Children were observed having indoor free play and having routine needs met. There were ample toys and activities available.
