I was greeted by Ms. Kongit Tesfazghi, and I explained the purpose of the visit. No one else was present during the visit. The facility had a Four-Star License issued July 31, 2018, and has an eighteen-month compliance history score of one-hundred (100) percent prior to today’s visit. The Annual Compliance visit was completed on November 9, 2023. There were no children present today. The following child care requirements were monitored and observed today using the Family Child Care Home Checklist and the Master Family Child Care Home Item Number Listing: Supervision: Not applicable. Staff/Child Ratio: No applicable. CPR: Ms. Tesfazghi was current with CPR. First Aid: Ms. Tesfazghi was current First Aid. Special Training: Ms. Tesfazghi continues to complete ongoing in-service training. CBC Qualification: Ms. Tesfazghi has a current criminal background check (CBC) letter on file. ITS-SIDS: Ms. Tesfazghi is current with ITS-SIDS training. No infants are enrolled. The infant safe sleep policy and safe sleep poster were posted. Emergency Medical Care Plan: The Emergency Care Plan was posted. Administration of Medication: There are no children with medication, and there are no children with allergies. Storage of Hazardous Substances: I did not observe and hazardous substances accessible to children. Storage of Medication: No medication was stored. General Safety: There are no safety concerns. Discipline: Not applicable. Adequate/Approved Space: Not applicable. Program Records: I reviewed all information required to be posted. The monthly fire drills were complete and up to date and compliance was met. The quarterly drills (shelter-in-place/lock drill) were not up to date, and a violation was cited. The playground inspections up-to-date, and a violation was cited. License Posted: The license was observed posted. Permit Restrictions: Not applicable. The last Sanitation Inspection was completed January 30, 2024 with a demerit score of six (6). One (1) violation was cited. Violation Number Comment Rule 1854 The operator did not conduct a quarterly lockdown or shelter-in-place drill and or the drill record was incomplete. The shelter-in-place drill/lockdown drills were not completed at least every three (3) months; completed May 2, 2023, August 1, 2023, November 6, 2023, February 5, 2024, and June 4, 2024. .1719(a )(16) & .1721(e )(7) An unannounced/follow-up visit will be completed to monitor when children are present. Ms. Tesfazghi stated that the child that attends during the morning hours was absent today, due to a scheduled private appointment. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before June 27, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. The following Technical Assistance (TA) was provided: Family child care home operators must conduct a shelter-in-place or lockdown drill, as defined in 10A NCAC 09 .0102, at least every three (3) months. Refer to Child Care Rule .1719(a)(16). They must document shelter-in-place and lockdown drills. The record must include the date of each drill, time of day, the length of time to reach the designated location and the signature of the person conducting the drill. Refer to Child Care Rule .1721(e)(7). At the completion of the visit, this visit summary was reviewed, and a copy was provided to you.
