Upon arrival I was greeted by the receptionist and stated the purpose of my visit. After I signed in and received my visitor’s sticker, I walked to the afterschool program. I greeted Ms. B. group leader and the teachers and restated the purpose of my visit. Ms. B accompanied during the visit, as the program coordinator was out today. This facility currently operates with a five (5) star rated license effective June 6, 2022. Ownership information was confirmed as, CHARLOTTEMECKLENBURG BOARD OF EDUCATION, a Department of Public Instruction program. All contact information was reviewed, and it was reported that no changes had been made. The following spaces were observed: cafeteria, black top, gym. Children were observed transitioning from school into the cafeteria for afterschool and snack. Snack consisted of cheese it crackers, cheese stick, milk and vegetable juice blend . Children had access to gross motor activities and were provided several types of sports balls as well as other portable materials. Indoors children were actively engaged in activity center play which consisted of fine motor toys, art, writing, books, dramatic play/social studies/world culture, books, science; and math. The children had a variety of choices indoors as well as the outdoor environment. INSPECTIONS: The last documented fire drill was conducted 0 2 -16 -2026 The last documented shelter-in-place or lockdown drill was conducted 02-16-2026. The last documented playground inspection was conducted 02 -16- 2026 . The last Annual Compliance was conducted 03/ 11/2025. Compliance History prior to today’s visit is 97%. The attached checklist was used to note the requirements we monitored today. Staff and children’s records were reviewed today. The required public school off-site verification forms for children’s records, Staff records, and transportation were reviewed and in compliance. I also monitored the following items today: Supervision, Staff/child ratio, health, transportation, safety, program, and management. During this visit, the consultant could not get into regulatory. An official visit summary will be emailed to you. Violation Number Comment Rule 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. Staff that started in October 2024 did not have this training completed until February 2026. .1103(b) If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. I walked to the cafeteria where sht school aged program was located and wax greeted Ms. M. and Ms. P. the group leaders. I stated the purpose of my visit, and was informed that Mrs. Blount, site administrator, was in another part of the building. I proceeded with my monitoring visit unassisted, Ms. M. assisted me with questions and locating the needed documentations. Children were observed playing in the activity areas, there was a location set up with pillows for cozy, several art stations, manipulative table with legos, and a table set up with card games/flash cards. Children were observed transitioning to bathrooms to outside play and actively participating in gross motor play. Staff had hula hoops and balls available for portable equipment. The following items were monitored: supervision, staff/child ratio, CPR/first aid, IT-SIDS, CBC qualification, Emergency Medical Care Plan, administration of medication, storage of hazardous products, storage of medication, general safety, discipline, adequate/approved space, program records, license posted, permit restrictions, safe arrival and departure were found in compliance. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance with the violations cited today was provided. REQUIRED RESPONSE: You shall correct the violation(s) immediately. Send two (2) signed corrections letters to the address below or via email by November 1, 2023: The format to follow is: -Name of your facility, -ID number, -date of the visit, -date you are submitting the letter, -citing each violation number, -when AND how it was corrected AND how you will prevent the reoccurrence of this violation, -Close with YOUR SIGNATURE REQUIRED RESPONSE: If it is stated in your correction letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by this date, you must contact me with a proposed timeline of the corrections. In some cases, the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited today may negatively affect this score. Comments: It is important for licensed providers to be knowledgeable of all the licensing requirements that apply to their type of program. Child care consultants can make unannounced visits at any time. It is the provider's responsibility to be in compliance with all the applicable laws and rules.
