The center currently has a Five-Star rated license. The center license and Summary of the Law was prominently posted. I was greeted by T. Gary, Owner/Director. I informed Mrs. Gary the reason for my visit. I conducted the walkthough of the center alone and Mrs. Gary assisted me with the visit. Permit information-the program operates with a Five- Star license, issued March 25, 2019 . The center has seven (7) points for education, six (6) points for program standards and one (1) quality point. License restriction include: Daytime care only; No cooking allowed in building; Meets enhanced ratios; Meets enhanced space. The program is still located at: 2917 Sloan Drive, Charlotte, NC 28208. The phone number is: (704) 394-8095. The center had sixteen (16) children present during today’s visit and twenty-four (24) children enrolled. The following items were monitored: Supervision, S/C Ratio, Storage of Hazardous Substances, Storage of Medication, Adequate Approved Space, Program Records, License Posted and Permit Restrictions. The center has not hired any new staff since their last visit. The center’s last fire drill was documented on 3/6/25. The center’s last lockdown drill was on 1/6/25. The last sanitation inspection was completed on 12/30/24 with four (4) demerits, receiving a superior rating. The center playground inspections were monitored and in compliance. There was one (1) violation cited during todays visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. Departure times for children were not available for review. 10A NCAC 09 .0302(d)(4) Violations must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 4/8/25, stating how each violation was corrected and how compliance will be maintained in the future include the violation number, statement of compliance, license ID and your signature in the letter or e-mail. Failure to correct the violations and send the written statement by the due date listed above may result in an unannounced follow-up visit being conducted or an administrative action may be recommended. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Upon arrival I was greeted by Ms. T. Gray. I shared the reason for the visit, the children were getting off the bus from school and once they were taken care of Ms. Gray assisted me with today’s visit. The lead teacher in the classroom was absent. Your program currently operates with a Five (5) Star License. The license was posted, with restrictions to: first (1st) shift care, meets enhanced ratios, meets enhanced space and no cooking allowed. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six (96%) percent as of today’s visit. Ownership: The facility owner is Humpty Dumpty Child Care Company Inc. which is current and active. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The following items above were found in/out of compliance. The following inspections are below. •Last fire drill was conducted on 4/5/24. •The fire inspection was completed on 3/6/2024. A copy was provided to the Consultant. •Last Shelter in Place/Lockdown Drill was conducted 4/15/24. •Last Playground Inspection was conducted on 4/5/24. •The incident report and report log was up to date and completed. •The Sanitation Inspection was completed on 2/20/23 with a Superior Classification. The children were observed arriving at After School, with limited outdoor time as there was evidence of a thunderstorm. Upon arrival the children walked to their classroom, participated in self-care routines such as hand-washing. The children had snack which consisted of cheese and crackers with apple juice. The menu was not posted in the classroom but a digital copy was provided. As a reminder, the menu has to be posted and correct until the meal is completed. The outdoor learning environment was monitored and was in compliance. A reminder was offered to rake the mulch daily, especially around critical height structures. A new fence is being built and meets all requirements. Staff Records: The staff-training worksheets were completed before the visit. There are no new staff files to monitor. Medication: There is no emergency medication used and there is no medication on site so this was not monitored. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. One (1) violation was observed today and it was corrected during the visit. The menu was not posted in a location in the After School room, the menu was in compliance and easily accessible just not posted. 10A NCAC 09 .0901(b) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Family child care homes are required to test water for lead once. o DCDEE encourages you to utilize the Clean Classrooms for Children’s program. You can take the pre-enrollment webinars which is available at www.cleanwaterforUSkids.org/carolina then enroll – At www.cleanwaterforUSkids.org/carolina completing the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos. Keep mindful that once you evaluate potential hazards and receive results if there are recommendations and/or water mitigation support is needed they will provide direction in how to mitigate and ways to fund said mitigation. We jointly reviewed the visit summary and documentation of my findings and violations with you, I encouraged you to ask any questions and you stated that you had none. This center has a five-star license and an 18-month compliance history score of 91%. The center’s license and NC child care law were prominently posted. I arrived at the center at 3:45pm and contacted T. Gary (Owner/Director) via cell phone. Ms. Gary informed me that she was not present at the center today and that Ms. T. Watson would be the caregiver for the school age program. Ms. Watson arrived at the center at 4:30pm with the children. Upon her arrival, I discussed the Annual Compliance Monitoring guidelines with her. Staff/child ratio, supervision, capacity, group size and space were observed in compliance today. The center’s Emergency Drill Log and Report was reviewed. Fire drills were documented monthly as required. Shelter-in-place and Lock-down drills were reviewed and in compliance. The playground safety inspections were reviewed and documented. The center last had a sanitation inspection on 6/29/23. The center received 0 demerits and received superior classification. The inspection was completed by the Mecklenburg County Health Department. The center last had a fire inspection on 5/4/23. There were five (5) children in attendance today. Children were observed upon arrival transitions and indoor free play. The center does provide transportation, the vehicle used to transport children has been monitored and in compliance. The playground was not monitored today due to inclement weather and will be monitored during the centers next visit. Three (3) violations were cited today. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook.
